Licensed practical nurses (LPNs) in hospitals carry out hands-on patient care under the direction of registered nurses and physicians. The bulk of an LPN’s hospital shift revolves around direct patient contact. That includes checking vital signs like blood pressure, temperature, pulse, respiratory rate, and oxygen levels. LPNs change wound dressings, insert catheters, assist with bathing and dressing, help patients eat, and monitor blood sugar for diabetic patients. In hospitals specifically, they also collect lab samples and run routine tests.
Medication administration is one of the most time-intensive parts of the job. LPNs prepare and deliver prescribed medications following the “five rights” protocol: right patient, right drug, right dose, right route, right time. They document every medication given, track patient reactions, and report anything unusual. In some states, LPNs can also start IV fluid drips and administer IV medications, though this varies by state law and sometimes requires additional certification or on-site RN supervision.
Documentation runs alongside everything else. LPNs chart their assessments, record treatments, note any conversations with patients or family members, and call physicians when orders need clarification or a patient’s condition changes. This charting isn’t just administrative busywork. It’s the legal record of care and the primary way the rest of the healthcare team stays current on each patient.
How LPN Duties Differ From RN Duties
The simplest way to understand the split: RNs assess, diagnose, and plan. LPNs gather information and carry out the plan. An LPN takes your vital signs and reports the numbers. The RN interprets those numbers, identifies problems, and builds a care strategy. RNs also coordinate with other professionals to adjust treatment as a patient’s condition evolves.
This distinction matters in practice. LPNs contribute valuable data to patient assessments, but they don’t independently formulate nursing diagnoses or design care plans. They implement the care plan that an RN or physician has already established. That’s why LPN practice is legally classified as “dependent,” meaning an LPN works under the direction of an RN, physician, or other authorized provider who communicates the plan of care.
Supervision in the Hospital
LPNs don’t work unsupervised, but the supervising RN doesn’t need to stand over their shoulder either. The RN must be continuously available by phone or other communication, and they conduct periodic on-site visits to verify that care meets safe practice standards. Before assigning tasks to an LPN, the supervising RN evaluates the patient’s stability, the complexity of the care needed, and whether the specific LPN has the training and skill to handle it.
Certain tasks trigger stricter rules. IV therapy, for instance, often requires the supervising RN to be physically present on-site rather than just reachable by phone. These requirements vary by state, so an LPN’s exact scope of practice in a hospital in Ohio may look different from one in Texas or California.
What a Typical Shift Looks Like
A hospital LPN’s day starts with a shift handoff. The outgoing team gives a detailed report on each patient: current condition, new orders, status changes, and tasks still pending. This briefing sets the pace for everything that follows.
Morning rounds come next. The LPN visits each assigned patient, checks vital signs, observes for changes in condition or pain levels, and assists with personal care. Patients who need help with hygiene, dressing, or eating get attention during this window, though nursing assistants often handle simpler tasks under the LPN’s oversight.
Mid-shift typically focuses on medication passes and treatments: wound care, catheter management, ostomy care, insulin administration. After those tasks wrap up, there’s a dedicated block for documentation and physician communication, including calling in for new orders or flagging concerns. Throughout the shift, the LPN stays ready to respond to urgent changes or emergencies, even during scheduled breaks.
Hospital Departments That Employ LPNs
LPNs work across a wider range of hospital units than many people realize. Common placements include emergency departments, cardiac telemetry units, rehabilitation floors, nephrology units, GI labs, and long-term acute care or progressive care wings. The specific duties shift depending on the department. An LPN in cardiac telemetry spends more time monitoring heart-related readings, while one in rehabilitation focuses on helping patients regain mobility and independence after surgery or injury.
Certifications That Expand the Role
LPNs who want to take on more responsibility in a hospital can pursue post-licensure certifications. The National Association for Practical Nurse Education and Service (NAPNES) offers certifications in pharmacology, long-term care, and IV therapy. An IV therapy certification is particularly relevant for hospital LPNs, since it opens the door to starting IVs and administering intravenous medications in states that allow it. Pharmacology certification deepens medication management skills, which can make an LPN more valuable on busy hospital floors where complex drug regimens are common.
Pay in a Hospital Setting
LPNs working in general medical and surgical hospitals earn an average of $58,500 per year. That’s actually on the lower end compared to some other LPN work settings, which may surprise people who assume hospital jobs always pay more. The tradeoff is often broader clinical experience, exposure to a wider variety of conditions, and a clearer pathway to advancing into an RN role for those who choose to continue their education.
