What Does It Mean If Your Heart Rate Is Low?

A resting heart rate below 60 beats per minute (bpm) is technically considered low, a condition called bradycardia. But that number alone doesn’t tell you much. A heart rate in the 40s can be perfectly healthy in one person and a sign of a serious problem in another. What matters is whether your heart is pumping enough blood to meet your body’s needs.

When a Low Heart Rate Is Normal

A resting heart rate between 40 and 60 bpm is common in healthy young adults, well-conditioned athletes, and during sleep. If you feel fine, have good energy, and aren’t experiencing dizziness or fatigue, a heart rate in this range generally isn’t a problem.

Athletes are the classic example. Years of endurance training cause the heart to become more efficient, pumping more blood with each beat so it doesn’t need to beat as often. Research published in Circulation found that this adaptation involves two things working together: the heart’s natural pacemaker physically remodels itself, and the nervous system increases its calming influence on heart rate. Interestingly, the same research found that genetics play a significant role. Many endurance athletes appear to carry inherited traits that predispose them to lower resting heart rates, which may have made them more suited to endurance sports in the first place. Former athletes often maintain low resting heart rates long after they stop training, suggesting these changes can be lasting.

When a Low Heart Rate Is a Problem

A slow heart rate becomes concerning when the heart can’t pump enough oxygen-rich blood to the brain and body. The symptoms reflect that oxygen shortage:

  • Dizziness or lightheadedness
  • Fainting or near-fainting
  • Unusual fatigue, especially during physical activity
  • Shortness of breath
  • Chest pain
  • Confusion or memory problems

If you’re experiencing any of these alongside a low heart rate, your body is telling you something isn’t right. The distinction between harmless and dangerous bradycardia almost always comes down to symptoms. A marathon runner at 42 bpm with no complaints is fine. Someone at 55 bpm who keeps nearly passing out needs evaluation.

Common Causes of a Low Heart Rate

Beyond fitness level, several things can slow your heart rate. Some are easily correctable, others require ongoing management.

Medications are one of the most common culprits. Beta blockers, a widely prescribed class of blood pressure and heart medications, work by deliberately slowing the heart and reducing the force of each beat. If you take one of these (common names include metoprolol, atenolol, or propranolol) and notice your heart rate dropping lower than expected, the medication is likely responsible.

Thyroid problems can also slow the heart. An underactive thyroid reduces your overall metabolism, and heart rate drops along with it. Electrolyte imbalances, particularly changes in potassium levels, affect the electrical signals that keep the heart beating at a steady pace. Sleep apnea, a condition where breathing repeatedly stops during sleep, can alter heart rhythm over time. And problems with the heart’s own electrical system, where the natural pacemaker or the wiring that carries signals through the heart degrades, become more common with age.

How a Low Heart Rate Is Evaluated

The main tool for investigating a slow heart rate is an electrocardiogram (ECG), which records the electrical activity of your heart through sensors placed on your chest. This test shows not just how fast the heart beats, but whether the electrical signals are traveling through it normally.

Because a slow heart rate can come and go, a single ECG in a doctor’s office sometimes misses the problem. In that case, you might wear a portable monitor. A Holter monitor records your heart’s activity continuously for a day or more as you go about normal life. An event recorder works similarly but is worn for up to 30 days. You press a button when you feel symptoms, and it captures what your heart was doing at that moment.

Blood tests typically check thyroid function and electrolyte levels to rule out metabolic causes. If fainting has been a problem, a tilt table test can help. You lie flat on a table that’s then tilted upright while your heart rate and blood pressure are monitored, revealing how your cardiovascular system handles positional changes. A sleep study may also be recommended if sleep apnea is suspected.

How Low Heart Rate Is Treated

Treatment depends entirely on the cause. If a medication is slowing your heart too much, adjusting the dose or switching to a different drug often solves the problem. If an underactive thyroid is responsible, treating the thyroid condition brings heart rate back up. Electrolyte imbalances can be corrected with dietary changes or supplements.

When the issue is the heart’s own electrical system, and no reversible cause can be found, a pacemaker is the standard treatment. This small device is implanted under the skin near the collarbone and sends electrical impulses to the heart when it detects the rate dropping too low. Modern pacemakers are reliable and long-lasting, and the implant procedure itself is relatively brief.

If you have no symptoms and your low heart rate isn’t caused by a medication or underlying condition, treatment usually isn’t needed at all. Many people live their entire lives with resting heart rates in the 50s or even 40s without any issues.

What Counts as Normal by Age

The 60-to-100 bpm range applies to adults and adolescents aged 13 and up. Children have naturally faster hearts. Newborns range from 100 to 205 bpm, infants from 100 to 180 bpm, and school-age children from 75 to 118 bpm. A heart rate of 55 in a healthy adult is unremarkable, but the same number in a toddler would be a medical emergency. These ranges apply while awake and at rest. Heart rate naturally drops during sleep and rises during activity.
