What Does It Mean to Have High Cortisol?

Having high cortisol means your body is producing more of this stress hormone than it needs, and it’s staying elevated long enough to cause problems. Cortisol plays a vital role in managing your energy, blood sugar, blood pressure, and immune response, but when levels stay high for weeks or months, it can quietly damage multiple systems in your body. The cause might be something as common as chronic stress and poor sleep, or something less common like a tumor or long-term medication use.

What Cortisol Does in Your Body

Cortisol is produced through a chain reaction involving three organs: the hypothalamus and pituitary gland in your brain, and the adrenal glands that sit on top of your kidneys. Together, these form what’s called the HPA axis. When you encounter stress, your brain signals the adrenal glands to release cortisol, which triggers a burst of useful short-term changes: your blood sugar rises to fuel your muscles, your blood pressure increases, and non-essential functions like digestion and immune defense temporarily dial down.

Once the stressor passes, cortisol levels are supposed to drop back to baseline. The system is designed to be self-regulating. But when stress is constant, or when something else is forcing cortisol production to stay high, that feedback loop breaks down. Your body stays stuck in a low-grade emergency mode, and the very changes that help you survive short-term stress start causing long-term harm.

Common Causes of High Cortisol

The most common reason for elevated cortisol is simply chronic stress. Ongoing work pressure, sleep deprivation, financial worry, or relationship strain can keep your HPA axis firing day after day. Sleep loss is a particularly potent driver, because cortisol follows a daily rhythm (highest in the morning, lowest at night) and poor sleep disrupts that cycle.

The single most common medical cause, however, is taking corticosteroid medications like prednisone. These drugs are prescribed for conditions like asthma, autoimmune diseases, and joint inflammation, and when taken in large amounts for a long time, they flood the body with synthetic cortisol. This can produce the same effects as naturally overproducing it.

Less commonly, tumors can drive cortisol overproduction. A small, noncancerous growth on the pituitary gland can pump out excess signaling hormones that tell the adrenal glands to keep making cortisol. This specific situation is called Cushing’s disease, and it accounts for about 70% of cases where the body itself is overproducing cortisol. Tumors on the adrenal glands themselves can also be responsible. In total, only about 10 to 15 new cases per million people are diagnosed in the U.S. each year, so these causes are rare.

Symptoms to Recognize

High cortisol doesn’t always announce itself with dramatic symptoms. Many of its effects overlap with things people attribute to aging, stress, or being out of shape. But the pattern matters. If you’re noticing several of these changes together, especially if they’re getting worse over time, cortisol could be the common thread:

  • Weight gain concentrated in specific areas: fat accumulating in the face (sometimes called “moon face”), the belly, and between the shoulder blades, even if your arms and legs stay relatively thin
  • Skin changes: wide, purple stretch marks on the abdomen that look different from the silvery-white marks caused by normal weight gain
  • Muscle weakness: particularly noticeable in the upper arms and thighs, making it hard to climb stairs or lift things overhead
  • Blood sugar problems: persistently high blood sugar that can progress to type 2 diabetes
  • High blood pressure
  • Bone thinning: osteoporosis or unexpected fractures, especially unusual for your age
  • Excess hair growth in women, particularly on the face

Mood and cognitive symptoms are also common. Anxiety, depression, difficulty concentrating, and memory problems frequently accompany high cortisol. Sleep disturbances can create a vicious cycle: high cortisol disrupts sleep, and poor sleep pushes cortisol even higher.

Long-Term Health Risks

When cortisol stays elevated for months or years, it disrupts nearly every major system. The long-term risks go well beyond feeling stressed. Persistently high cortisol increases your risk for heart disease, heart attack, and stroke. It promotes chronic inflammation while simultaneously weakening your immune system’s ability to fight infections and regulate itself, which can contribute to autoimmune conditions.

Metabolic damage is one of the most well-documented consequences. High cortisol pushes blood sugar up, promotes fat storage (particularly visceral fat around the organs), and can accelerate the path toward type 2 diabetes and obesity. Bone density decreases over time, raising fracture risk. Digestive problems, chronic headaches, and persistent muscle tension are also common in people living with sustained cortisol elevation.

How Cortisol Levels Are Tested

If you suspect your cortisol is too high, a doctor can order testing, but the timing and method matter. Cortisol naturally peaks in the early morning and drops throughout the day. A standard blood test drawn between 6 a.m. and 8 a.m. has a normal range of roughly 10 to 20 mcg/dL, while a late-afternoon draw should fall between 3 and 10 mcg/dL. Ranges vary between labs, so the numbers alone don’t tell the full story.

A single blood draw isn’t enough to diagnose a cortisol problem. Recommended initial screening tests include collecting urine over 24 hours (at least twice), testing saliva late at night when cortisol should be at its lowest, or a suppression test. The suppression test works by giving you a small dose of a synthetic steroid at 11 p.m. and checking your blood cortisol the next morning at 8 a.m. In a healthy system, the medication signals your body to stop making cortisol. A morning level above 1.8 mcg/dL suggests the feedback loop isn’t working properly, and levels above 5 mcg/dL point to significant overproduction.

Doctors generally reserve these tests for people with multiple suggestive symptoms, not for general screening. If two different types of tests both come back abnormal, further investigation into the cause (such as imaging of the pituitary or adrenal glands) typically follows.

Lowering Cortisol Through Daily Habits

For the majority of people whose cortisol is elevated by chronic stress rather than a medical condition, daily habits are the most effective intervention. None of these are surprising on their own, but the evidence for their direct effect on cortisol is stronger than many people realize.

Physical activity reliably lowers cortisol, with studies showing benefits in older adults and people with major depression. You don’t need intense exercise. Even a 10-minute walk in a natural setting can measurably reduce cortisol and calm brain activity. Spending time outdoors, sometimes called “forest bathing” in research, has a direct cortisol-lowering effect independent of exercise.

Sleep is non-negotiable. Sleep deprivation raises cortisol levels and increases anxiety and irritability, which feeds back into more stress and worse sleep. Protecting your sleep schedule is one of the highest-leverage things you can do.

Mind-body practices have solid evidence behind them. Mindfulness meditation, yoga, tai chi, and structured breathing exercises all reduce both cortisol levels and subjective feelings of stress. Yoga specifically has been shown to lower cortisol alongside heart rate and blood pressure.

Dietary patterns matter too. A balanced, plant-heavy diet like the Mediterranean diet helps ensure you’re getting the nutrients your body needs to regulate stress hormones. Some herbs, including ashwagandha, rhodiola, lemon balm, and chamomile, show potential for reducing cortisol. Teas made from lemon balm and chamomile are generally safe, but herbal supplements in concentrated form are worth discussing with a provider first, since they can interact with medications.

When High Cortisol Is a Medical Condition

There’s an important distinction between stress-driven cortisol elevation and Cushing’s syndrome, which is a clinical condition caused by sustained, significant cortisol excess. Cushing’s syndrome can result from long-term corticosteroid use (the most common cause overall) or from tumors that force the body to overproduce cortisol. The subset caused specifically by a pituitary tumor is called Cushing’s disease.

The symptoms of Cushing’s syndrome tend to be more severe and progressive than what you’d see from lifestyle stress alone. Unexplained osteoporosis or high blood pressure in a young person, rapid development of multiple symptoms, or a child whose weight is climbing while their height percentile drops are all red flags that warrant testing. Treatment depends on the cause: gradually tapering off corticosteroid medications when possible, or surgery to remove a tumor when one is identified.
