When your eyes are dilated, the black center of each eye (the pupil) expands noticeably, making it look like your eyes are mostly black with only a thin ring of color around the edges. In normal indoor lighting, pupils are roughly 2 to 4 millimeters across. When dilated, they can expand to 6, 7, or even 8 millimeters, overtaking most of the visible colored part of the eye. The overall effect is striking: your eyes look darker, wider, and sometimes a little glassy.
What Dilated Pupils Actually Look Like
The pupil is the round black opening in the center of your iris, the colored ring that gives your eyes their hue. Normally, the pupil takes up a relatively small portion of the visible eye. When it dilates, that black circle grows so large that only a slim border of iris color remains visible. In people with lighter eye colors like blue, green, or hazel, the change is especially obvious because the contrast between the dark pupil and the pale iris is hard to miss. In people with very dark brown eyes, dilation can be harder to spot at a glance since the pupil and iris are closer in color, but it’s still noticeable up close.
Both pupils usually dilate together and stay round. If one pupil is noticeably larger than the other, that’s called anisocoria, and it can look unsettling. A slightly oval or irregularly shaped dilated pupil can also occur in certain neurological conditions.
How Dilation Feels, Not Just How It Looks
Dilated pupils aren’t just a cosmetic change. Because the pupil controls how much light enters your eye (like the aperture on a camera), a wider opening floods the inside of your eye with more light than usual. That creates a few predictable effects: increased sensitivity to light, blurry vision (especially for things up close), and sometimes mild headaches. You may find yourself squinting outdoors or struggling to read your phone.
The blurriness happens because dilation interferes with your eye’s normal focusing reflex. Normally, your pupil shrinks slightly when you look at something nearby, helping sharpen the image. When the pupil is locked open or slow to respond, that fine-tuning doesn’t work as well, and close-up objects look soft or fuzzy.
Why Pupils Dilate After an Eye Exam
The most common reason people see their own dilated eyes is after a visit to the eye doctor. Dilating drops, typically containing medications like tropicamide or phenylephrine, temporarily paralyze the tiny muscle that constricts the pupil. This gives your doctor a wide-open window to examine the retina and the structures at the back of your eye.
The effects last anywhere from a few hours to most of the day, depending on the type of drop used and your individual response. Children and young adults tend to stay dilated longer. Your eye doctor can’t predict exactly how long the blur and light sensitivity will last for you, so it’s a good idea to bring sunglasses to your appointment. Driving may be uncomfortable or difficult until the drops wear off.
Emotions and Adrenaline
Your pupils don’t just respond to light. They also react to your emotional state. The sympathetic nervous system, the branch responsible for the fight-or-flight response, triggers pupil dilation when you’re stressed, scared, excited, or intensely focused. This is an involuntary reflex: a surge of adrenaline relaxes the muscle that keeps your pupil small while activating the muscle that pulls it open.
This is also why pupils dilate slightly when you’re attracted to someone or deeply engaged in a task. The change is subtler than what you’d see from eye drops or substances, but it’s real and measurable. When the stressor or emotional trigger passes, your parasympathetic nervous system (the “rest and digest” branch) takes over and your pupils return to their normal size.
Substances That Cause Noticeable Dilation
A number of recreational and prescription drugs cause pupils to dilate, sometimes dramatically. Stimulants are the most well-known culprits. Cocaine, amphetamines, methamphetamine, and MDMA (ecstasy) all produce rapid, pronounced pupil enlargement that’s often one of the first visible signs of use.
Hallucinogens have a similar effect. LSD causes marked dilation that can last for hours, and psilocybin mushrooms produce moderate to significant widening. Mescaline, ketamine, and even cannabis can enlarge the pupils to varying degrees.
On the prescription side, several common medication categories can cause mild to moderate dilation as a side effect:
- Antidepressants, particularly SSRIs and older tricyclic types, due to their effects on the signaling system that normally constricts the pupil
- ADHD medications like methylphenidate and amphetamine-based drugs
- First-generation antihistamines (the kind that cause drowsiness)
- Medications for overactive bladder or Parkinson’s disease that block the same nerve signals controlling pupil constriction
Medical Conditions That Affect Pupil Size
Sometimes dilated pupils aren’t caused by drops, emotions, or substances. They’re a sign of an underlying condition. One of the more recognized examples is Adie syndrome (also called tonic pupil), where one pupil is larger than normal and reacts very slowly to light. People with Adie syndrome often notice that the affected pupil looks slightly oval rather than perfectly round. Under magnification, segments of the iris muscle constrict while others don’t, creating an uneven, worm-like movement. Some people also notice difficulty reading or focusing on nearby objects because the eye’s focusing ability is weakened.
Third nerve palsy, a condition affecting one of the cranial nerves that controls eye movement and pupil size, can also cause one pupil to stay dilated. Other conditions linked to abnormal pupil dilation include Guillain-Barré syndrome, multiple sclerosis, diabetes, and chronic alcoholism.
When Uneven Pupils Are an Emergency
If you notice that one pupil is suddenly larger than the other, especially alongside a new headache, eye pain, vision loss, or confusion, that combination can signal a life-threatening problem. Brain aneurysms, strokes, brain tumors, and infections like meningitis can all cause one pupil to blow open while the other stays normal. This happens because rising pressure inside the skull compresses the nerve controlling that pupil.
A small, stable difference in pupil size is actually common and harmless in about 20% of the population. The red flag is a sudden, new change, particularly when paired with pain, weakness on one side of the body, slurred speech, or rapidly worsening vision. That warrants an emergency room visit immediately.
