High cholesterol means there’s more cholesterol circulating in your blood than your body can use, and the excess is slowly building up inside your artery walls. It’s one of the strongest risk factors for heart disease and stroke, but it produces no symptoms on its own. Most people find out only through a routine blood test.

What matters most isn’t just the total number. The type of cholesterol that’s elevated, your other health conditions, and your age all shape what high cholesterol actually means for you personally.

What the Different Numbers Tell You

A standard cholesterol panel measures several things, and each one tells a different part of the story. LDL cholesterol, often called “bad” cholesterol, makes up most of the cholesterol in your blood and is the primary driver of artery damage. HDL cholesterol works in the opposite direction: it picks up excess cholesterol from your bloodstream and ferries it back to your liver, which flushes it out. Triglycerides are a type of fat your body uses for energy, and high levels often travel alongside high LDL.

When your doctor says your cholesterol is high, they’re usually most concerned about LDL. An LDL level of 160 mg/dL or above is generally considered elevated for someone at low risk of heart disease. At 190 mg/dL or higher, it’s classified as severe hypercholesterolemia and typically warrants treatment regardless of other factors. Total cholesterol above 200 mg/dL is considered abnormal, though the breakdown between LDL and HDL matters more than the total alone.

HDL tells you about your protection. Research from the American Heart Association found that HDL of at least 35 mg/dL cut the risk of ischemic stroke nearly in half compared to lower levels. Higher HDL is generally better, and low HDL can be a concern even when LDL looks acceptable.

What Happens Inside Your Arteries

High cholesterol is dangerous because of what it does over years, not days. When too much LDL circulates in your blood, it seeps into the walls of your arteries and gets trapped there. Once stuck, the LDL particles undergo chemical changes. They oxidize and clump together, which triggers your immune system to respond as if there’s an infection.

Your body sends immune cells called macrophages to clean up the modified cholesterol. But instead of resolving the problem, these cells gorge on cholesterol and become bloated “foam cells” that accumulate in the artery wall. The connection between high LDL and cardiovascular events is one of the most studied relationships in medicine. People with total cholesterol in the highest range (around 290 mg/dL) face roughly 60% greater risk of ischemic stroke compared to those with the lowest levels. For strokes caused specifically by atherosclerosis in large arteries, having very high LDL more than doubles the risk.

Genetic studies that isolate the effect of LDL from other lifestyle factors confirm the relationship is causal, not just a correlation. Each standard deviation increase in genetically determined LDL cholesterol raises the risk of large artery stroke by about 28%. Heart disease risk climbs even more steeply, since the coronary arteries feeding the heart are especially vulnerable to plaque buildup.

The flip side is equally important. Higher HDL is consistently linked to lower stroke risk, particularly for strokes involving small blood vessels deep in the brain. This is why your cholesterol panel needs to be read as a whole picture rather than fixating on one number.

Why Your Cholesterol Is High

For most people, high cholesterol comes from a combination of diet, activity level, and genetics. The dietary culprit that matters most may surprise you: it’s saturated fat, not cholesterol in food. A randomized crossover study published in the American Journal of Clinical Nutrition found that saturated fat intake was significantly correlated with LDL levels, while dietary cholesterol intake had no meaningful effect. In practical terms, the butter, cheese, and fatty meat in your diet raise your LDL more than the eggs do.

Carrying excess weight, being sedentary, and smoking all push cholesterol levels in the wrong direction. So does aging. Your liver becomes less efficient at clearing LDL from the blood as you get older, which is why cholesterol tends to creep up in middle age even without major diet changes.

For a smaller group, genetics play an outsized role. Familial hypercholesterolemia is an inherited condition that affects roughly 1 in 200 to 250 people worldwide, making it the most common inherited cardiovascular condition. People with this condition have LDL levels that are high from childhood, often well above 190 mg/dL, because their liver cells can’t pull LDL out of the blood efficiently. If your cholesterol is very high and you have a family history of early heart attacks (before age 55 in men or 65 in women), familial hypercholesterolemia is worth investigating. Even with this genetic predisposition, diet, exercise, and smoking status still strongly influence how high cholesterol climbs and how much damage it does.

Why You Don’t Feel It

High cholesterol produces no pain, no fatigue, no noticeable symptoms. This is what makes it so dangerous. Plaque can build for decades before it causes a problem, and the first sign is often a heart attack or stroke rather than a warning symptom.

There is one visible exception. Some people with very high cholesterol develop xanthelasmas: small, yellowish, flat or slightly raised patches of skin near the inner corners of the eyelids. These are cholesterol deposits under the skin, and having them strongly suggests elevated blood lipids. They’re harmless on their own but serve as a visible signal that something needs attention. Most people with high cholesterol, though, will never develop them.

How Doctors Decide What to Do About It

Modern guidelines don’t treat cholesterol numbers in isolation. Instead of simply labeling a level as “high” and prescribing medication, clinicians estimate your overall 10-year risk of having a cardiovascular event using calculators that factor in your age, blood pressure, cholesterol levels, diabetes status, and smoking history. For adults 40 to 75, this risk score drives the conversation about treatment.

At low risk (under 5% chance of a heart attack or stroke in the next 10 years), lifestyle changes are the first recommendation: reducing saturated fat, increasing physical activity, losing weight if needed. At intermediate risk (7.5% to 20%), a statin is more likely to be discussed. At high risk (20% or above), statin therapy is strongly recommended. For anyone with LDL at or above 190 mg/dL, medication is typically recommended regardless of other risk factors, because the cholesterol level alone signals significant danger.

For adults under 40, age itself keeps the 10-year risk score low, so medication is rarely started unless there’s a genetic condition like familial hypercholesterolemia. That doesn’t mean high cholesterol in your 20s or 30s is harmless. It means the damage is accumulating, and lifestyle changes made early have the biggest payoff over a lifetime.

What You Can Change

Reducing saturated fat is the single most effective dietary change for lowering LDL. This means eating less red meat, full-fat dairy, butter, and coconut oil, and replacing them with unsaturated fats from olive oil, nuts, avocados, and fatty fish. Soluble fiber from oats, beans, and fruits also helps by binding cholesterol in the gut before it reaches the bloodstream.

Regular aerobic exercise raises HDL and lowers triglycerides. The effect is modest, typically raising HDL by a few points, but it compounds over time and improves cardiovascular fitness in ways that go beyond cholesterol numbers alone. Losing even 5 to 10% of body weight can meaningfully improve your entire lipid panel if you’re carrying extra weight.

If lifestyle changes aren’t enough, or if your risk is high enough to warrant it, statins remain the most widely prescribed and studied option. They work by slowing cholesterol production in the liver, which forces the liver to pull more LDL out of the bloodstream. Most people tolerate them well, and they reduce cardiovascular events by 25 to 35% in large trials. Your cholesterol level after treatment matters more than where you started, which is why follow-up blood work is part of the process.
