A pulled muscle often looks swollen, red, or bruised, though mild strains sometimes show no visible changes at all. The more severe the tear, the more dramatic the appearance. A minor pull might only cause slight puffiness around the injured area, while a serious tear can produce deep bruising, a noticeable dent in the muscle, or visible bunching where the muscle fibers have separated.
Swelling and Redness
Swelling is usually the first visible sign. Within minutes of the injury, the area around the damaged muscle fibers begins to puff up as your body sends extra fluid and blood flow to the site. The skin over the injured muscle may look slightly raised or feel tight compared to the same spot on the other side of your body. In mild strains, this swelling can be subtle enough that you notice it more by touch than by sight.
Redness often accompanies the swelling, especially in the first few hours. The skin over the pull may feel warm to the touch. This is your body’s inflammatory response kicking in, flooding the area with blood to begin repairs. On lighter skin tones the redness is easy to spot. On darker skin, the area may look slightly darker or have a faint sheen from the taut, swollen skin beneath.
When Bruising Appears
Bruising doesn’t usually show up right away. A healthcare provider examining a pulled muscle may not see visible bruising until at least 24 hours after the injury. That delay catches people off guard. You might strain your hamstring during a run and see nothing that evening, then wake up the next morning to find a patch of discoloration spreading across the back of your thigh.
The color of a bruise changes predictably over about two weeks. It starts as a pinkish or red mark, then deepens to dark blue or purple within a day or two. From there it fades through violet, then green, then dark yellow, and finally a pale yellow before disappearing completely. A bruise that follows this progression is healing normally.
One thing that surprises many people: bruising can appear well below the actual injury site. When small blood vessels tear inside the muscle, the leaked blood pools and travels downward with gravity. A hamstring strain near the back of the knee, for example, might produce bruising that shows up on the calf a day or two later. This doesn’t mean the injury is spreading. It just means blood is settling into the surrounding tissue.
What Different Severities Look Like
Muscle strains fall into three general grades, and each one looks noticeably different on the surface.
A grade 1 (mild) strain involves only a small number of torn fibers. You’ll feel pain and possibly some tightness, but the area may look completely normal or show only minor swelling. There’s typically no bruising, and the muscle still holds its normal shape. This is the kind of pull where someone else looking at your leg or arm might not see anything wrong at all.
A grade 2 (moderate) strain tears a larger portion of the muscle. Swelling is more obvious, bruising is common within a day, and you may notice the injured area looks puffy or slightly misshapen compared to the uninjured side. The skin over the muscle can appear stretched and tight from the fluid buildup underneath. Moving the muscle is painful enough that you’ll instinctively avoid using it.
A grade 3 (severe) strain is a complete or near-complete tear of the muscle. This is where the visual signs become unmistakable. You may see a visible gap or dent in the muscle where the fibers have separated. In some cases, the torn muscle bunches up above or below the tear site, creating a lump. A classic example is a complete bicep tendon tear, which causes the muscle belly to roll up toward the shoulder, creating what’s sometimes called a “Popeye” deformity because of the ball-shaped bulge on the upper arm. Severe bruising, significant swelling, and an obvious change in the muscle’s contour are all hallmarks of this grade.
How Location Affects Appearance
Where you pull a muscle changes how visible the injury looks from the outside. Muscles close to the skin’s surface, like the calf or bicep, tend to show bruising and swelling more readily. A calf strain (sometimes called “tennis leg”) often produces dramatic bruising along the back of the lower leg, sometimes pooling all the way down to the ankle. The calf may look noticeably larger on the injured side from the swelling alone.
Hamstring strains are among the most common muscle pulls, especially in runners and athletes. Because the hamstring muscles sit on the back of the thigh beneath a layer of tissue, mild strains can be hard to see. Moderate and severe tears, though, produce bruising that spreads across a wide area of the back of the thigh and can migrate down toward the knee over several days.
Deep muscles, like those in the lower back or deep hip, may cause significant pain without much visible change on the surface. The swelling and any bleeding happen far enough beneath the skin that you won’t see bruising or puffiness. This is one reason back strains can feel severe while looking deceptively normal.
Signs That Suggest Something More Serious
Most pulled muscles, even painful ones, follow a predictable pattern: swelling peaks in the first 48 hours, bruising appears and then slowly fades over two weeks, and the muscle gradually returns to its normal shape. But certain visual signs suggest the injury may be more than a standard strain.
If the area around the injury looks unusually tight, firm, or full, and the swelling seems to be getting worse rather than stabilizing, this could indicate a condition called compartment syndrome. This happens when swelling inside a muscle compartment builds up so much pressure that it cuts off blood flow. The muscle may feel rock-hard to the touch and look visibly bulging. Pain from compartment syndrome is severe and out of proportion to what you’d expect from a simple strain. It’s also accompanied by numbness, tingling, or a burning sensation under the skin. Acute compartment syndrome is a medical emergency that can cause permanent muscle damage if not treated quickly.
A blood clot forming at the injury site can also change the appearance. If a lump develops over the pulled muscle and doesn’t shrink over the first few days, a pool of blood (hematoma) may have collected in the damaged tissue. Small hematomas resolve on their own, but larger ones sometimes need medical attention.
Any visible deformity in the muscle’s shape, like a clear gap, dent, or bunched-up lump, points to a severe tear that likely needs imaging and possibly surgical repair. If you can see that the muscle no longer looks like its counterpart on the other side of your body, that’s worth getting evaluated promptly.
