Bloating is a feeling of fullness, tightness, or pressure in your abdomen, often described as your stomach feeling “swollen” or stuffed. It’s one of the most common digestive complaints: a survey of nearly 89,000 Americans found that about 1 in 7 had experienced bloating in the past week alone. Sometimes the sensation comes with visible swelling of the belly, and sometimes it doesn’t. Understanding the difference, and what’s actually happening inside, can help you figure out what to do about it.
Bloating vs. Distension
Doctors distinguish between two things that people often lump together. Bloating is the subjective sensation, the feeling of pressure or fullness regardless of whether your belly looks any different. Distension is the measurable, visible expansion of the abdomen. You can have one without the other. Many people who feel intensely bloated produce completely normal amounts of intestinal gas. The problem lies in how their body perceives and responds to that gas, not in how much is actually there.
In some cases, the body reacts to normal gas levels with an unusual physical reflex: the diaphragm contracts downward and the abdominal wall muscles relax, pushing the belly outward. This creates visible distension even though nothing abnormal is happening inside the gut. So bloating can be a perception issue, a gas issue, a fluid issue, or some combination of all three.
Where the Gas Comes From
There are two main sources of gas in your digestive tract: air you swallow and gas produced by bacteria in your large intestine.
Swallowed air, called aerophagia, is the biggest contributor to stomach gas. Everyone swallows small amounts of air while eating, drinking, and swallowing saliva. But certain habits dramatically increase that volume: eating quickly, gulping drinks, chewing gum, and smoking all force extra air into the digestive system. Most of this air gets released through burping, but some travels further down.
The second source is bacterial fermentation. Your large intestine houses billions of bacteria that help break down food your small intestine couldn’t fully digest, particularly certain carbohydrates. As bacteria ferment these leftovers, they produce carbon dioxide, hydrogen, and methane. No human cell can produce hydrogen or methane on its own. It’s entirely the work of gut microbes. The specific mix of gases depends on which bacterial species dominate your gut. Some people harbor more methane-producing organisms, while others have bacteria that convert hydrogen into hydrogen sulfide (the compound responsible for the unpleasant smell).
Why Certain Foods Trigger It
Not all carbohydrates are created equal when it comes to bloating. A group of short-chain carbohydrates collectively known as FODMAPs are particularly notorious. These sugars aren’t absorbed well in the small intestine, so they linger there, drawing water into the gut through osmosis. When they finally reach the large intestine, bacteria ferment them rapidly, producing a burst of gas. The combination of extra water and extra gas stretches the intestinal wall, and that stretch is what you feel as bloating.
Common high-FODMAP foods include onions, garlic, wheat, beans, certain fruits like apples and pears, and dairy products (for those who don’t fully digest lactose). Artificial sweeteners like sorbitol and mannitol follow the same pattern. Not everyone reacts to every FODMAP equally, which is why bloating triggers vary so much from person to person.
Hormones and Fluid Retention
Bloating isn’t always about gas. Fluid retention in the abdomen creates a similar sensation of fullness and visible swelling, and hormonal shifts are a major driver. Progesterone normally acts as a natural diuretic by blocking aldosterone, a hormone that tells the kidneys to hold onto sodium (and water follows sodium). When progesterone drops, as it does before a menstrual period or during menopause, that protective effect disappears. The kidneys retain more sodium, and fluid accumulates in tissues, including the abdomen.
Estrogen plays a related role by regulating how the kidneys handle sodium and water. When estrogen levels decline, the kidneys become less efficient at flushing sodium out, and the system that controls fluid balance becomes overactive. Chronic stress compounds the problem: elevated cortisol binds to the same kidney receptors as aldosterone, promoting even more sodium and water retention. This is why bloating often worsens during periods of hormonal change and high stress simultaneously. Fluid-based bloating and gas-based bloating can occur at the same time, making the sensation feel more intense than either one alone.
When Your Nerves Amplify the Signal
Some people feel bloated constantly, even when tests show no excess gas, no fluid buildup, and no structural problem. This often points to visceral hypersensitivity, a condition where the nerves lining the digestive tract become chronically overexcited. Your gut has its own extensive nervous system, sometimes called the “second brain,” with nerve endings woven through every layer of the digestive organs. These nerves respond to everything: food passing through, bacterial activity, stretching, and inflammation.
When these nerves get stuck in a heightened state, they interpret normal digestive activity as painful or uncomfortable. A standard amount of gas that wouldn’t bother most people registers as intense pressure. This often develops after a triggering event, such as a gut infection, injury, or period of severe stress. The original problem resolves, but the nerves continue sending amplified pain signals. Visceral hypersensitivity is closely linked to irritable bowel syndrome and other functional gut disorders, and it explains why many people with chronic bloating have completely normal test results.
Conditions Linked to Chronic Bloating
Occasional bloating after a large meal or a particularly gassy food is normal. Chronic bloating that persists or keeps returning can signal an underlying condition. Irritable bowel syndrome is the most common culprit, but the list is long: celiac disease, inflammatory bowel disease, gastroparesis (where the stomach empties too slowly), diabetes, hypothyroidism, and small intestinal bacterial overgrowth (SIBO) can all produce persistent bloating.
SIBO occurs when bacteria that normally live in the large intestine colonize the small intestine, where they ferment food prematurely and produce excess gas. A breath test that measures hydrogen and methane levels can help identify it. Blood tests for vitamin deficiencies and stool tests for undigested fats may also reveal problems with absorption that contribute to bloating.
What Actually Helps
The most effective approach depends on what’s driving your bloating. For swallowed-air bloating, slowing down at meals, avoiding straws and gum, and eating with your mouth closed can make a noticeable difference. For fermentation-related bloating, identifying and reducing your personal FODMAP triggers is one of the most evidence-backed strategies. This typically involves temporarily removing high-FODMAP foods, then reintroducing them one at a time to see which ones your gut tolerates.
Over-the-counter options exist but have limitations. Simethicone, one of the most widely available anti-gas products, works by merging small gas bubbles in the gut into larger ones that are easier to pass. Despite its popularity, evidence that it reliably reduces bloating symptoms is limited. For fluid-based bloating, increasing potassium intake (through foods like bananas, potatoes, and leafy greens) helps the kidneys excrete excess sodium. Regular physical activity also promotes healthy gut motility and helps gas move through the system rather than pooling.
For bloating driven by visceral hypersensitivity, the solution is less about what you eat and more about calming the nervous system. Gut-directed approaches that address the brain-gut connection, including stress management and certain behavioral therapies, have shown benefit for people whose bloating persists despite dietary changes.
Signs That Need Medical Attention
Most bloating is uncomfortable but harmless. Certain patterns, however, warrant a closer look. Bloating that gets progressively worse over time, lasts longer than a week, or comes with persistent pain is worth investigating. Accompanying symptoms like unexplained weight loss, fever, vomiting, blood in your stool, or signs of anemia (unusual fatigue, pale skin) are red flags that point to something beyond routine digestive discomfort.
