What Does It Mean When Your Heel Hurts?

Heel pain is one of the most common foot complaints, and in the vast majority of cases it comes from one of a handful of predictable causes. The most likely culprit is plantar fasciitis, a condition where the thick band of tissue running along the bottom of your foot becomes inflamed or damaged. But several other conditions can produce heel pain that feels similar, and where exactly it hurts, when it hurts, and how it hurts all point toward different answers.

Plantar Fasciitis: The Most Common Cause

Plantar fasciitis accounts for the majority of heel pain cases. The plantar fascia is a strong band of tissue that connects your heel bone to the base of your toes, supporting the arch of your foot. When it’s overused or stretched too far, it develops small tears and swells, making it painful to walk or stand.

The hallmark symptom is pain with your first steps in the morning. After sleeping or sitting for a while, the fascia tightens up. When you stand, it stretches suddenly, and that sharp, stabbing pain hits the bottom of your heel. The pain typically fades after a few minutes of walking as the tissue loosens, then returns after long periods on your feet.

Risk factors include spending long hours standing, a sudden increase in physical activity, tight calf muscles, high arches or flat feet, and carrying extra body weight. Runners and people who wear unsupportive shoes are especially prone to it. About 90% of cases improve with conservative measures like stretching, supportive footwear, and rest, though recovery often takes several months. Roughly 10% of people don’t improve with these approaches and develop chronic, debilitating pain that requires more aggressive treatment.

Heel Fat Pad Syndrome

Your heel has a built-in cushion: a pad of fatty tissue and elastic fibers that absorbs shock every time your foot hits the ground. Over time, this pad can thin out or lose its elasticity, leaving the heel bone with less protection. This is called heel fat pad syndrome, and it’s considered the second leading cause of heel pain after plantar fasciitis. It’s also frequently misdiagnosed as plantar fasciitis.

The key difference is how the pain behaves. Fat pad pain tends to feel like a deep, bruise-like ache right in the center of the heel, and it often worsens with prolonged standing or walking on hard surfaces. It doesn’t have the same dramatic “first step” pattern that plantar fasciitis does. Age is the biggest risk factor, since the fat pad naturally loses volume and elasticity over time. Increased body weight accelerates the process by putting more pressure on the pad with every step.

Achilles Tendon Problems

If your pain is at the back of your heel rather than the bottom, the Achilles tendon is a likely suspect. This tendon connects your calf muscles to your heel bone, and it can become inflamed or damaged from overuse.

Insertional Achilles tendonitis specifically affects the lower portion of the tendon where it attaches to the heel bone. It can cause pain, stiffness, and sometimes a bony bump at the back of the heel. While it’s most common in long-distance runners and people who are very active, it can affect anyone. The pain typically worsens with activity and improves with rest, though morning stiffness is common.

Achilles problems at the heel can also be associated with inflammatory conditions like ankylosing spondylitis and psoriatic arthritis. If you have heel pain along with stiffness in your lower back or other joints, that connection is worth exploring.

Stress Fractures

A stress fracture in the heel bone (calcaneus) can mimic other causes of heel pain, but there are some distinguishing features. Stress fracture pain tends to get worse with activity and improve when you stop moving, which is somewhat the opposite of plantar fasciitis, where the worst pain comes after rest.

A simple test can help differentiate the two: squeeze your heel bone from the sides, pressing inward with your thumb on one side and fingers on the other. If that squeeze produces pain, it may point toward a stress fracture rather than a soft tissue problem. Stress fractures are more common in runners, military recruits, and anyone who has recently ramped up high-impact activity quickly. They also occur more frequently in people with low bone density.

Nerve-Related Heel Pain

Sometimes heel pain has a neurological source. Tarsal tunnel syndrome occurs when the tibial nerve, which runs along the inside of the ankle, becomes compressed or damaged. This produces a distinctly different type of pain: burning, tingling, numbness, or a “pins and needles” sensation in the bottom of your foot or inside of your ankle.

The quality of the pain is what sets nerve issues apart. Instead of the sharp, mechanical pain of plantar fasciitis, nerve pain often feels electric, burning, or radiating. Symptoms typically worsen during or after physical activity, and in severe or long-standing cases, the pain can become constant. You may also notice weakness in the small muscles of your foot.

Heel Pain in Children and Teens

If your child complains of heel pain, the most likely cause is Sever’s disease (calcaneal apophysitis). It’s extremely common between ages 8 and 14, coinciding with growth spurts. During these periods, the growth plate at the back of the heel bone is vulnerable to irritation from the pull of the Achilles tendon and the impact of activity.

Most children report pain in both heels. The pain worsens with squeezing the heel or putting weight on it, and you may notice your child limping, walking on their toes, or feeling stiff at the heel after sleeping. Active kids, especially those in running and jumping sports, are most susceptible. The condition resolves on its own once the growth plate closes, but rest, ice, and supportive shoes help manage symptoms in the meantime.

How to Manage Heel Pain at Home

For the most common causes of heel pain, a few consistent habits make a real difference. Stretching is one of the most effective interventions, particularly for plantar fasciitis. While sitting, grasp your toes and gently pull them toward you until you feel a stretch through the arch of your foot. Hold for at least 30 seconds without bouncing, and repeat one or two times, two to three times a day. Calf stretches are equally important since tight calves increase strain on the plantar fascia.

Supportive shoes matter more than most people realize. Avoid walking barefoot on hard floors, especially first thing in the morning. Shoes with good arch support and a cushioned sole reduce the load on your heel with every step. Over-the-counter insoles or heel cups can help, particularly for fat pad syndrome where the goal is to restore some of the lost cushioning.

Ice the painful area for 15 to 20 minutes after activity, and reduce or modify activities that aggravate the pain. If you’re a runner, switching temporarily to low-impact exercise like cycling or swimming can allow healing while maintaining fitness.

Treatments Beyond Home Care

When stretching, rest, and better footwear aren’t enough after several weeks, there are additional options. Physical therapy can address underlying biomechanical issues, strengthen the foot and ankle, and use targeted manual techniques to speed recovery.

Steroid injections can significantly reduce pain in the short term. In controlled studies, patients receiving steroid injections showed meaningful pain reduction compared to placebo. However, repeated injections carry real risks, including thinning of the heel fat pad and even rupture of the plantar fascia, both of which can create new problems.

For chronic cases that haven’t responded to months of conservative treatment, shockwave therapy is an option with a strong track record. The procedure delivers focused sound waves to the affected area to stimulate healing. Treatment typically involves three sessions spaced one to two weeks apart, with success rates around 75 to 80%. Patterns That Point to Something Bigger

Most heel pain is a local, mechanical problem. But in some cases, it signals a systemic condition. Heel pain that comes with swelling, redness, and warmth could indicate gout, particularly if it comes on suddenly. Persistent heel pain combined with morning stiffness lasting more than 30 minutes, especially in the lower back or other joints, can be an early sign of ankylosing spondylitis or psoriatic arthritis. These inflammatory conditions cause pain where tendons attach to bone, and the heel is one of the most common sites.

Heel pain that doesn’t improve after six to eight weeks of consistent home care, pain that is severe enough to interfere with daily activities, or pain accompanied by numbness, fever, or visible deformity all warrant a professional evaluation. An accurate diagnosis matters because the treatments for these conditions are quite different from one another, and treating the wrong one wastes time while the real problem persists.
