Fish oil supplies two omega-3 fatty acids, EPA and DHA, that your body needs but can’t efficiently make on its own. These fats get built directly into your cell membranes, where they calm inflammation, support heart and brain health, ease joint stiffness, and play a role in fetal development during pregnancy. The benefits are well-studied, though they vary in strength depending on the condition and the dose.
How Omega-3s Work in Your Body
When you take fish oil, EPA and DHA don’t just float around in your bloodstream. They physically embed themselves into the outer walls of your cells and even into the membranes of your mitochondria, the tiny power plants inside each cell. EPA slots into membranes in an extended shape that stabilizes the cell wall and helps protect it from oxidative damage. DHA, which is slightly larger, interacts differently and tends to make membranes more fluid, which is especially important in brain and eye tissue.
Once embedded, these fatty acids change how your cells communicate. They reorganize small clusters of fat and cholesterol on the cell surface that act as signaling hubs, influencing how immune cells activate and respond. EPA and DHA also bind to receptors on immune cells that dial down the activity of a key inflammatory switch inside the cell, reducing the production of inflammatory signals at the genetic level.
Perhaps the most important thing omega-3s do is shift your body’s inflammatory balance. They compete with omega-6 fats (abundant in vegetable oils and processed food) for the same enzymes. When omega-3s win that competition, your body produces fewer of the compounds that promote blood clotting, constrict blood vessels, and drive inflammation. Instead, omega-3s get converted into specialized molecules called resolvins, protectins, and maresins that actively help inflamed tissue heal and return to normal. This is why fish oil’s benefits show up across so many different conditions: the underlying mechanism, controlling inflammation at the cellular level, is relevant almost everywhere in the body.
Heart Health and Triglycerides
The cardiovascular effects of fish oil are its best-established benefit. At prescription-strength doses (about 4 grams per day of combined EPA and DHA), fish oil lowers triglycerides by roughly 20 to 30 percent in people with elevated levels, and by 30 percent or more in people with very high triglycerides. The American Heart Association recommends about 1 gram per day of EPA plus DHA for people with documented coronary heart disease, ideally from oily fish like salmon, mackerel, or sardines.
EPA in particular appears to protect the heart through several routes beyond triglyceride reduction. It integrates into HDL (“good” cholesterol) particles and acts as an antioxidant, scavenging harmful reactive molecules. It also stabilizes cell membranes in ways that may protect against irregular heart rhythms. One important nuance: in people with very high triglycerides, combination EPA and DHA products can raise LDL cholesterol by 15 to 36 percent, though this effect is not significant at moderate triglyceride levels.
Depression and Mood
Fish oil has a meaningful effect on depressive symptoms, particularly in people who already have depression. A large meta-analysis published in the British Journal of Nutrition found that each additional gram per day of omega-3s produced a significant improvement in depression severity, with the strongest benefits at doses around 1.5 grams per day. Beyond that dose, returns diminish. Among people with existing depression, omega-3 supplementation increased the odds of remission by 148 percent compared to placebo, translating to roughly 19 additional remissions per 100 patients.
The effect was smaller but still present in people without a depression diagnosis, suggesting omega-3s support mood broadly rather than only treating clinical illness. Fish oil does not, however, appear to prevent depression from developing in the first place. If you’re already in a good place mentally, supplementation is unlikely to change that. The benefit is strongest as a complement to existing treatment for people who are struggling.
Joint Pain and Stiffness
For people with inflammatory joint conditions like rheumatoid arthritis, fish oil taken consistently for three to four months reduces morning stiffness, the number of painful joints, and overall pain intensity. A meta-analysis of trials on inflammatory joint pain found that omega-3 supplementation also reduced the use of NSAIDs like ibuprofen, with a moderate effect size. The reductions aren’t dramatic enough to replace conventional treatment, but they can meaningfully improve daily comfort and reduce reliance on pain medications that carry their own risks with long-term use.
Pregnancy and Infant Development
DHA is a structural component of brain and retinal tissue, which makes it especially important during pregnancy and early infancy when those systems are developing rapidly. Infants who received DHA-supplemented formula had significantly better visual acuity than those on standard formula. On developmental assessments, DHA supplementation was linked to modestly higher scores on motor development, though the effect on mental development scores was not statistically significant. The benefits were most consistent when DHA was given directly to infants rather than only through maternal supplementation, suggesting that the timing and directness of delivery matters.
Bleeding Risk Is Lower Than You Think
One of the most persistent concerns about fish oil is that it thins the blood and increases surgical bleeding risk. Many surgeons still ask patients to stop fish oil before procedures. But the largest analysis to date, published in an American Heart Association journal, found no increase in perioperative bleeding among fish oil users. In fact, the fish oil group required fewer blood transfusions than the placebo group. The researchers concluded that surgeries should not be delayed for patients taking fish oil. That said, if your surgeon asks you to stop, it’s worth discussing the evidence rather than simply ignoring the request.
How to Get the Most From Fish Oil
Fish oil comes in two main chemical forms, and the difference matters more than most people realize. The natural triglyceride form is absorbed significantly better than the ethyl ester form, which is the cheaper and more common type found in many supplements. In one comparison, EPA absorption from the triglyceride form was 68 percent versus just 20 percent from ethyl esters when taken without food. The body’s digestive enzymes break down ethyl esters 10 to 50 times more slowly than triglycerides, which explains the gap. Triglyceride-form oils are also more chemically stable and less prone to oxidation inside the capsule.
Regardless of form, taking fish oil with a meal that contains fat dramatically improves absorption. Eating it with a fat-containing meal boosted EPA absorption from the triglyceride form to 90 percent. For ethyl esters, the improvement was even more striking: absorption roughly tripled, jumping from about 20 percent to 60 percent, though this still fell short of the triglyceride form taken without food. If you take fish oil on an empty stomach, particularly an ethyl ester product, you may be absorbing very little of what you’re paying for.
How Much to Take
There is no official omega-3 recommendation for the general healthy population. The American Heart Association recommends eating fatty fish at least twice per week, which provides roughly 500 milligrams of EPA and DHA per day. For people with heart disease, the target is about 1 gram per day of combined EPA and DHA. For lowering high triglycerides, the effective dose in clinical trials is 4 grams per day, which typically requires a prescription product.
For mood support, the research points to a sweet spot around 1 to 1.5 grams per day, with diminishing returns above 2 grams. When checking supplement labels, look at the actual EPA and DHA content per serving, not the total “fish oil” amount. A 1,000-milligram fish oil capsule often contains only 300 milligrams of actual EPA and DHA, meaning you’d need three or more capsules to reach a meaningful dose.
