A high hemoglobin A1c means your blood sugar has been elevated over the past two to three months. The A1c test measures the percentage of your red blood cells that have glucose stuck to them, and a result of 6.5% or higher indicates diabetes, while 5.7% to 6.4% falls in the prediabetes range. Below 5.7% is considered normal.
How the A1c Test Works
Glucose in your bloodstream naturally attaches to hemoglobin, the oxygen-carrying protein inside red blood cells. The more glucose circulating in your blood, the more hemoglobin gets coated. Since red blood cells live about three months before your body replaces them, an A1c test captures a rolling average of your blood sugar over that entire lifespan. That’s what makes it more useful than a single finger-stick glucose reading, which only shows what’s happening in the moment.
The result is reported as a percentage. An A1c of 7% corresponds to an estimated average blood sugar of roughly 154 mg/dL. Each percentage point above that reflects meaningfully higher average glucose levels and a greater risk of complications over time.
What the Numbers Mean
The diagnostic thresholds set by the American Diabetes Association break down like this:
- Below 5.7%: Normal blood sugar regulation
- 5.7% to 6.4%: Prediabetes, meaning blood sugar is higher than normal but not yet in the diabetes range
- 6.5% or higher: Diabetes
If you already have a diabetes diagnosis, the general target is an A1c of 7% or less. Some people can reach below 6.5% through a combination of lifestyle changes and medication. Your specific target may differ depending on your age, how long you’ve had diabetes, and other health conditions.
Why a High A1c Matters for Your Health
Persistently high blood sugar damages blood vessels, both the tiny ones that supply your eyes, kidneys, and nerves, and the larger ones feeding your heart and brain. About half of all people with diabetes eventually develop complications in the small blood vessels, which can show up as vision loss (retinopathy), kidney damage, or numbness and pain in the hands and feet (neuropathy). These risks climb as A1c rises and stays elevated.
The damage isn’t limited to small vessels. High A1c is also linked to heart attack, stroke, and peripheral artery disease. Research published in the Journal of the American Heart Association found that people with both types of diabetes who maintained poor blood sugar and blood pressure control had significantly higher rates of complications. Those with type 1 diabetes faced a threefold increase in risk, while those with type 2 had roughly a 1.5 times higher risk compared to people with well-controlled levels.
The encouraging finding from that same research: when people kept their A1c below 6.5% and their blood pressure under control, the connection between large-vessel damage and subsequent small-vessel complications weakened considerably. In the well-controlled group, having heart or artery disease did not independently increase the risk of developing eye, kidney, or nerve problems. That’s a strong case for keeping A1c as close to target as possible.
When the A1c Result Can Be Misleading
Certain conditions can push your A1c reading higher or lower than your actual blood sugar average, which is worth knowing before you interpret a result in isolation.
Iron deficiency anemia is one of the most common causes of a falsely elevated A1c. When you’re low on iron, your red blood cells live longer than usual, giving glucose more time to accumulate on hemoglobin. This can make blood sugar control look worse than it really is. The same effect occurs in late pregnancy, when iron deficiency is common, and can cause a higher A1c reading even in women without diabetes.
On the other hand, any condition that shortens the life of your red blood cells will make your A1c appear falsely low. This includes hemolytic anemia, recent significant blood loss, and certain genetic hemoglobin variants like sickle cell disease. People with sickle cell trait or disease face particularly unreliable A1c readings due to chronic anemia, faster red cell turnover, and the structural differences in their hemoglobin. Kidney failure can also interfere. Patients on dialysis often show A1c values that underestimate their true blood sugar levels, and alternative markers like glycated albumin may give a more accurate picture.
If any of these conditions apply to you, a single A1c number may not tell the full story. Continuous glucose monitoring or other blood sugar tests can fill in the gaps.
How Long It Takes to Lower Your A1c
Because the test reflects a three-month window, changes in diet, exercise, or medication won’t show up in your next A1c reading overnight. It typically takes two to three months of consistent effort before you see a meaningful drop. That’s not a delay in progress. It’s the biology of the test itself: your body needs to cycle through a new generation of red blood cells that were exposed to lower glucose levels before the number shifts.
Most people retest every three months to track their trajectory. A drop of even 0.5% to 1% over one testing cycle reflects real improvement in daily blood sugar management and translates to measurably lower complication risk over the years ahead.
