Low potassium means your blood levels have dropped below 3.5 mmol/L, a condition called hypokalemia. Potassium is one of the most important minerals in your body, controlling how your muscles contract, how your nerves fire, and how your heart keeps a steady rhythm. When levels fall too low, those systems start to malfunction in ways that range from mild fatigue to dangerous heart rhythm problems.

Why Potassium Matters So Much

Every cell in your body uses potassium to generate electrical signals. Your heart, skeletal muscles, and digestive tract all depend on a precise balance of potassium inside and outside your cells. When blood potassium drops, those electrical signals become unreliable. Muscles can’t contract properly, your gut slows down, and your heart’s rhythm can become erratic.

The normal range sits between 3.5 and 5.0 mmol/L. A reading just below 3.5 is considered mild. Once levels drop below 2.7, the electrical problems become visible on a heart monitor, showing changes like flattened or inverted waves and extra deflections in the heartbeat pattern. At critically low levels (below 1.7 mmol/L), a life-threatening heart rhythm called torsades de pointes can develop.

Symptoms From Mild to Severe

Mild drops in potassium often produce no symptoms at all, which is why many people discover the problem only through routine blood work. These are easy to dismiss or blame on stress and poor sleep.

Low potassium rarely comes from diet alone. In most cases, your body is losing potassium faster than you can replace it. The causes generally fall into a few categories.

Medications

Certain blood pressure medications called diuretics (water pills) are one of the most frequent culprits. These drugs work by pushing sodium and water out through your urine, and some types pull potassium along with them. Not all diuretics cause this problem. Potassium-sparing diuretics like spironolactone and triamterene actually raise potassium levels instead of lowering them, so the type you take matters.

Digestive Losses

Chronic diarrhea, prolonged vomiting, and long-term laxative use can all drain potassium from your body. Even sustained stomach suctioning (sometimes used in hospitals) removes enough fluid to trigger potassium loss through the kidneys. Rarely, certain types of colon growths called villous adenomas cause massive potassium secretion into the bowel.

Kidney Problems

Your kidneys are responsible for fine-tuning how much potassium stays in your blood versus how much gets flushed out. Several conditions can cause the kidneys to dump too much. Cushing’s syndrome, in which the body overproduces stress hormones, directly increases potassium loss through the kidneys. Primary hyperaldosteronism, where the adrenal glands produce too much of the hormone aldosterone, does the same thing.

Rarer genetic conditions like Bartter syndrome and Gitelman syndrome cause chronic kidney potassium wasting from birth. Even eating large amounts of natural licorice (the real kind, not the candy flavoring) can block an enzyme in the kidneys and trigger potassium loss.

Shifts Inside Your Cells

Sometimes the total potassium in your body is fine, but too much of it moves from your blood into your cells, making blood levels appear low. Insulin can trigger this shift, which is why potassium sometimes drops after insulin administration. A rare inherited condition called familial periodic paralysis causes sudden, dramatic shifts of potassium into muscle cells, leading to temporary episodes of weakness or paralysis.

The Magnesium Connection

One of the most overlooked factors in low potassium is magnesium. Between 10% and 40% of people with low potassium also have low magnesium, and this combination creates a stubborn problem. When magnesium is depleted, the kidneys open more channels that let potassium escape into the urine. Low magnesium also impairs the pumps that move potassium into your cells for storage.

This is clinically important because if your magnesium is low, potassium supplements alone won’t fix the problem. Your body will keep wasting potassium no matter how much you take in. Both minerals need to be corrected at the same time. If you’re on a diuretic, magnesium depletion makes the potassium-wasting effect even worse.

How Doctors Figure Out the Cause

A single blood test tells your doctor that potassium is low, but not why. The next step is usually a urine test to measure how much potassium your kidneys are excreting. If your kidneys are conserving potassium (putting very little into the urine), the loss is likely happening through your gut, from vomiting, diarrhea, or laxative use. If your kidneys are dumping large amounts of potassium despite low blood levels, the problem points to a kidney issue, a hormone imbalance, or a medication effect.

Your doctor may also check magnesium levels, kidney function, and hormone levels depending on your history. An electrocardiogram (ECG) helps assess whether your heart rhythm has been affected, which determines how urgently treatment needs to start.

How Low Potassium Is Treated

Treatment depends on how low your levels are and whether you have symptoms. For mild cases, the preferred approach is oral potassium supplements or increasing potassium-rich foods in your diet. If your potassium has dropped below 2.5 mmol/L, you can’t tolerate oral supplements, or your heart rhythm is affected, potassium may need to be given through an IV in a medical setting.

Beyond replacing potassium directly, treatment often involves addressing whatever caused the drop. That might mean switching to a potassium-sparing diuretic, treating an underlying hormone disorder, or managing chronic diarrhea. If magnesium is also low, both will be replaced simultaneously.

Potassium-Rich Foods Worth Knowing

The recommended daily potassium intake is 3,400 mg for adult men and 2,600 mg for adult women (2,900 mg during pregnancy). Most people don’t hit these targets. Adding a few potassium-dense foods to your regular meals can make a meaningful difference, especially if you’re on a medication that increases potassium loss.

A medium baked potato with skin delivers 926 mg, nearly a third of the daily target for women. One cup of cooked spinach provides 839 mg. A half cup of cooked lentils adds 366 mg. Bananas get all the publicity, but potatoes, leafy greens, beans, and lentils actually pack more potassium per serving. Incorporating these foods regularly is one of the simplest ways to support healthy potassium levels over time.

Warning Signs That Need Urgent Attention

Most cases of mildly low potassium are manageable and not immediately dangerous. But certain symptoms signal that levels may have dropped to a critical point. Irregular heartbeats or a racing, fluttering sensation in your chest are the most worrisome, especially if you already have heart disease. Sudden severe muscle weakness, difficulty breathing, or an inability to move your limbs also warrant emergency care. These symptoms suggest potassium has fallen low enough to disrupt your heart’s electrical system or cause muscle paralysis, both of which need rapid correction.
