High triglycerides mean your blood carries more fat than your body can efficiently use or clear, and that excess is quietly increasing your risk for heart disease, metabolic problems, and in extreme cases, a painful inflammation of the pancreas. What the Numbers Mean

Triglyceride results fall into four categories:

High triglycerides rarely exist in isolation. They tend to show up alongside a cluster of metabolic problems: higher blood sugar, lower HDL (“good”) cholesterol, elevated blood pressure, and more small, dense LDL particles. Together, these form what’s known as metabolic syndrome.

The link runs through insulin resistance. When your cells stop responding normally to insulin, your liver compensates by ramping up triglyceride production. That overproduction floods the bloodstream with fat-carrying particles and also contributes to fat buildup in the liver itself. Even a single session of vigorous exercise, whether cardio or resistance training, speeds up triglyceride clearance from the blood by 25 to 40 percent in the 12 to 24 hours that follow. Consistency matters more than intensity, but the evidence favors pushing yourself rather than staying at a leisurely pace.

On the dietary side, the highest-impact changes are cutting back on added sugars, refined carbohydrates, and alcohol. Losing even a modest amount of weight, if you’re carrying extra, directly reduces the liver’s triglyceride production. These changes can move your numbers meaningfully within a few months.

Medication

When triglycerides stay persistently elevated at 150 mg/dL or above despite lifestyle changes, current guidelines recommend evaluating your overall cardiovascular risk to decide whether medication makes sense. For most people in this range, the first consideration is a statin, which lowers both LDL cholesterol and overall cardiovascular risk. Statins don’t target triglycerides specifically, but they reduce the total burden of harmful particles in your blood.

Prescription omega-3 fatty acids are another option. At therapeutic doses (much higher than what’s in a standard fish oil supplement), they lower triglycerides by 27 to 45 percent. Over-the-counter fish oil supplements deliver far less, typically reducing triglycerides by only 5 to 10 percent at a dose of one gram of EPA and DHA per day. The gap between prescription and supplement-grade omega-3s is significant.

For people with extremely high triglycerides above 1,000 mg/dL due to a genetic condition called familial chylomicronemia syndrome, newer targeted therapies are now recommended to bring levels down and prevent pancreatitis.
