What Does Adderall Show Up as on a Drug Screen?

Adderall shows up as amphetamine on a standard drug screen. The initial immunoassay test used in most workplace and clinical settings doesn’t distinguish between Adderall, other prescription amphetamines, or illicit amphetamine use. It simply flags the sample as positive for the amphetamine class. If the result triggers a confirmatory test, the lab can identify the specific compounds present and, in some cases, determine whether the source was a prescription medication or something else.

How the Initial Screen Works

Most drug screens use a method called immunoassay, which detects broad categories of drugs rather than individual medications. Amphetamines are one of the five standard drug classes tested in both federal workplace panels and most private employer tests. The others are marijuana, cocaine, opiates, and PCP.

The initial screen has a cutoff concentration of 500 ng/mL for amphetamines in urine. If your sample falls below that threshold, it’s reported as negative. If it’s at or above that level, the sample moves to confirmatory testing. For oral fluid (saliva) tests, the cutoff is much lower at 50 ng/mL, meaning smaller amounts can trigger a positive result.

Because the immunoassay casts a wide net, it can’t tell the difference between Adderall, Vyvanse, illicit methamphetamine, or even certain over-the-counter cold medications. That’s why a positive initial screen is never treated as a final answer in any properly run testing program.

What Happens During Confirmatory Testing

When an initial screen comes back positive for amphetamines, the lab runs a second, more precise test using a technique called gas chromatography-mass spectrometry. This test identifies the exact molecules in your sample and measures their concentrations against lower cutoff levels: 250 ng/mL for amphetamine and 250 ng/mL for methamphetamine.

Adderall contains a specific mix of amphetamine salts, both the “d” (dextro) and “l” (levo) forms of amphetamine. Illicit methamphetamine, by contrast, is typically all d-methamphetamine or a 50/50 mix of both forms. Labs can use chiral derivatization, a process that separates these mirror-image molecules, to determine which forms are present in a sample. This ratio helps distinguish between prescription Adderall use, over-the-counter decongestant use (which contains the l-form of methamphetamine), and illicit drug use. So while the initial screen just says “amphetamine,” the confirmatory test can paint a much more detailed picture.

How a Prescription Protects You

If you have a valid Adderall prescription and test positive for amphetamines, the process doesn’t end with the lab result. In regulated testing programs, a Medical Review Officer (MRO) reviews every positive result before it’s reported to an employer. The MRO contacts you directly and gives you the opportunity to provide evidence of a legitimate prescription.

This isn’t just a casual check. The MRO will call your pharmacy to verify the prescription is real, confirm the prescribing physician, and review whether the timing and dosage are consistent with the test result. Photos of a pill bottle label aren’t sufficient on their own. If everything checks out, the MRO reports the result as negative to the employer, and no further action is taken. You’re not required to disclose your diagnosis or the reason for the prescription to your employer.

Detection Windows by Test Type

How long Adderall remains detectable depends on the type of test and how frequently you’ve been taking it.

  • Urine: A single dose clears in roughly 2 to 3 days. If you’ve been taking Adderall regularly at higher doses, allow up to 7 days for it to fall below detectable levels.
  • Saliva: Oral fluid tests generally detect amphetamines for a shorter window than urine, typically 1 to 2 days, but the cutoff concentration is much lower (25 ng/mL on confirmation), so even small amounts register.
  • Hair: Hair follicle tests detect amphetamine use over a 3-month window. They’re designed to reveal patterns of repeated use rather than a single dose.
  • Blood: Blood tests have the shortest detection window, generally under 24 hours, and are rarely used for routine employment screening.

One major variable is urine acidity. Amphetamine is a weak base, which means acidic urine causes the body to flush it out much faster. Research shows that shifting from alkaline urine (pH around 8) to acidic urine (pH around 5) can increase amphetamine excretion by up to 11 times. This means factors like diet, hydration, and certain medications that change urine pH can meaningfully shorten or extend the detection window.

Medications That Cause False Positives

Even if you’ve never taken Adderall or any amphetamine, certain medications can trigger a false positive on the initial immunoassay screen. The list is longer than most people expect. Common culprits include pseudoephedrine and phenylephrine (found in many cold and sinus medications), bupropion (an antidepressant also used for smoking cessation), phentermine (a weight-loss medication), and nasal decongestant inhalers. Several other prescription drugs can also cross-react, including certain antipsychotics, the ADHD medication atomoxetine, trazodone, fluoxetine, and even metformin.

A false positive on the initial screen will be caught by the confirmatory test, which identifies the actual molecules present. If the confirmatory test doesn’t find amphetamine or methamphetamine above the cutoff levels, the result is reported as negative. This is one of the key reasons the two-step testing process exists.

Federal and DOT Testing

If your job falls under Department of Transportation regulations (trucking, aviation, rail, transit, pipeline, or maritime), amphetamine testing is mandatory and follows strict federal protocols. The DOT requires laboratory-based urine testing for all five standard drug classes, and the process always includes MRO review. Rapid point-of-care tests or at-home kits don’t satisfy DOT requirements.

Private employers who aren’t covered by DOT rules can set their own testing policies, but most follow the same five-panel framework and cutoff levels. Some employers use expanded panels (7, 10, or 12 drugs), but amphetamines are included in virtually all of them. The same MRO verification process applies in most professional testing programs regardless of the employer type.
