What Does Folic Acid Do for Men: Sperm, Heart & More

Folic acid supports several key functions in men’s health, from DNA repair and sperm quality to cardiovascular protection and mood regulation. The recommended daily intake for adult men is 400 mcg, with an upper limit of 1,000 mcg from supplements or fortified foods. While folic acid is often associated with pregnancy, men have their own compelling reasons to pay attention to this B vitamin.

Sperm Quality and DNA Integrity

The most well-studied benefit of folic acid for men relates to fertility. Folate plays a direct role in DNA synthesis and repair, which matters because sperm cells are constantly being produced and their genetic integrity is critical for successful conception.

A randomized controlled trial published in the Journal of Clinical Medicine tested high-dose folic acid supplementation in infertile men undergoing IVF. After three months, men taking folic acid saw a significant drop in sperm DNA fragmentation, from 8.5% to 6.4%. The placebo group showed no change. Interestingly, conventional sperm measures like count and motility didn’t improve, but the reduction in DNA fragmentation is independently associated with better fertility outcomes. DNA fragmentation is one of those hidden factors that standard semen analysis doesn’t always catch, yet it can be a major reason couples struggle to conceive.

If you and your partner are trying to conceive, or if you’re planning to in the near future, starting folic acid supplementation at least three months before is a reasonable step. Sperm take roughly 74 days to fully develop, so the benefits need time to take effect.

Heart Health and Blood Vessel Function

Folate helps your body break down homocysteine, an amino acid that circulates in your blood. When homocysteine builds up, it damages blood vessel walls and raises the risk of heart disease. Folate acts as a key ingredient in the chemical reaction that converts homocysteine back into methionine, a harmless amino acid your body can use.

Data from two large studies, the Nurses’ Health Study and the Kuopio Ischemic Heart Disease Risk Factor Study, found that the top 20% of folate consumers had significantly less cardiovascular disease than those with the lowest intake. But the cardiovascular benefits go beyond just lowering homocysteine. Research from the American Heart Association shows that folate directly improves how blood vessels function by interacting with the enzyme that produces nitric oxide, a molecule that keeps arteries relaxed and flexible. Folate also reduces the production of damaging free radicals inside blood vessel walls. These effects occur independently of any change in homocysteine levels, which means folate is doing more for your arteries than a single blood marker would suggest.

Mood and Brain Function

Low folate levels are linked to depression, anxiety, and cognitive decline. Folate is essential for producing neurotransmitters that regulate mood, including serotonin and dopamine. When levels drop low enough to cause folate-deficiency anemia, symptoms can include problems with memory, understanding, and judgment. Psychological effects range from mild depression and anxiety to, in severe cases, confusion.

Men are less likely than women to have their folate levels checked routinely, which means deficiency can go unnoticed. If you’ve been experiencing persistent low mood, fatigue, or brain fog alongside a diet low in leafy greens, legumes, and fortified grains, low folate is worth considering as a contributing factor.

Hair and Premature Graying

There’s a modest but real connection between folate and hair health. Folate-deficiency anemia can cause thinning hair and changes in hair and nail pigmentation. A 2017 study of 52 adults with premature graying found deficiencies in folic acid, biotin, and vitamin B12. This doesn’t mean folic acid supplements will reverse gray hair or regrow lost hair, but it does suggest that adequate folate is one piece of maintaining healthy hair follicle function. If you’re noticing early graying or unusual hair thinning, a B-vitamin panel is a reasonable thing to explore.

Folic Acid vs. Methylfolate

You’ll see two main forms sold as supplements: folic acid (the synthetic form) and methylfolate (sometimes labeled as 5-MTHF or L-methylfolate, the form your body actually uses). When you swallow folic acid, your intestinal cells have to convert it into methylfolate before it can enter your bloodstream. A study in The Journal of Nutrition tested both forms head-to-head in men and found their short-term bioavailability was equivalent. Peak blood levels were nearly identical (33.4 vs. 31.8 nmol/L), and the overall absorption curves matched.

The difference shows up at higher doses. Your gut has a limited capacity to convert folic acid, so large doses can result in unmetabolized folic acid floating around in your blood. The long-term significance of this isn’t fully settled, but methylfolate bypasses the conversion step entirely, which some people prefer. Methylfolate also has a practical safety advantage: it won’t mask the anemia caused by vitamin B12 deficiency the way folic acid can (more on that below). For most men taking a standard dose, either form works fine.

How Much You Need and Safety Concerns

Adult men need 400 mcg of dietary folate equivalents (DFE) per day. You can get this from food sources like spinach, black-eyed peas, asparagus, Brussels sprouts, and fortified cereals and breads. In the United States, enriched grain products have been fortified with folic acid since 1998, so many men get a baseline amount without thinking about it.

The tolerable upper limit from supplements or fortified foods is 1,000 mcg per day. There’s no upper limit set for folate from whole foods because no adverse effects from food-based folate have been reported. The main safety concern with supplemental folic acid is that it can mask a vitamin B12 deficiency. High-dose folic acid corrects the anemia that B12 deficiency causes, making blood work look normal while neurological damage from B12 deficiency quietly progresses. This is particularly relevant for men over 50, vegetarians, and vegans, all of whom face higher risk of B12 deficiency.
