Folic acid supports several critical functions in women’s bodies, from building healthy DNA to producing red blood cells to protecting a developing baby’s brain and spine. It’s a synthetic form of folate (vitamin B9), and women of childbearing age need at least 400 micrograms daily. But its benefits extend well beyond pregnancy, playing roles in heart health, mood regulation, and energy levels throughout every stage of life.
How Folic Acid Works in Your Cells
Folic acid’s most fundamental job is helping your body build and repair DNA. Every time a cell divides, it needs to copy its entire genetic blueprint, and folate is essential for producing one of DNA’s building blocks (thymidine). When folate runs low, the wrong molecule gets swapped into your DNA, which can lead to errors in cell division and eventually damage tissues that turn over quickly, like blood cells and the lining of your gut.
Folate also serves as a key player in a process called methylation, which controls how your genes are expressed. Think of methylation as a system of on/off switches across your DNA. The most well-known benefit of folic acid for women is its ability to prevent neural tube defects, serious birth defects of the brain and spine like spina bifida and anencephaly. The neural tube forms in the first 28 days after conception, often before a woman even knows she’s pregnant. That’s why health guidelines recommend all women of reproductive age take 400 mcg of folic acid daily, not just those actively trying to conceive.
The evidence behind this recommendation is strong. After the United States began fortifying grain products with folic acid in the late 1990s, the prevalence of neural tube defects dropped by 28%. The severity of cases also declined dramatically, with a 70% reduction in the most serious upper-spine lesions. These numbers represent one of the clearest public health wins from a single vitamin intervention.
During pregnancy, the recommended intake rises to 600 mcg daily to support the rapid cell division happening in both the mother’s body and the developing fetus. Breastfeeding women need about 500 mcg daily.
Fertility and Trying to Conceive
Folic acid appears to play a role before conception, too. Women who consume higher levels of folic acid may experience fewer ovulation problems, which is one of the more common barriers to getting pregnant. While folic acid alone isn’t a fertility treatment, ensuring adequate levels removes one potential obstacle. Since the neural tube closes so early in pregnancy, starting supplementation at least one month before conception (ideally three months) gives your body time to build up sufficient folate stores.

This connection has been well documented in research. Women with low folate levels are more likely to experience depressive symptoms, and some studies have found that folate supplementation can enhance the effectiveness of antidepressant medications. This doesn’t mean folic acid is a standalone treatment for depression, but it does mean that a deficiency can make mood disorders harder to manage.
Menopause and Hot Flashes
Preliminary research suggests folic acid may help reduce hot flashes in postmenopausal women. The proposed mechanism is similar to how hormone replacement therapy works: folic acid interacts with the same neurotransmitter pathways (serotonin and norepinephrine) that regulate body temperature in the brain. A small study of 46 postmenopausal women found promising results with daily folic acid supplementation over four weeks, though researchers have noted that larger, more rigorous trials are needed before this can be considered a reliable alternative to conventional treatment.
Folate From Food vs. Supplements
Natural folate is found in dark leafy greens, legumes, citrus fruits, and liver. Folic acid is the synthetic version used in supplements and fortified foods like bread, cereal, and pasta. Your body absorbs folic acid more efficiently than natural food folate, which is one reason fortification programs have been so effective.
The recommended daily intake for adult women is 400 mcg. Most women eating a varied diet that includes fortified grains will meet this baseline, but supplementation becomes important during pregnancy, when trying to conceive, or when dietary intake is inconsistent.
Safety and Upper Limits
The safe upper limit for folic acid from supplements and fortified foods is 1,000 mcg per day for adults. This cap exists for an important reason: high doses of folic acid can mask a vitamin B12 deficiency. Folic acid corrects the anemia that B12 deficiency causes, making blood tests look normal, but it does nothing to stop the nerve damage that B12 deficiency also causes. If the deficiency goes undetected for too long, permanent damage to the brain, spinal cord, and nerves can result.
This is particularly relevant for women over 50, vegetarians, and vegans, who are at higher risk for B12 deficiency. If you’re taking a folic acid supplement, it’s worth ensuring your B12 intake is also adequate. Many prenatal and multivitamins include both for this reason.
