CoQ10 is your body’s cellular fuel helper. It sits inside nearly every cell and plays a critical role in converting the food you eat into usable energy. Without enough of it, your cells struggle to power themselves, and organs that demand the most energy, like your heart and brain, feel the effects first. Beyond energy production, CoQ10 doubles as an antioxidant, protecting cells from the kind of damage that accelerates aging and disease.
How CoQ10 Powers Your Cells
Every cell in your body contains tiny structures called mitochondria, often described as cellular power plants. CoQ10 works inside these mitochondria as a shuttle, carrying electrons between different stages of the energy-production chain. This process converts fats and carbohydrates into ATP, the molecule your cells actually burn for fuel. Without CoQ10 in this chain, the whole system stalls.
While it’s ferrying electrons, CoQ10 also helps push hydrogen ions across a membrane inside the mitochondria. This creates a kind of pressure gradient, like water building behind a dam. When those ions flow back through, the rush of energy is captured and used to form ATP. It’s an elegant system, and CoQ10 is one of the few components that can’t be substituted.
Because CoQ10 dissolves in fat rather than water, it can embed itself directly into cell membranes. That positioning lets it intercept free radicals before they damage the fatty structures that make up cell walls, mitochondrial membranes, and the protective coatings around lipoproteins in your blood (the particles that carry cholesterol). This antioxidant role is separate from its energy function but equally important.
Your Body Makes Less as You Age
Your body produces CoQ10 naturally, but production peaks in your mid-20s and declines steadily after that. In heart tissue, CoQ10 levels at age 65 are roughly half of what they were at age 25. The retina and the tissue behind it lose about 40% of their CoQ10 between youth and age 80. This decline is one reason why mitochondrial function deteriorates with age, contributing to fatigue, slower recovery, and increased vulnerability to chronic disease.
Certain medications accelerate this loss. Statins, the cholesterol-lowering drugs taken by tens of millions of people, block the same biochemical pathway your body uses to produce CoQ10. This is why muscle pain and weakness are common statin side effects. Clinical trials have tested CoQ10 supplements at doses ranging from 100 to 400 mg per day to counteract statin-related muscle symptoms, though results have been mixed and the ideal dose hasn’t been firmly established.
Heart Health and Blood Pressure
The heart is one of the most energy-hungry organs in the body, beating roughly 100,000 times a day. It’s no surprise that CoQ10 levels in heart tissue matter. A meta-analysis of clinical trials found that CoQ10 supplementation improved ejection fraction (the percentage of blood your heart pumps out with each beat) by an average of 3.67% in people with heart failure. That may sound modest, but for a struggling heart, it translates to meaningfully better function. The improvements were most notable in people with less severe heart failure and in studies using doses of 100 mg per day or less.
CoQ10 also appears to influence blood pressure. A Cochrane review of clinical trials found that people with elevated blood pressure who took CoQ10 saw their systolic pressure (the top number) drop by an average of 11 mmHg and their diastolic pressure (the bottom number) drop by about 7 mmHg. Those are reductions comparable to some first-line blood pressure medications, though the trials were small, involving only 96 total participants, so the evidence warrants some caution.
Migraine Prevention
CoQ10 has earned a specific place in migraine management. The American Headache Society recognizes it as a nutraceutical option for migraine prevention, with research supporting a dose of 300 mg per day to reduce how often migraines occur. The connection likely ties back to mitochondrial function: brain cells are energy-intensive, and migraines may partly result from impaired energy metabolism. For people who experience frequent migraines and want to try a supplement before or alongside prescription options, CoQ10 is one of the better-studied choices.
Fertility and Egg Quality
One of the more compelling areas of CoQ10 research involves female fertility. Eggs are among the most mitochondria-dense cells in the body, and their quality depends heavily on energy production. As women age, declining CoQ10 levels contribute to reduced egg quality, poorer fertilization rates, and higher rates of embryo failure.
A review of nearly 80 studies found that CoQ10 supplementation can enhance ovarian function and improve both egg and embryo quality by restoring mitochondrial health and reducing oxidative stress. In one randomized controlled trial involving young women with diminished ovarian reserves, those who took CoQ10 had more eggs retrieved, better fertilization rates, and more high-quality embryos. The rate of canceled embryo transfers due to poor development dropped from about 23% in the control group to just 8% in the CoQ10 group.
Lab studies reinforce the finding. When CoQ10 was added to the culture medium used to mature eggs outside the body, maturation rates jumped from 49% to nearly 76%. Research also suggests CoQ10 may counteract ovarian aging and reduce DNA damage in eggs, though most of this work comes from animal and laboratory models rather than large human trials.
Skin Aging and UV Protection
CoQ10 levels in the skin decline with age just as they do elsewhere in the body, and this drop leaves skin cells more vulnerable to oxidative damage from UV light and environmental pollutants. Topical CoQ10 can penetrate into the living layers of skin, not just sit on the surface. A six-month study of CoQ10 applied around the eyes showed a measurable reduction in wrinkle depth, and a separate four-week trial found improvements in both wrinkle roughness and depth compared to untreated skin.
The UV protection angle is also notable. CoQ10 reduces the production of collagenase, an enzyme triggered by sun exposure that breaks down collagen. It also stabilizes mitochondrial function in skin cells exposed to UV radiation, essentially helping them maintain their energy supply under stress. Applied topically, CoQ10 improved the skin’s overall antioxidant status within one to two weeks of regular use. These effects don’t replace sunscreen, but they add a layer of cellular-level defense.
How to Get CoQ10
Your body synthesizes CoQ10 on its own, and you also get small amounts from food. Organ meats, particularly beef heart, are the richest dietary sources. Fatty fish like sardines and mackerel, along with whole grains and some nuts, contribute smaller amounts. However, the total CoQ10 you get from a typical diet is estimated at only 3 to 6 mg per day, far below the doses used in clinical trials.
Supplement doses in research range widely: 100 mg per day for heart failure support, 300 mg per day for migraine prevention, and anywhere from 100 to 600 mg per day for fertility and other uses. CoQ10 is fat-soluble, so it absorbs better when taken with a meal that contains some fat. The ubiquinol form is already in its reduced (active) state and may absorb more efficiently than the ubiquinone form, though both raise blood levels effectively at adequate doses. Side effects are uncommon and typically limited to mild digestive upset.
