At 6 weeks of pregnancy, the developing baby is technically an embryo, not yet a fetus, and it measures about 5 millimeters from top to bottom, roughly the size of a lentil. Despite being so small, this is one of the most active periods of development in the entire pregnancy. A heartbeat is just becoming detectable, the brain is dividing into distinct regions, and the earliest versions of the eyes, limbs, and internal organs are taking shape.
Embryo, Not Fetus
One important clarification: at 6 weeks, the correct term is “embryo.” The embryonic stage runs from roughly the third week of pregnancy through the end of the eighth week. After the eighth week, the embryo is reclassified as a fetus. So while “fetus at 6 weeks” is a common search, what you’re actually looking at is an embryo in the middle of its most rapid phase of organ formation.
Size and General Appearance
The crown-rump length, measured from the top of the head to the bottom of the torso, averages about 5 mm at exactly 6 weeks. That’s smaller than a pea. The embryo has a curved, C-shaped posture and a prominent head end that’s disproportionately large compared to the rest of the body. There’s also a visible tail-like structure at the bottom that will recede over the coming weeks. At this point, the embryo doesn’t look distinctly human, but the basic body plan is being laid down rapidly.
The Heart and Early Circulation
Six weeks is when a cardiac rhythm first becomes visible on ultrasound. The heart rate at this stage typically measures around 100 to 120 beats per minute. The heart itself is a simple tube-like structure that has begun to loop and will eventually divide into four chambers, but that process isn’t complete yet. Blood is circulating between the embryo and the yolk sac, a temporary structure that provides nutrients and produces early blood cells before the placenta takes over.
Brain and Nervous System
The neural tube, which forms the foundation of the brain and spinal cord, has closed by this point. At the head end, the tube has expanded into three primary brain vesicles: the forebrain, midbrain, and hindbrain. These are simple fluid-filled bulges, not functioning brain regions, but they’re the precursors to everything the brain will eventually become. Later in development, these three vesicles subdivide further into five distinct regions. The spinal cord is also forming along the length of the embryo.
Limb Buds
Small limb buds are present at 6 weeks. The arm buds appear first, with the leg buds following about two days behind. At this stage, these buds look like tiny paddles or flippers with no fingers, toes, or joints. The arms and legs always develop on a slight delay from each other, so the legs will consistently look a little less developed than the arms throughout the embryonic period. Individual fingers won’t begin to separate for another couple of weeks.
Early Facial Structures
The face is just beginning to organize, though it’s far from recognizable. Dark spots mark the location of the future eyes, where lens tissue is actively forming. By about 6 weeks, the primitive lens has started producing its earliest fibers, and the optic structures have moved outward from the brain toward the surface. Small depressions are also forming where the nostrils and ears will eventually develop. These features are all at the pit or groove stage, meaning they’re shallow indentations rather than anything resembling finished facial features.
Internal Organs in Progress
Week 6 falls in the middle of organogenesis, the period when all major internal organs begin to form. The lungs and heart are descending into the chest cavity. The liver is one of the earliest organs to develop and is already a significant structure relative to the embryo’s size, partly because it takes on the job of producing blood cells. Early versions of the thyroid, thymus, and adrenal glands are forming from different tissue layers. The kidneys are going through the first of several developmental stages (the embryo actually builds and discards two preliminary kidney systems before the final one takes hold). None of these organs are functional yet in any meaningful sense. They’re structural blueprints that will mature over the coming months.
What an Ultrasound Shows at 6 Weeks
If you have a transvaginal ultrasound at 6 weeks, the most reliably visible structures are the gestational sac and the yolk sac inside it. The yolk sac appears as a small, round, bright ring and is one of the first signs that the pregnancy is developing normally. Next to the yolk sac, the fetal pole (the earliest visible form of the embryo) may be detected, though at 5 mm it can be difficult to see clearly. In some cases, flickering cardiac activity is visible, but it’s common for it not to be detectable until closer to 7 weeks. A scan at 6 weeks that doesn’t show a heartbeat isn’t necessarily a cause for concern, since even a few days can make a significant difference at this stage.
What Hasn’t Developed Yet
At 6 weeks, there are no bones. The skeleton is entirely soft cartilage and won’t begin to harden for several more weeks. There are no distinguishable fingers or toes. The sex organs haven’t differentiated, so the embryo has no identifiable biological sex yet. The cerebral cortex, the part of the brain responsible for consciousness, thought, and sensation, won’t develop until much later in pregnancy. The embryo has no capacity to feel pain at this stage, as the neural pathways required for pain processing don’t form until the second or third trimester.
