Swollen lymph nodes almost always mean your immune system is actively fighting something, most often a common infection. Lymph nodes are small, bean-shaped structures scattered throughout your body that filter fluid and trap bacteria, viruses, and other threats. When they detect something harmful, immune cells inside the node multiply rapidly, and that burst of activity is what makes the node swell to a size you can feel. A normal lymph node measures less than 10 mm across (roughly the width of a pencil eraser), so once it grows beyond that, you’re likely to notice it under your skin.
Why Lymph Nodes Swell
Think of each lymph node as a checkpoint. Fluid from nearby tissues drains through it, and specialized immune cells inside inspect that fluid for anything foreign. When they find a virus, bacterium, or abnormal cell, those immune cells start dividing and recruiting reinforcements. The node fills with more cells and fluid than usual, stretching its capsule and becoming firm or tender to the touch. This is a reactive process, meaning the node is reacting to a threat rather than being the source of a problem itself.
The swelling can be localized (one area) or generalized (multiple areas at once). Localized swelling typically points to an infection or injury in the region that drains into those particular nodes. Generalized swelling, where you notice enlarged nodes in two or more separate body regions simultaneously, more often signals a system-wide process like a viral illness, an autoimmune condition, or, less commonly, something more serious.
The Most Common Causes
Viral infections are by far the most frequent reason for swollen lymph nodes. Colds, flu, mono (caused by Epstein-Barr virus), and other respiratory viruses routinely trigger neck node swelling that resolves on its own. COVID-19 and routine vaccinations can do the same.
Bacterial infections are the next most common cause. Skin infections from staph or strep bacteria often cause rapid swelling in the nearest node group. A cut on your hand might enlarge a node in your armpit; an infected tooth can swell the nodes under your jaw. Strep throat is a classic cause of tender, swollen cervical (neck) nodes in children and adults alike.
Beyond everyday infections, other causes include:
- Autoimmune diseases: Lupus, for example, lists swollen lymph nodes as a common symptom. Rheumatoid arthritis and sarcoidosis can also trigger persistent node enlargement.
- Sexually transmitted infections: Herpes simplex, syphilis, and other STIs frequently cause swollen inguinal (groin) nodes.
- Cat-scratch disease: A bacterial infection spread through cat scratches or bites that can cause node swelling lasting several months.
- Medications: Certain drugs, including the anti-seizure medication phenytoin and some malaria-prevention medications, can cause lymph node swelling as a rare side effect.
- Cancer: Lymphomas, leukemias, and cancers that have spread from another organ can enlarge lymph nodes. This is a less common cause overall, but it’s the one most people worry about.
What the Location Tells You
Your body has roughly 600 lymph nodes, but you can only feel the ones close to the skin’s surface. The location of swelling often narrows down the cause because each node group drains a specific region.
Nodes along the sides of your neck and under your jaw handle drainage from your throat, mouth, sinuses, and ears. These are the ones that swell during colds, sore throats, and ear infections. Nodes behind your ear can enlarge with scalp infections or conditions like rubella.
Armpit (axillary) nodes drain the arms, chest wall, and breast tissue. Swelling here can follow a hand or arm wound, a recent vaccination in that arm, or, less commonly, breast-related conditions. Groin (inguinal) nodes filter fluid from the legs, feet, and genital area. These nodes are somewhat larger to begin with, with a normal short-axis diameter of up to 15 mm, so they need to be noticeably bigger before they’re considered truly enlarged. Lower-extremity skin infections and STIs are the most common causes of inguinal swelling.
Nodes deeper in the body, like those in the chest and abdomen, can’t be felt by hand. They’re only discovered through imaging. Swelling in these hidden node groups tends to carry more clinical significance because everyday infections rarely cause it.
How to Tell Benign From Concerning
Most swollen lymph nodes are benign. The characteristics of the node itself offer useful clues about what’s driving the swelling.
Nodes that are tender, soft, and move freely under your fingers when you press on them are typically reacting to a nearby infection. Pain is actually somewhat reassuring because it usually reflects rapid swelling from an active immune response. Nodes that appeared suddenly alongside cold or flu symptoms, a sore throat, or a visible skin infection fit this pattern.
Nodes that raise more concern tend to be painless, hard or rubbery, and fixed in place (they don’t slide under your fingers). Malignant nodes also tend to be larger in all dimensions and may have a rounder shape compared to the oblong shape of healthy nodes. A node that keeps growing over weeks without an obvious infection, or one that appears above the collarbone (supraclavicular area), warrants prompt evaluation because this location has a higher association with serious underlying conditions.
How Long Swelling Normally Lasts
With a typical viral or bacterial infection, lymph nodes generally stay swollen for a few days and then gradually shrink back to normal as the infection clears. Sometimes they take a bit longer. A node that swelled during a bad respiratory infection might remain slightly palpable for two to three weeks even after you feel better.
Certain infections cause longer-lasting enlargement. Cat-scratch disease, for instance, can keep nodes swollen for several months. Mono is another common culprit behind nodes that linger for weeks.
The timeline becomes important when swelling persists without explanation. If a lymph node stays enlarged for more than a few days and you don’t feel sick, or if nodes don’t return to normal after an illness resolves, that’s worth a medical evaluation. The same applies if you notice nodes growing progressively larger over time.
Symptoms That Warrant Attention
Swollen nodes alone are rarely an emergency, but certain accompanying symptoms change the picture. Unexplained weight loss, drenching night sweats (enough to soak your sheets), and persistent fevers that come and go without an obvious infection are collectively known as “B symptoms” in medicine. When these appear alongside enlarged lymph nodes, they suggest the body is dealing with something more systemic, potentially a lymphoma or another serious condition.
Other signals to watch for: nodes that grow steadily over two to four weeks, swelling in multiple unrelated areas of the body at once (like the neck and groin simultaneously), nodes larger than about 2 cm (roughly the size of a grape), skin redness and warmth over a node suggesting it may be forming an abscess, and difficulty swallowing or breathing due to enlarged nodes pressing on nearby structures.
What Happens During Evaluation
When a swollen node needs investigation, the process typically starts with a physical exam and a detailed history. Your doctor will feel the node’s size, texture, and mobility, and ask about recent illnesses, travel, animal exposures, medications, and other symptoms. Blood tests can check for signs of infection, inflammation, or blood cell abnormalities that point toward specific causes.
If the cause isn’t clear from initial testing, imaging with ultrasound is often the next step. Ultrasound can measure the node precisely and look at its internal structure. Nodes that have lost their normal oval shape or their bright center on ultrasound are more likely to be abnormal. For nodes that remain unexplained or look suspicious on imaging, a biopsy (removing a small sample or the entire node for examination under a microscope) gives a definitive answer. This is the only way to confirm or rule out cancer with certainty.
