GERD chest pain most commonly feels like a burning sensation behind your breastbone that can range from mild warmth to intense, radiating heat. But burning is only one version. Some people experience pressure, tightness, or a squeezing feeling that can be surprisingly hard to distinguish from heart-related pain. Episodes last anywhere from a few seconds to a few hours, depending on the trigger and severity.
The Burning Sensation
The hallmark feeling is what most people call heartburn: a hot, burning discomfort that starts behind the breastbone and can creep upward toward your throat. It often begins in the lower chest and spreads, sometimes reaching the neck or jaw. The sensation comes from stomach acid washing back into the esophagus, which lacks the protective lining your stomach has. That direct contact between acid and exposed tissue creates the burn.
The intensity varies widely. Some episodes feel like mild warmth or irritation you can mostly ignore. Others produce a searing heat that makes it hard to focus on anything else. Eating a large or fatty meal, drinking alcohol or coffee, or consuming acidic foods like tomato sauce or citrus can all intensify the sensation. Bending over or lying down shortly after eating tends to make it worse because gravity is no longer helping keep acid where it belongs.
When It Doesn’t Feel Like Burning
Not everyone with GERD gets the classic burn. Some people feel chest pressure or tightness that mimics what you might imagine a heart problem feels like. This is called noncardiac chest pain, and it accounts for a significant number of emergency room visits each year. The pain can feel sharp, dull, or squeezing. It may sit right in the center of your chest or shift slightly to one side.
Another common but less expected symptom is globus sensation: the persistent feeling that something is stuck in your throat, even when nothing is there. Acid irritating the lining of the esophagus can trigger throat tightness and a lump-like sensation that radiates into the upper chest. This version of GERD often confuses people because it doesn’t match the “heartburn” label at all. They may worry about swallowing problems or throat conditions when acid reflux is actually driving the discomfort.
Timing and Triggers
GERD chest discomfort follows predictable patterns. The most common trigger window is 30 minutes to two hours after eating, when your stomach is producing the most acid to digest food. Lying down during this period makes symptoms significantly worse because the flat position reduces your swallowing frequency, which is one of your body’s main ways of clearing acid from the esophagus.
Nighttime is especially problematic. In a large U.S. survey of nearly 12,000 people with GERD, 88.9% reported nighttime symptoms. The supine sleeping position allows acid to pool in the esophagus for longer stretches, which is why many people wake up with chest burning or a sour taste in their mouth. Eating at least three to four hours before bed, sleeping on your left side, and elevating the head of your bed all reduce the likelihood of overnight episodes.
Specific foods and habits act as triggers for different people. Common culprits include spicy or fried foods, chocolate, peppermint, carbonated drinks, and alcohol. Tight clothing around the waist, stress, and smoking can also provoke episodes. Tracking which situations bring on your chest symptoms helps you identify your personal pattern.
How It Differs From Heart Pain
The overlap between GERD chest pain and cardiac chest pain is real and sometimes impossible to sort out on your own. Both can cause pressure or burning behind the breastbone. Both can come and go. Both can subside on their own within minutes or hours, which is why a short episode that resolves is not automatically safe to dismiss.
There are some general patterns that help distinguish the two. GERD pain typically worsens after meals, when lying flat, or when bending over. It often improves with antacids or sitting upright. Cardiac pain is more likely to come on with physical exertion, radiate into the left arm, jaw, or back, and be accompanied by shortness of breath, sweating, or lightheadedness. But these patterns have plenty of exceptions. If you experience unexplained chest pain, especially with those additional symptoms, treating it as potentially cardiac until proven otherwise is the safer approach.
How Night Symptoms Affect Daily Life
GERD’s chest sensations don’t just cause momentary discomfort. When symptoms hit at night, the downstream effects on daily functioning are substantial. Among people with nighttime GERD symptoms, 68.3% report sleep difficulties. About half have trouble falling asleep, and 58.3% have trouble staying asleep. Waking up with a burning chest or acid taste disrupts sleep architecture in ways that carry into the next day.
Those sleep disruptions translate into measurable real-world consequences: roughly one extra healthcare visit per year, a 5.5% increase in overall work impairment, and a 10.9% increase in impairment during daily activities. Mental health takes a hit too. People with sleep-disrupting GERD score lower on both physical and mental health quality measures compared to those whose symptoms stay in the daytime. If your chest discomfort is regularly waking you up or preventing you from falling asleep, that pattern itself is a signal that your reflux needs better management.
Recognizing a Pattern
Doctors often use a simple scoring tool called the GerdQ to assess whether your symptoms point toward GERD. One of its core questions asks how many days per week you experience a burning feeling behind your breastbone. Scores of 8 or higher (out of 18) indicate a high likelihood of GERD. The questionnaire also asks about regurgitation, nausea, and sleep disruption to build a fuller picture.
If you’re noticing chest burning or pressure several times a week, especially tied to meals or lying down, that frequency pattern is diagnostically meaningful. A single episode after a heavy meal is common and not necessarily a concern. But recurring symptoms, particularly ones that wake you at night or that you’re adjusting your eating habits around, suggest something worth addressing beyond occasional antacid use. In some cases, a pH monitoring test can confirm the diagnosis and measure how much acid is actually reaching your esophagus, which helps guide the level of treatment you need.
