What Does an LPN Do? Roles and Responsibilities

A licensed practical nurse (LPN) provides hands-on patient care under the supervision of a registered nurse or physician. LPNs work across nearly every healthcare setting, from nursing homes to hospitals to private clinics, handling essential tasks like taking vital signs, administering medications, and helping patients through appointments and procedures. In Texas and California, the same role goes by “licensed vocational nurse” (LVN), but the responsibilities are identical.

Day-to-Day Responsibilities

An LPN’s daily work revolves around direct patient contact. In a typical clinic day, that means greeting and rooming patients, collecting health histories, preparing patients for exams or procedures, drawing lab specimens, and giving injections as directed by a provider. After the visit, LPNs often handle follow-up instructions, making sure patients understand their next steps through both verbal and written communication.

Behind the scenes, LPNs review medical records before appointments so the provider has relevant information ready, complete diagnostic tests that have been ordered, and document everything that happened during the visit. In facilities like nursing homes and rehabilitation centers, LPNs also oversee nursing assistants, making sure day-to-day resident care stays on track. They participate in health maintenance, patient education, counseling, and collaborative care planning, though always under an RN’s supervision.

What LPNs Cannot Do

The clearest way to understand the LPN role is to know where its boundaries fall. LPNs cannot perform a comprehensive initial assessment or interpret clinical data independently. They don’t develop nursing care plans, formulate nursing diagnoses, or supervise registered nurses. Certain components of IV therapy are also off-limits in many states. These restrictions exist because the LPN training path is shorter and more focused than an RN’s, so tasks requiring independent clinical judgment stay with the RN or physician.

That said, scope of practice varies by state. Some states allow LPNs to start basic IV lines or hang certain fluids, while others restrict IV work entirely. If you’re considering this career, checking your state board of nursing’s specific rules is essential.

Where LPNs Work

The largest single employer of LPNs is skilled nursing facilities, which account for about 27% of the roughly 630,000 LPNs working in the U.S. as of 2023. General hospitals employ another 13%, followed closely by home health care services at 12% and physicians’ offices at nearly 12%. Assisted living and continuing care retirement communities round out the top five at about 7%.

The work feels different depending on the setting. In a nursing home, LPNs often manage a group of residents over an entire shift, monitoring medications, tracking changes in condition, and coordinating with nursing assistants. In a doctor’s office, the pace is appointment-driven, with more variety in the types of patients and concerns that come through the door. Home health LPNs travel to patients’ homes, which offers more autonomy in the moment but still requires reporting back to an RN supervisor.

Education and Licensing

LPN training programs typically run about 12 months through a community college or technical school. The curriculum covers anatomy, pharmacology, nutrition, and clinical skills, with supervised clinical rotations built in. After completing an approved program, graduates must pass the NCLEX-PN, the national licensing exam administered by the National Council of State Boards of Nursing. You cannot practice as an LPN without passing this exam and holding an active state license.

Compared to the two or four years required for an RN degree, the 12-month timeline is a significant draw for people who want to enter healthcare quickly. Many LPNs later use their experience as a stepping stone toward an RN program, often receiving credit for coursework and clinical hours already completed.

Certifications and Specialization

Beyond the baseline license, LPNs can earn over 20 specialty certifications to deepen their expertise or move into specific areas of care. Some of the most common options include:

  • Certified Wound Care Associate for LPNs working in long-term care or surgical settings
  • Certified Hospice and Palliative Licensed Nurse for end-of-life care

Specialty certifications can open doors to higher-paying positions and signal expertise to employers, particularly in competitive job markets. They also fulfill continuing education requirements that most states mandate for license renewal.

LPN vs. RN vs. CNA

The nursing workforce operates in layers. Certified nursing assistants (CNAs) handle the most basic care: bathing, feeding, repositioning, and recording vital signs. LPNs sit in the middle, performing clinical tasks like wound care, medication administration, and catheter management while supervising CNAs. Registered nurses hold the broadest scope, independently assessing patients, creating care plans, administering all types of IV medications, and making clinical judgments that guide treatment.

In practice, LPNs often serve as the connective tissue in a care team. They spend more time at the bedside than RNs in many facilities, catching subtle changes in a patient’s condition and communicating those observations up the chain. That combination of clinical skill and consistent patient contact makes the role both demanding and central to how care actually gets delivered.
