High protein in urine, called proteinuria, means your kidneys are letting protein slip through that they’d normally keep in your bloodstream. A healthy kidney filters out less than 150 milligrams of protein per day. When that number climbs higher, it can signal anything from a temporary response to exercise or fever to an early warning sign of kidney disease, diabetes, or other chronic conditions.
A single abnormal reading doesn’t necessarily mean something is wrong. But persistent protein in your urine, confirmed across multiple tests, deserves attention because it can reflect real damage to your kidney’s filtering system.
How Your Kidneys Normally Keep Protein In
Your kidneys filter about 50 gallons of blood every day through tiny structures called glomeruli. Each one contains a fine mesh membrane that acts like a molecular sieve, letting water and small waste products pass through while holding back larger molecules like albumin, the most abundant protein in your blood. The mesh carries a negative electrical charge that actively repels albumin, adding a second layer of protection beyond size alone.
Wrapped around that membrane are specialized cells called podocytes, which extend tiny “feet” that interlock to form yet another barrier. When disease, high blood sugar, or high blood pressure damages any part of this system (the membrane, the electrical charge, or the podocytes), proteins start leaking through into your urine. In animal studies, albumin leakage begins as soon as the membrane’s structure is disrupted, even before the podocyte layer shows visible damage. That’s why protein in urine can be one of the earliest detectable signs of kidney trouble.
Temporary Causes That Usually Aren’t Serious
Not every positive test means kidney damage. Transient proteinuria comes and goes, and it’s the most common reason for a one-time abnormal result. Known triggers include:
- Intense exercise: Heavy workouts can temporarily increase protein filtration, which resolves within a day or two of rest.
- Fever and illness: Acute infections push more protein into the urine while your body fights them off.
- Dehydration: Concentrated urine can produce a higher protein reading even when the total amount your kidneys are leaking hasn’t changed.
- Emotional or physical stress: Stress hormones can temporarily alter kidney filtration.
There’s also a form called orthostatic proteinuria, where protein levels rise after you’ve been upright for a long time but return to normal after lying down. This is particularly common in younger people who are tall and slim. It’s considered benign and typically doesn’t require treatment.
Conditions That Cause Persistent Proteinuria
When protein keeps showing up in your urine across repeated tests, the cause is usually a condition that’s actively damaging your kidneys’ filtering system. The most common culprits are diabetes and high blood pressure, which together account for the majority of chronic kidney disease cases. Over time, elevated blood sugar stiffens and scars the glomerular membrane, while uncontrolled blood pressure physically strains those delicate filters.
Autoimmune disorders like lupus can cause the immune system to attack the kidneys directly. Kidney-specific diseases, including a group of conditions collectively called glomerulonephritis, inflame the filters and increase protein leakage. Nephrotic syndrome is a rarer but more severe form where the kidneys release very large amounts of protein, often enough to cause visible symptoms.
Proteinuria During Pregnancy
Protein in the urine takes on special significance during pregnancy. After 20 weeks of gestation, the combination of new-onset high blood pressure and proteinuria is one of the hallmark patterns used to diagnose preeclampsia. This condition affects blood flow to the placenta and can become dangerous for both mother and baby if it progresses. Quantitative urine protein measurement, either through a single urine sample ratio or a 24-hour collection, is part of the standard diagnostic workup. Pregnant women who notice sudden swelling in the face or hands, severe headaches, or vision changes alongside a high protein result should treat it as urgent.
What You Might Notice
Mild proteinuria rarely produces symptoms you can see or feel. Most people discover it through a routine urine test. As protein loss increases, though, certain signs become more apparent. Foamy or frothy urine that looks like the foam won’t settle is the most recognizable one. This happens because proteins act like a surfactant, creating bubbles when urine hits the water.
When protein loss becomes heavy, your blood loses its ability to hold fluid inside blood vessels. That fluid leaks into surrounding tissues, causing swelling (edema) in the ankles, feet, hands, and around the eyes, especially noticeable in the morning. Some people also experience unexplained weight gain from fluid retention or notice that their urine output changes.
How It’s Detected and Confirmed
The simplest screening tool is a urine dipstick, the test strip dipped into a urine sample during a routine checkup. It’s fast and inexpensive, but it has real limitations. Dipstick tests are good at ruling protein in when levels are already moderate to high: at the 1+ reading, sensitivity exceeds 95% for detecting significant protein loss. But they’re poor at catching early-stage leakage. For detecting mildly elevated albumin (the earliest sign of kidney damage), dipstick sensitivity drops to roughly 44%, meaning it misses more than half of cases at that stage.
That’s why a more precise test called the urine albumin-to-creatinine ratio (UACR) is the preferred follow-up. It measures the actual concentration of albumin relative to creatinine in a single urine sample, which corrects for how dilute or concentrated your urine is. A UACR below 30 mg/g is normal. Values between 30 and 300 mg/g indicate moderately increased albumin, and anything above 300 mg/g signals severely increased levels. A 24-hour urine collection, where you save all urine produced in a full day, provides the most comprehensive measurement but is less convenient.
Why Persistent Proteinuria Matters Long-Term
Chronic proteinuria isn’t just a marker of existing kidney damage. It accelerates further damage. Proteins passing through the kidney’s filtering system irritate and scar the tubules downstream, creating a cycle where leakage causes more injury, which causes more leakage.
The risks extend well beyond the kidneys. A large study tracking young adults found that those with proteinuria had a 71% higher risk of sudden cardiac arrest compared to those without it, after adjusting for other health factors. At higher levels of protein loss (3+ to 4+ on a dipstick), that risk nearly tripled. The combination of proteinuria and chronic kidney disease was especially dangerous: people with both conditions in advanced kidney disease stages faced a ninefold increase in cardiac arrest risk compared to those with neither.
These numbers reflect the broader pattern that proteinuria is a strong, independent predictor of cardiovascular problems. The same vascular damage that injures kidney filters tends to affect blood vessels throughout the body, including those feeding the heart and brain.
What Happens After an Abnormal Result
A single abnormal dipstick reading is typically repeated to rule out temporary causes. If a second test still shows protein, a UACR or 24-hour collection confirms the finding. From there, the focus shifts to identifying the underlying cause. Blood tests measuring kidney function, blood sugar, and markers of autoimmune activity are standard. In some cases, imaging or a kidney biopsy may be needed.
Treatment targets the root cause. For diabetes-driven proteinuria, tighter blood sugar control and specific blood pressure medications that reduce pressure inside the kidney’s filters can significantly slow protein leakage. For autoimmune causes, immunosuppressive therapy addresses the inflammation. Across all causes, controlling blood pressure is one of the most effective ways to reduce proteinuria and protect remaining kidney function. Dietary changes, particularly reducing sodium and sometimes moderating protein intake, can help lower the burden on damaged kidneys.
