What Does a Vaginal Swab Test For? BV, STIs & More

A vaginal swab can test for a wide range of conditions, from common vaginal infections like yeast and bacterial vaginosis to sexually transmitted infections like chlamydia, gonorrhea, and trichomoniasis. The specific conditions being screened depend on why the swab is being collected. A swab ordered because of unusual discharge will be analyzed differently than one collected during routine STI screening or a late-pregnancy checkup.

Common Vaginal Infections

The most frequent reason for a vaginal swab is to identify what’s causing symptoms like itching, odor, or abnormal discharge. A standard vaginitis panel can detect several organisms at once, including the bacteria behind bacterial vaginosis (specifically Gardnerella vaginalis and Atopobium vaginae), multiple Candida yeast species (including C. albicans and C. glabrata), and the parasite Trichomonas vaginalis. The panel also measures levels of healthy Lactobacillus bacteria, which helps paint a fuller picture of your vaginal flora.

Bacterial vaginosis (BV) is diagnosed when the normal balance of vaginal bacteria shifts. Clinicians look for at least three of four signs: a thin, milky discharge, a pH above 4.5 (healthy vaginal pH sits between 3.8 and 4.5), a fishy odor when a chemical solution is applied to the sample, and the presence of “clue cells,” which are skin cells coated in bacteria visible under a microscope. A yeast infection, by contrast, shows up as branching filaments or budding yeast cells on a microscope slide. Trichomoniasis reveals itself through tiny, actively moving parasites along with elevated white blood cells.

Sexually Transmitted Infections

Vaginal swabs are one of the primary tools for STI screening. They can detect chlamydia, gonorrhea, trichomoniasis, herpes, and HPV. For chlamydia and gonorrhea, labs typically use nucleic acid amplification testing, which identifies genetic material from the bacteria and is considered the gold standard for accuracy.

HPV testing has traditionally required a cervical sample collected during a Pap smear, but research shows vaginal swabs perform comparably. In one study comparing the two methods, vaginal swabs detected high-risk HPV strains with sensitivity above 90% and specificity above 96%, making them a viable alternative for HPV DNA screening. Herpes testing via swab works best when an active sore or blister is present, since the virus needs to be shedding at the collection site.

Group B Strep During Pregnancy

If you’re pregnant, you’ll have a vaginal and rectal swab collected during week 36 or 37 of your pregnancy. This screens for Group B Streptococcus (GBS), a bacterium that’s harmless to most adults but can cause serious illness in newborns during delivery. According to the CDC, screening at this stage is one of the most effective ways to prevent GBS disease in a baby’s first week of life. A positive result doesn’t mean anything is wrong with you. It simply tells your care team to give you antibiotics during labor to protect the baby.

What Happens With Your Sample

Once collected, your swab may be analyzed in several ways depending on what’s being tested. Some assessments happen immediately in the clinic. A wet mount involves placing a drop of vaginal fluid on a glass slide and examining it under a microscope for yeast, parasites, clue cells, and white blood cells. A separate preparation using a potassium hydroxide solution breaks down other cells so yeast structures are easier to spot, and also produces the “whiff test,” where a fishy smell suggests BV.

Molecular testing and cultures are sent to an outside lab. Results from a BV test typically come back in one to three days. STI results from molecular testing usually take a similar timeframe, though some clinics have rapid point-of-care tests that return results within an hour or two.

Self-Collected Swabs

You don’t always need a clinician to collect the sample. Self-collected vaginal swabs, where you insert the swab yourself, are now widely used in clinics and through at-home testing kits. Their accuracy is remarkably close to clinician-collected samples. A meta-analysis found that self-collected vaginal swabs detected chlamydia with 92% sensitivity and 98% specificity compared to clinician-collected cervical swabs. For gonorrhea, the numbers were even higher: 98% sensitivity and 97% specificity.

At-home kits work the same way. You collect your own sample, seal it, and mail it to a lab. Turnaround for home kits is generally a few days to a week, depending on the company.

How to Prepare for a Vaginal Swab

For the most accurate results, avoid sexual intercourse, douching, and any vaginal products (spermicides, creams, suppositories) for two days before the test. These can wash away the very cells or organisms the lab needs to detect. If you’re on your period, check with your provider. Menstrual blood can raise vaginal pH and potentially affect results for conditions like BV, though STI testing is generally unaffected.

The swab itself takes only a few seconds. A clinician inserts a soft-tipped swab a few inches into the vaginal canal and rotates it gently. If you’re doing it yourself, you’ll receive instructions to insert the swab about two inches and turn it for 10 to 30 seconds. Most people describe it as mildly uncomfortable rather than painful.
