What Does a Vitamin B12 Rash Look Like?

A vitamin B12 rash can take several forms depending on what’s causing it. The two most common types are an acne-like eruption triggered by high-dose B12 supplements and an allergic skin reaction related to cobalt sensitivity (cobalt is a core component of the B12 molecule). These look quite different from each other, so identifying which type you’re dealing with matters.

The Acne-Like Eruption

The most widely reported B12 skin reaction is an acneiform eruption, meaning it looks like acne but isn’t true acne. It appears as clusters of red, inflamed bumps and pustules, often on the face, chest, and upper back. The breakout can appear suddenly, sometimes within days to weeks of starting a B12 supplement or receiving an injection.

The key visual difference between this eruption and regular acne is the absence of comedones and cysts. Comedones are the blackheads and whiteheads that characterize ordinary acne. If you’re seeing inflamed red bumps and pus-filled spots but no blackheads or deep cystic lumps, and the timing lines up with starting B12, that pattern is characteristic of a supplement-triggered reaction. The bumps tend to be more uniform in size and appearance than typical acne breakouts, which usually include a mix of blackheads, whiteheads, and inflamed lesions at different stages.

This reaction happens because excess B12 alters the behavior of skin bacteria. When B12 levels spike, the bacteria that naturally live on your skin shift their metabolism in ways that promote inflammation, producing compounds that trigger those angry-looking pustules. This is why the eruption is most common with high-dose supplementation rather than the small amounts you get from food.

Allergic Rash From Cobalt Sensitivity

A different type of B12 rash stems from an allergy to cobalt, the metal at the center of every B12 molecule. If you have a cobalt allergy (which can be confirmed through patch testing), taking B12 in any form can trigger a skin reaction. The UK’s Medicines and Healthcare products Regulatory Agency specifically warns that people with known cobalt allergies should watch for sensitivity reactions when taking B12.

Cobalt sensitivity reactions typically look like allergic contact dermatitis: red, itchy, sometimes scaly patches of skin. These can develop gradually over days (subacute) or persist over weeks (chronic). The rash may appear at the injection site if you received a B12 shot, or it can show up more broadly across the body. Hives, which are raised, itchy welts that can appear anywhere, are another common presentation.

In rarer cases, cobalt allergy can trigger something called an erythema multiforme-like reaction. This produces distinctive target-shaped lesions: round spots with a darker center, a paler ring around it, and a red outer border. These can appear on the hands, forearms, feet, and sometimes the face or trunk. If you see these target-like marks after taking B12, that’s a reaction worth getting evaluated promptly.

Severe Reactions to Watch For

Most B12 rashes are uncomfortable but not dangerous. However, serious allergic reactions do occur. An extensive rash that spreads rapidly across large areas of the body, or a rash with blistering, signals a more significant immune response. Blistering is particularly important to notice because it indicates deeper skin involvement beyond surface irritation.

Another pattern to be aware of is symmetrical drug-related intertriginous and flexural exanthema, which has been reported in B12 hypersensitivity cases. This shows up as a well-defined red rash concentrated in skin folds: the groin, armpits, inner elbows, and behind the knees. It appears symmetrically on both sides of the body, which helps distinguish it from a random rash or infection.

Where B12 Rashes Typically Appear

The acne-like eruption favors the face (especially the forehead, cheeks, and chin), the chest, and the upper back. These are areas with a high density of oil glands, which makes sense given that the reaction involves skin bacteria and sebum production.

Allergic rashes from cobalt sensitivity are less predictable in location. If triggered by an injection, redness, swelling, and itching often start at the injection site and may spread. With oral supplements, the rash can appear anywhere but tends to favor areas where contact dermatitis commonly shows up: the hands, arms, and trunk. Hives from an allergic response can pop up on any part of the body.

How to Tell If B12 Is the Cause

Timing is the strongest clue. If a rash appeared within days to a few weeks of starting B12 supplements or injections, and you haven’t changed anything else (new skincare products, medications, or laundry detergent), B12 is a reasonable suspect. The acneiform eruption specifically tends to emerge quickly and affect areas that were previously clear.

For the acne-like version, the absence of blackheads is the most useful visual marker. Look closely at the bumps. If they’re uniformly inflamed, red, and some are filled with pus, but you don’t see any of the small dark or white plugged pores that come with regular acne, that fits the B12 pattern.

For allergic reactions, itching is a strong indicator. Regular acne isn’t typically itchy. If your rash itches significantly, especially if it looks more like hives or patchy dermatitis than pimples, an allergic mechanism is more likely. A history of reacting to cobalt-containing jewelry or metal objects strengthens this connection, since B12 contains cobalt and can trigger the same immune pathway.

What Happens When You Stop B12

The acneiform eruption typically clears once B12 supplementation is reduced or stopped, though it can take several weeks for existing breakouts to fully resolve. Your skin bacteria need time to return to their baseline behavior after B12 levels drop. Standard acne treatments often have limited effect on these breakouts because the underlying cause is the excess vitamin, not the usual acne triggers.

Allergic reactions from cobalt sensitivity also resolve after stopping B12, though the timeline depends on severity. Mild contact dermatitis-type reactions may clear within a week or two. More extensive rashes or hives can take longer, and some people benefit from antihistamines or topical treatments to manage symptoms during that window.

If you need B12 supplementation for a diagnosed deficiency, the situation gets more complicated. Switching between the two main forms of supplemental B12 sometimes helps with allergic reactions, since the non-cobalt portions of the molecule differ. But because both forms contain cobalt, people with true cobalt allergies may react to either one.
