What Does a Cavity Look Like? From White Spots to Holes

A cavity doesn’t always look like an obvious hole in your tooth. In its earliest stage, it appears as a chalky white spot on the enamel surface, often so subtle you’d miss it without looking closely. As decay progresses, it darkens to brown or black and eventually breaks through the enamel to form a visible pit or hole. Knowing what each stage looks like can help you catch decay before it becomes painful or expensive to treat.

The Earliest Sign: White Spots

Before a cavity becomes a cavity, the enamel starts losing minerals in a process called demineralization. This shows up as a small, matte, opaque white patch on the tooth surface. Dentists sometimes call these “white spot lesions,” and they have a distinctly chalky look compared to the natural gloss of healthy enamel. The spot appears because the weakened enamel scatters light differently than the surrounding intact surface.

These white spots are actually more visible when the tooth is dry. Saliva partially masks them by filling in microscopic pores in the damaged enamel. If you’ve ever noticed a faint white patch on a tooth after brushing that seems to fade once your mouth gets wet again, that could be early demineralization. At this stage, the surface is still intact with no hole, and the process can sometimes be reversed with fluoride and good oral hygiene.

Brown and Dark Spots

As decay continues, those white spots pick up pigment and shift to yellow, light brown, or dark brown. If you run your tongue over the area, it may feel slightly rough compared to the smooth enamel around it. A brown or dark spot concentrated in a single area of the tooth, particularly in the grooves of a back molar or near the gumline, is a strong indicator of active decay rather than a harmless stain.

This is where people often confuse cavities with coffee or tea stains. A key difference: staining from food and drinks tends to affect the whole tooth or multiple teeth evenly, not a single isolated spot. Stains also tend to change over time with whitening or cleaning, while a cavity spot stays put or gets darker. If you see a localized black, brown, or gray spot that doesn’t brush away, that’s more consistent with decay than surface discoloration.

Visible Holes and Pitting

Once enough enamel has weakened, the outer surface collapses inward and creates an actual break or hole in the tooth. This is what most people picture when they think of a cavity. It can range from a tiny pit you can barely see to a large, obvious crater. On the chewing surfaces of molars, cavities often start in the natural grooves and fissures where food and bacteria collect, so they can look like the groove has become unnaturally wide or deep.

At this point, the hole may expose the softer layer beneath the enamel called dentin. Dentin is naturally yellowish, so you might notice a yellow or brownish color inside the pit. If the decay is active, the inside of the cavity feels soft or sticky rather than hard. Some cavities look dark brown or black inside because the decayed material discolors as it breaks down. If more than half the tooth surface is involved, the structure can start crumbling or chipping away in pieces.

Dark Shadows Through the Enamel

Not all cavities are visible on the surface. When decay reaches the dentin layer but the enamel above hasn’t fully collapsed yet, you may notice a dark gray or bluish shadow showing through the tooth. This is especially common on the biting surfaces of back teeth, where the enamel can look mostly intact from the outside while significant decay spreads underneath.

Cavities that form between teeth are particularly hard to spot. Because the contact point between two teeth hides the decay, the first visual clue is often a dark shadow along the edge of the tooth where it meets its neighbor. You might also notice brown, yellow, or black discoloration right at the contact area. These interproximal cavities are a major reason dentists take X-rays, since the decay is frequently invisible to the naked eye until it’s fairly advanced.

Decay Along the Gumline

Root surface cavities look different from cavities on the chewing surfaces. They form at or just below the gumline, in areas where the gum has receded enough to expose the root. Unlike the hard enamel that covers the crown of the tooth, root surfaces are covered by a much thinner, softer material, so decay can progress faster here.

Early root decay shows up as a slight color change to light brown or yellowish, and the surface feels soft when pressed. If the decay stops progressing (sometimes called an arrested lesion), it hardens and turns dark brown or black with a smooth, shiny surface. Active root cavities stay soft and are lighter in color. These are more common in older adults, whose gums have receded over years, exposing vulnerable root surfaces to bacteria.

Decay Around Existing Fillings

If you already have fillings, new decay can develop at the margins where the filling meets the natural tooth. Visually, this looks like a gap or dark line forming around the edge of the filling. The edge of the filling may appear to be pulling away from the tooth, or you might see brown or black discoloration creeping along the border.

Research on amalgam (silver) fillings found that wider gaps at the filling margin are directly associated with recurrent decay beneath the restoration. A gap you can catch a fingernail on or see darkening around is worth having checked. With tooth-colored fillings, the discoloration at the margins can be harder to spot, but look for any new brown or gray shadowing where the filling meets the tooth.

How to Tell if a Spot Is a Cavity

A few practical rules can help you sort out whether what you’re seeing is decay or something harmless:

  • Location matters. Spots in the grooves of molars, between teeth, or along the gumline are more suspicious than general discoloration across the whole tooth.
  • Texture matters. Healthy enamel feels glassy and smooth. Demineralized enamel feels rough or chalky when you run your tongue over it.
  • Persistence matters. A stain that fades with brushing or whitening is surface discoloration. A dark spot that stays or grows is more likely decay.
  • Holes are definitive. Stains don’t create pits or holes. If you can feel a physical dip or catch in the tooth surface, that’s structural breakdown.

Keep in mind that many cavities, especially between teeth or under existing dental work, are invisible without X-rays. What you can see in the mirror represents only the cavities on exposed, visible surfaces. Pain, sensitivity to hot or cold, or a sharp ache when biting down can signal decay in areas you can’t visually inspect.
