A knee with fluid buildup looks noticeably larger and puffier than your other knee. The normal bony contours and skin creases around the kneecap disappear, replaced by a smooth, swollen appearance that can make the joint look almost balloon-like. A healthy knee contains only about 3 to 4 milliliters of fluid, roughly less than a teaspoon. When that volume increases from injury, arthritis, or infection, the change is often visible to the naked eye.
How a Swollen Knee Looks Compared to Normal
The easiest way to spot fluid on the knee is to compare it to your other knee. Stand in front of a mirror with both legs straight and look for asymmetry. A knee with excess fluid will look puffy on the sides and above the kneecap, filling in the natural hollows that normally sit on either side of the kneecap’s edges. Those small indentations, which are easy to see on a healthy knee, get smoothed out as fluid accumulates inside the joint capsule.
The skin over the swollen knee often looks stretched and shiny. In lighter skin tones, it may appear slightly pink or flushed. In darker skin tones, the color change can be subtler, sometimes showing as a darkening or slight purple hue. The kneecap itself can look like it’s sitting in a cushion of fluid rather than resting snugly against the bone underneath. When significant fluid is present, the knee naturally holds itself in a slightly bent position, typically around 15 to 25 degrees of flexion, because that’s the angle where the joint capsule has the most room to accommodate the extra volume.
Where the Swelling Sits Matters
Fluid inside the knee joint itself produces a generalized, diffuse swelling that wraps around the entire kneecap. The whole knee looks uniformly bigger. This is different from bursitis, where a fluid-filled sac outside the joint becomes inflamed and creates a very localized bump. Prepatellar bursitis, sometimes called housemaid’s knee, produces a distinct egg-shaped swelling right on top of the kneecap. You can often see exactly where the swollen area starts and stops because the surrounding tissue looks normal. Another common type, pes anserine bursitis, causes swelling on the inner side of the knee just below the joint line.
If your swelling is concentrated in one specific spot rather than spread across the whole joint, bursitis is more likely than fluid inside the joint capsule. Both can make the knee warm and tender to the touch, but the visual pattern is distinct.
A Simple Test You Can Try at Home
You can check for mild fluid by doing a version of what clinicians call the bulge sign. Sit with your leg straight and relaxed. Using the flat of your hand, stroke firmly downward along the inside of your knee two or three times, pushing any fluid toward the outer side. Then quickly stroke down the outside of the knee and watch the inner side. If you see a small wave or bulge of fluid ripple back across the inner surface, there’s excess fluid in the joint.
For larger amounts of fluid, press down on the kneecap with one or two fingers while your leg is straight. In a normal knee, the kneecap sits firmly against the thighbone. If there’s significant fluid underneath, the kneecap will feel like it’s floating and will bounce or tap against the bone when you push it. That spongy, bouncy sensation confirms the joint is holding extra fluid.
What the Fluid Itself Looks Like
You can’t see the fluid through your skin, but if a doctor drains it with a needle (a procedure called aspiration), the color and consistency reveal a lot about what’s causing the problem.
- Clear and straw-colored: Normal synovial fluid is nearly colorless and slightly sticky, like egg whites. Fluid that’s clear but increased in volume often points to osteoarthritis or a minor injury. It means there’s little to no active inflammation.
- Cloudy or yellow: Cloudiness comes from white blood cells flooding the joint in response to inflammation. This is typical during acute flares of gout or similar crystal-related conditions.
- Milky white or paste-like: Some gouty joints produce fluid with visible white chunks or a thick, milky consistency. This is caused by heavy crystal deposits.
- Bloody or red-tinged: Blood in the joint fluid usually signals a ligament tear, fracture, or a condition that damages the joint lining. A sudden, bloody swelling after a knee injury is a strong indicator of structural damage.
- Green or dark yellow and opaque: Pus-like fluid suggests a joint infection, which is a medical emergency.
How Different Causes Change the Appearance
Osteoarthritis produces a gradual, mild-to-moderate swelling that tends to come and go. The knee looks a bit puffy but rarely turns red or hot. Pain worsens with activity, and you might hear clicking or grinding when you bend the joint. The swelling develops slowly over days or weeks.
Gout and other inflammatory conditions look dramatically different. The knee can swell rapidly over hours, turning red and visibly inflamed. The skin may look stretched tight and feel hot to the touch. The pain is intense and often comes on suddenly, sometimes overnight. This combination of rapid swelling, redness, and heat distinguishes inflammatory causes from the slower, less angry-looking swelling of wear-and-tear arthritis.
A knee infection, or septic arthritis, produces swelling alongside warmth, skin color changes, and severe pain that comes on fast. You may also have a fever. The joint can look similar to a gout flare, which is why infections require urgent medical evaluation to distinguish from other causes. An infected joint that goes untreated can cause permanent damage within days.
What It Feels Like
Beyond appearance, fluid on the knee creates a sensation of tightness or pressure, as if the joint is being inflated from the inside. Bending the knee fully becomes difficult because the extra fluid takes up space and resists compression. Straightening the knee completely can feel equally restricted. Many people describe a heavy, stiff feeling that’s worst after sitting still for a while and loosens slightly with gentle movement.
With larger amounts of fluid, you may feel the liquid shifting inside the joint when you change position. Some people notice a sloshing sensation when walking or climbing stairs. The knee can feel unstable or wobbly, not because the ligaments are damaged, but because the fluid changes how the kneecap tracks and how weight distributes across the joint.
