Most people with fatty liver feel nothing at all. The condition is frequently called a “silent” disease because it produces no obvious symptoms in its early stages, and many people only discover it through routine blood work or an imaging scan done for another reason. When symptoms do appear, they tend to be vague: a dull discomfort in the upper right side of your abdomen, persistent tiredness, or a general sense of feeling unwell.
Why Most People Don’t Feel It
The liver itself has very few pain-sensing nerves inside its tissue. Fat can accumulate in liver cells for years without triggering any sensation you’d notice. Studies have found fatty liver in people who were being screened for completely unrelated reasons and had zero complaints. Even when the disease progresses to the inflammatory stage, or when early scarring develops, many people still report no symptoms. You can technically have significant liver damage and feel fine, which is part of what makes fatty liver dangerous.
The Symptoms That Do Show Up
When fatty liver does produce noticeable feelings, the most common are fatigue, a vague sense of malaise, and pain or discomfort in the upper right part of your belly, just below and behind your lower ribs. That abdominal sensation is usually described as a dull ache, a feeling of fullness, or a mild pressure rather than a sharp pain. It comes from the liver’s outer capsule stretching as the organ swells with fat and inflammation.
The fatigue is often the symptom people notice most, and it can be surprisingly severe relative to what seems like a “mild” condition. Research published in the World Journal of Hepatology outlines several overlapping reasons for this. Damaged liver cells trigger low-grade inflammation throughout the body, which reaches the brain and disrupts the chemical pathways that regulate energy and motivation. Specifically, it shifts how the body uses tryptophan (a building block for serotonin, the “well-being” chemical), steering it toward producing compounds that are actually toxic to nerve cells instead.
On top of that, the energy-producing machinery inside your cells, called mitochondria, doesn’t work as efficiently when the liver is inflamed. This means your muscles and brain get less fuel at a cellular level. Even before cirrhosis develops, the liver can start struggling to clear ammonia from your blood. Small elevations in ammonia that wouldn’t show up on a standard blood test can still cause mental fogginess, forgetfulness, and reduced concentration. Gut bacteria changes that often accompany fatty liver add another layer, allowing inflammatory molecules to leak into the bloodstream and sustain a low-level “sickness” feeling.
Brain Fog and Cognitive Changes
If you’ve noticed trouble concentrating, mild forgetfulness, or a fuzzy-headed feeling you can’t quite explain, it may be connected to your liver. When liver function declines, the organ becomes less effective at filtering toxins, particularly ammonia, from the blood. These toxins affect the nervous system and can produce what’s clinically called hepatic encephalopathy, though in its earliest and mildest form it looks a lot like what people casually call “brain fog.”
Early signs include poor concentration, difficulty with judgment or decision-making, subtle personality shifts, and mood changes. These symptoms tend to creep in slowly, so you might chalk them up to stress, poor sleep, or aging. In more advanced liver disease, these cognitive effects can worsen to include noticeable confusion, disorientation, and significant drowsiness. Fatty liver is listed as one of the common causes of this type of brain-function decline.
What Changes as the Disease Progresses
Simple fatty liver (fat accumulation without significant inflammation) sits at one end of a spectrum. When inflammation and cell damage develop, the condition has progressed. Even at this stage, many people still feel nothing. But those who do tend to notice that their fatigue deepens and their abdominal discomfort becomes more persistent.
If scarring continues and reaches the stage of cirrhosis, new and more obvious physical signs can emerge. Fluid can build up in the abdomen, a condition called ascites, which causes rapid weight gain of two to three pounds per day over several days and a visibly distended belly. Your ankles and lower legs may swell. Other symptoms at this stage include shortness of breath, difficulty sitting comfortably, bloating, constipation, indigestion, and back pain. These are signs of serious liver damage and represent a significant escalation from the vague discomfort of earlier stages.
Muscle loss and the infiltration of fat into muscle tissue also become a factor in advanced disease. This means you may feel physically weaker and find that your exercise tolerance drops, which creates a frustrating cycle: the disease makes physical activity harder, but physical activity is one of the most effective tools for improving it.
Visible Signs on Your Body
Fatty liver doesn’t typically cause visible changes in early stages. As the disease advances, some people develop spider-like clusters of tiny blood vessels on the skin, yellowing of the eyes or skin (jaundice), or redness in the palms. People with fatty liver often have insulin resistance, which is associated with a skin condition that causes dark, thick, velvety patches in body folds like the armpits, groin, and back of the neck. This darkening develops slowly and sometimes includes skin tags in the same areas. It’s not caused by the liver itself but by the same metabolic dysfunction driving the fat buildup.
When Symptoms Start to Improve
The encouraging part of fatty liver is that it responds to weight loss more directly than many chronic conditions. Losing just 3 to 5 percent of your body weight is typically enough for fat to start disappearing from liver cells. For someone who weighs 200 pounds, that’s 6 to 10 pounds. A greater loss of about 10 percent of body weight is needed to improve the inflammation and scarring that cause the more persistent symptoms.
As liver fat decreases, the fatigue and abdominal discomfort that some people experience tend to ease. The cognitive fog linked to mild ammonia buildup can also clear as liver function recovers. Because the liver is unusually good at regenerating, early-stage fatty liver is considered fully reversible with sustained lifestyle changes. The key word is “sustained”: the fat returns if the habits that caused it return.
