What Does a Drug Test Look Like: Urine, Swab & Hair

Most drug tests you’ll encounter are plastic cup or cassette devices with small result windows, colored lines, and built-in verification features. The exact setup depends on whether you’re giving urine, saliva, or hair, but the experience is less clinical than most people expect. Here’s what each type looks like and what happens during the process.

The Urine Test Cup

A urine drug test cup is a sealed plastic container, usually white, that looks like an oversized specimen cup with a label panel on one side. Behind a peel-off label, you’ll find a result window containing rows of small lines marked with abbreviations for each substance being tested. Each row has two zones: a Control region marked “C” and a Test region marked “T” (or the drug abbreviation).

On the outside of the cup, a temperature strip is affixed to the surface, similar to the strip on a home thermometer. This changes color to display the specimen’s temperature. Collectors must check this within four minutes of receiving the cup, and the reading needs to fall between 90°F and 100°F to be considered valid. If the temperature is outside that range, the sample is flagged as potentially tampered with.

Some cups also have built-in adulteration test strips at the bottom. These are small chemically treated pads that change color when exposed to urine. After three to five minutes, the collector compares the pad colors against a printed chart to check for signs of tampering, including abnormal pH, unusual concentration levels, and the presence of oxidizing chemicals that could mask drug use.

How to Read the Lines

The result window is the part that confuses people most, because the logic is counterintuitive. A negative result (no drugs detected above the cutoff) shows two colored lines in each row: one at the C position and one at the T position. A positive result shows only one line at the C position, with no line at T. In other words, more lines means a cleaner result.

The intensity of the lines doesn’t matter. A faint line at the T position counts exactly the same as a bold one. As long as any visible line appears in the test region, that substance is negative. If no line appears at C, the entire test is invalid regardless of what shows at T, because the control line confirms the test strip functioned correctly.

For a standard 5-panel test (the type used in most federal and DOT workplace screening), the rows cover marijuana (THC), cocaine, amphetamines, opioids, and PCP. The opioid category alone now includes confirmation testing for seven specific substances: codeine, morphine, heroin (6-AM), hydrocodone, hydromorphone, oxycodone, and oxymorphone. The amphetamine category includes MDMA and MDA. So a “5-panel” test actually screens for 14 individual drugs when confirmations are run.

What the Collection Room Looks Like

If you’re testing at a clinic or collection site, expect a few things that seem unusual. The toilet water will be blue. This isn’t decorative. Collectors are required to add bluing agent to prevent anyone from using toilet water to dilute a sample. Water faucets in the collection area may be turned off or taped shut. Soap dispensers, trash cans, and even the space under the sink may be sealed or inspected.

You’ll typically enter a single-toilet room with a full-length privacy door. Nobody watches you provide the sample in a standard collection. The collector waits outside, and you hand over the sealed cup when you’re done. In multi-stall restrooms, which are less common, the collection may be monitored or all water sources will be fully secured.

Before and after each collection, the collector inspects the room to make sure no foreign substances are present and that no one has stashed anything that could be used to alter a sample.

The Mouth Swab Test

An oral fluid test is simpler and faster. The device looks like a stick with a small sponge or absorbent pad on one end, roughly the size of a lollipop. You or the collector swabs the inside of your cheek, and the pad absorbs saliva for a few minutes until a saturation indicator signals that enough fluid has been collected. The swab is then inserted into a testing cassette or sealed container for analysis.

These are increasingly common for on-site and pre-employment testing because they’re harder to tamper with and don’t require a private bathroom. The entire process takes just a few minutes and happens in plain view.

Hair Follicle Testing

A hair drug test doesn’t involve any cup, strip, or cassette at the collection stage. A collector cuts a small sample of hair (about 90 to 120 strands) from close to the scalp. The sample is roughly 1.5 inches long, which provides a detection window of approximately 90 days of drug use history.

The cut hair is wrapped in foil and placed into a sealed envelope or kit along with chain of custody paperwork. There are no on-site results with hair testing. The sample ships to a laboratory, and results typically come back in a few business days. If head hair isn’t available, body hair can sometimes be used, though the detection window may differ.

The Paperwork

Every formal drug test involves a Chain of Custody and Control Form (CCF). This is a multi-copy carbon form that tracks the sample from the moment it leaves your body to the final lab result. It includes sections for the employer’s information and contact number, the collector’s documentation of the specimen type (urine or oral fluid), expiration dates of testing devices, and a remarks section for anything unusual during collection.

You’ll be asked to sign the form and verify your identity, usually with a photo ID. The collector fills in their portion, seals the specimen with tamper-evident tape, and initials the seal. Each copy of the form goes to a different party: the lab, the employer’s review officer, and the donor (you).

What Happens if It Goes to a Lab

Instant cup tests give preliminary results on site, but any non-negative result gets sent to a certified laboratory for confirmation. The lab uses more precise analytical methods that can identify exactly which substance is present and at what concentration. Results come back as either “detected” or “not detected” for each specific drug.

Lab confirmation exists because the initial immunoassay strips can produce false positives. Certain medications, foods, and supplements can trigger a line to disappear on the cup test even when no illicit drug is present. The confirmation step eliminates those false readings, so a positive result on the cup doesn’t necessarily mean a positive final result. If your test is sent for confirmation, expect results within a few days rather than immediately.
