Red eyes happen when tiny blood vessels on the surface of your eye widen and fill with more blood than usual. This is one of the most common eye complaints, accounting for roughly 6% of visits to general practitioners and about 15% of ophthalmology consultations. Most of the time, the cause is minor and resolves on its own. But in certain cases, redness signals something that needs prompt attention.
Why Eyes Turn Red
The white part of your eye is covered by a thin, transparent membrane packed with microscopic blood vessels. When something irritates or inflames the eye, your immune system responds by widening those vessels, flooding the area with blood carrying immune cells and protective molecules like histamine. That rush of blood is what you see as redness. It’s essentially the same process as skin turning red around a cut or bug bite, just more visible because the tissue over your eye is so thin.
This means redness itself isn’t a disease. It’s a signal that something is triggering your eye’s defense system, whether that’s a stray eyelash, a night of poor sleep, seasonal allergies, or a serious infection.
The Most Common Causes
Allergies
Allergic reactions are one of the most frequent reasons for red eyes. Pollen, pet dander, dust mites, and mold can all trigger the response. The hallmark of allergic eye irritation is clear, watery discharge along with mild redness. Itching may or may not be present, but when it is, it can be intense. Both eyes are usually affected at the same time.
Bacterial and Viral Infections
Pink eye (conjunctivitis) is the classic infectious cause, and it helps to know which type you’re dealing with. Bacterial infections typically produce a thick yellow or green discharge that can crust over your eyelashes overnight and make your lids stick together. Pain is usually minimal, but the eye can look dramatically red and swollen.
Viral infections look different. The discharge is thinner and more watery, and instead of the crusty mess, you get a gritty, sandy feeling as if something is stuck in your eye. Light sensitivity is common, ranging from moderate to severe. Viral conjunctivitis spreads easily and often starts in one eye before moving to the other within a few days.
Dry Eye
Dry eye is a chronic condition that creates a self-reinforcing cycle of irritation. When your tear film becomes unstable, whether from reduced tear production or poor tear quality, the surface of your eye dries out. That dryness triggers inflammation, which damages the cells responsible for producing healthy tears, which makes the dryness worse. In people with dry eye, the blood vessels on the eye’s surface actually increase in diameter and density, producing persistent low-grade redness that comes and goes or never fully resolves.
Screen Time
If your eyes are red after a long day at the computer, your blink rate is likely to blame. You normally blink a certain number of times per minute, but when staring at a screen, that rate drops by roughly two-thirds. Worse, the blinks you do manage are often incomplete, meaning your eyelids don’t fully close. Since blinking is what spreads moisture across your eye’s surface, hours of reduced blinking effectively mimics dry eye. The fix is simple but easy to forget: take regular breaks and consciously blink fully when you notice strain.
Contact Lenses
Contact lenses sit directly on the surface of your eye, and they can cause redness in several ways. Wearing them too long, sleeping in them, or not cleaning them properly can lead to a condition sometimes called contact lens-induced acute red eye, where you wake up with red, irritated eyes. More seriously, bacteria can become trapped between the lens and your cornea, leading to infections that cause pain, redness, and potentially lasting damage. If you wear contacts and develop sudden redness with pain, remove your lenses right away.
Less Common but More Serious Causes
Some causes of red eye need fast treatment to prevent vision loss.
Acute angle-closure glaucoma happens when fluid drainage inside the eye suddenly becomes blocked, causing pressure to spike. It produces severe eye pain, redness, blurred vision, halos or rainbow-colored rings around lights, headache, and nausea or vomiting. This is an emergency. Without treatment within hours, it can cause permanent vision loss.
Uveitis is inflammation of the middle layer of the eye. It can appear on its own or alongside autoimmune conditions like ankylosing spondylitis, psoriatic arthritis, or certain types of vasculitis. Symptoms come on suddenly: eye pain, redness, light sensitivity, blurred vision, and dark floating spots in your field of vision. Left untreated, uveitis can scar the internal structures of your eye.
When Red Eyes Need Urgent Care
Most red eyes don’t require a trip to the doctor. Mild redness from allergies, a poor night’s sleep, or dry air typically resolves with rest, cool compresses, or lubricating eye drops. But certain combinations of symptoms change the picture entirely.
Seek care right away if your red eye comes with any of the following: eye pain (not just irritation, but actual pain), blurry or reduced vision, strong sensitivity to light, or significant swelling. You should also get seen if symptoms last longer than a week, are getting worse rather than better, or your eye is producing heavy pus or mucus. A fever or body aches alongside eye redness can indicate a systemic infection that’s affecting the eye.
Why “Get the Red Out” Drops Can Backfire
Over-the-counter eye drops marketed to reduce redness work by forcibly constricting the blood vessels on your eye’s surface. They can make your eyes look whiter within minutes, but they come with a well-documented trap: rebound redness. When the drug wears off, the blood vessels bounce back wider than they were before, making your eyes look redder than the original problem. This creates a cycle where you need the drops at shorter and shorter intervals to get the same effect. In some cases, people develop noticeable rebound redness after as little as five days of regular use.
If you want temporary relief without this risk, preservative-free artificial tears lubricate the surface without constricting blood vessels. They won’t make your eyes look instantly whiter, but they address the underlying dryness or irritation without creating a dependency cycle. For allergic redness, antihistamine eye drops are a better choice than redness-removing drops because they target the actual cause rather than masking the symptom.
Simple Ways to Reduce Eye Redness
For everyday redness that isn’t caused by an infection or acute condition, a few changes can make a noticeable difference. Cool compresses over closed eyes for five to ten minutes constrict blood vessels naturally and soothe irritation. If you work at a screen for long stretches, follow the 20-20-20 pattern: every 20 minutes, look at something 20 feet away for 20 seconds. This gives your blink rate a chance to reset. Keeping your environment humidified helps if dry air is a factor, particularly in winter or air-conditioned offices.
For contact lens wearers, sticking strictly to the recommended wearing schedule and replacing lenses on time prevents a significant share of redness episodes. And if you’ve been reaching for redness-reducing drops more than a couple of times a week, that’s a sign the underlying cause needs attention rather than another round of drops.
