What Does Hydrogen Peroxide Do to Teeth?

Hydrogen peroxide whitens teeth by breaking down the pigments trapped inside your enamel and the layer beneath it. It’s the active ingredient in nearly every whitening product on the market, from drugstore strips to professional treatments. But it does more than just brighten. Depending on the concentration and how long it sits on your teeth, it can also cause temporary sensitivity, gum irritation, and, with overuse, damage to your enamel.

How It Whitens Your Teeth

Tooth stains aren’t just sitting on the surface. Many are embedded within the structure of your enamel and dentin (the harder layer underneath). These stains come from organic molecules called chromophores, compounds that absorb light and give your teeth a yellow or brownish tint. Coffee, tea, red wine, and tobacco are common sources.

Hydrogen peroxide works by generating free radicals and reactive oxygen molecules that penetrate through the tiny prisms in your enamel and reach the dentin. Once inside, these reactive molecules attack the chromophores in two ways: they either break the chemical bonds that give the pigment its color or they oxidize the molecules into smaller, lighter-colored fragments that reflect less light. The result is teeth that look whiter, not because a coating has been applied, but because the internal staining has been chemically neutralized.

How deeply the peroxide penetrates depends on two things: concentration and contact time. A higher-concentration gel left on for a shorter period (typical of in-office treatments) and a lower-concentration gel worn for several hours (typical of at-home trays) can produce similar results through different routes. This is why a 10% carbamide peroxide tray worn overnight can match or even outperform a 6% hydrogen peroxide product used for a shorter session.

Hydrogen Peroxide vs. Carbamide Peroxide

Most whitening products use one of two forms of peroxide. Hydrogen peroxide is the active bleaching agent. Carbamide peroxide is a compound that breaks down into hydrogen peroxide and urea once it contacts your teeth, releasing the bleaching agent more slowly over time.

A 10% carbamide peroxide gel contains roughly 3.5% hydrogen peroxide. Because carbamide peroxide releases its peroxide gradually, products that use it are designed for longer wear times, often two to ten hours per session. This slower release is also why carbamide peroxide tends to cause less tooth sensitivity than an equivalent hydrogen peroxide product. At-home whitening systems typically range from 10% to 38% carbamide peroxide, with treatment periods lasting anywhere from six to 28 days depending on the concentration.

Tooth Sensitivity After Whitening

The most common side effect of hydrogen peroxide on teeth is sensitivity, and it happens for a straightforward reason. The peroxide temporarily weakens the outer enamel layer, which exposes the dentin underneath. Dentin contains microscopic tubes that lead to the nerve inside your tooth. When those tubes are exposed, cold drinks, hot food, or even a breath of cool air can trigger a sharp, uncomfortable zing.

For most people, this sensitivity lasts 24 to 48 hours after a whitening session. In some cases, mild discomfort can stretch to about three days. It resolves on its own as the enamel rehydrates and remineralizes. Using a sensitivity toothpaste containing potassium nitrate in the days before and after whitening can help blunt this effect.

What It Does to Your Gums

Hydrogen peroxide is a chemical oxidizer, and it doesn’t distinguish between tooth pigments and soft tissue. If the gel contacts your gums, you may notice redness, a white or blanched appearance, or a burning sensation. With properly fitted whitening trays or carefully applied strips, this contact is minimal. With ill-fitting trays or sloppy application, it’s more likely.

Mild gum irritation from whitening products typically fades within a few hours. If redness or soreness persists beyond that, or if you experience dizziness or nausea (signs of accidental ingestion), that warrants prompt medical attention. Swallowing undiluted hydrogen peroxide can burn internal tissues and cause serious harm.

The Risk of Overuse

Hydrogen peroxide is not safe to use on your teeth every day, especially at higher concentrations. Using it multiple days in a row without breaks can erode enamel rather than simply passing through it. Enamel doesn’t regenerate once it’s gone, so this kind of damage is permanent.

Practical limits look something like this:

  • At-home whitening kits: Limit use to seven to 14 sessions per treatment cycle, with rest days between sessions. No more than about 20 bleaching sessions per year.
  • In-office professional whitening: Once or twice per year is a reasonable ceiling.
  • DIY hydrogen peroxide rinses: If you’re swishing with a diluted solution (typically 1% to 3%), keep it brief and don’t do it daily. The concentration in a bottle of drugstore hydrogen peroxide (usually 3%) is significantly lower than professional whitening gels, but it can still irritate gums and weaken enamel with frequent use.

The temptation to whiten more often for faster results is common, but the chemistry doesn’t reward impatience. Overexposure strips away enamel, increases sensitivity, and can leave teeth looking translucent or chalky rather than white.

What You Can Realistically Expect

Hydrogen peroxide lightens teeth that are stained by food, drinks, or aging. It works best on yellow-toned discoloration and is less effective on gray or brown stains, which often have different causes (certain medications, trauma, or fluorosis). It also won’t change the color of crowns, veneers, or fillings, so if you have visible dental work, whitening can create a mismatch.

Results from at-home products typically become noticeable within a week or two of consistent use and can last several months before stains gradually return. In-office treatments produce more dramatic results in a single visit because they use much higher concentrations (sometimes 25% to 40% hydrogen peroxide) combined with heat or light to accelerate the reaction. But the underlying chemistry is identical: reactive oxygen breaks down pigments inside your tooth structure, and the effect is temporary if you continue exposing your teeth to staining substances.
