Most of the time, it signals a muscle strain, poor posture, or age-related wear and tear rather than something dangerous. But the location, duration, and accompanying symptoms can tell you a lot about what’s actually going on.

Muscle Strains and Ligament Sprains

The most frequent cause of neck pain is a strain (overstretched muscle) or sprain (overstretched ligament). These happen when your neck bends or twists beyond its normal range, whether from a car accident, a fall, sleeping in an awkward position, or simply turning your head too fast. The pain usually feels like a dull ache or stiffness on one or both sides, and it gets worse when you try to move your head in certain directions.

Most neck strains and sprains resolve within 4 to 6 weeks. Severe injuries, like those from a high-speed collision, can take longer. In the early days, gentle movement tends to help more than keeping your neck completely still. Prolonged immobilization with a collar can actually slow recovery by weakening the surrounding muscles.

Posture and Prolonged Sitting

If your neck hurts at the end of a workday or after scrolling your phone for a while, posture is the likely culprit. Holding your head forward of your shoulders, even by a couple of inches, dramatically increases the load on your neck muscles. Over hours and days, this creates a persistent aching tightness across the back of the neck and up into the base of the skull. Some people develop tension headaches from this pattern as well.

Age-Related Wear (Cervical Spondylosis)

By age 40, the discs between your neck vertebrae begin drying out and shrinking. When it does cause symptoms, you’ll typically notice a grinding stiffness in your neck, especially in the morning, that loosens up as you move through the day.

As the discs lose height, the body sometimes responds by growing small bony projections called bone spurs. These are usually harmless, but in some cases they can narrow the space around the spinal cord or nerve roots and cause more significant symptoms. The peak age range for symptomatic neck pain from these changes falls between 45 and 74, and women are affected at roughly 1.4 times the rate of men.

Pinched Nerves in the Neck

When a herniated disc or bone spur presses on a nerve root as it exits the spine, the pain often radiates down into the shoulder, arm, or hand. This is called cervical radiculopathy, and the specific pattern of tingling or numbness tells you which nerve is involved:

Surgery is reserved for cases where significant weakness progresses or conservative treatment fails after a reasonable trial.

When Neck Pain Needs Urgent Attention

Certain combinations of symptoms alongside neck pain point to something more serious. A very stiff neck paired with fever, sensitivity to light, and a severe headache can indicate meningitis. Neck pain after a significant impact, like a car crash or a fall from height, raises concern for fracture or ligament instability, especially if you feel unsteady on your feet or notice changes in coordination.

The most time-sensitive scenario involves the blood vessels running through the neck. An arterial dissection, where the inner wall of a neck artery tears, can cause sudden neck pain or headache followed by vision changes, difficulty speaking, facial drooping, or one-sided weakness. These are stroke symptoms and require emergency care immediately. This is rare, but it’s worth knowing that sudden, unusual neck pain combined with any neurological change is not something to wait out.

Other warning signs include unexplained weight loss alongside persistent neck pain (which can suggest infection or cancer affecting the spine), progressive weakness in both arms or legs, or loss of bladder or bowel control, which may indicate pressure on the spinal cord itself.

How Neck Pain Is Evaluated

Most neck pain does not require imaging. If you haven’t had trauma and you don’t have neurological symptoms like numbness, weakness, or coordination problems, a physical exam is usually enough to guide treatment. Standard X-rays catch only about 36% of cervical injuries, which is why CT scans have largely replaced them for trauma evaluation. MRI is the best tool for seeing soft tissue problems like disc herniations and nerve compression, but it’s typically reserved for cases where there are neurological symptoms or where pain hasn’t responded to initial treatment.

Doctors use established screening criteria (the NEXUS and Canadian C-Spine rules) to determine whether someone with a neck injury actually needs imaging. If you’re alert, not intoxicated, have no tenderness along the spine’s midline, and can rotate your head 45 degrees in each direction, imaging after a minor injury is unlikely to reveal anything actionable.

Treatment That Works

Exercise and manual therapy form the backbone of neck pain treatment across nearly every category. The specifics depend on what’s causing the pain.

For neck pain with limited range of motion, clinical guidelines recommend mobility exercises combined with upper back and arm strengthening. Hands-on treatment targeting the upper back (thoracic spine manipulation) has strong support for improving neck motion, which may seem counterintuitive but makes sense given how closely the two regions work together.

Whiplash-related neck pain benefits most from education and an early return to normal activities. The instinct to protect and immobilize the neck is understandable but counterproductive. Gentle movement, combined with manual therapy and targeted exercises, helps control pain while restoring function. For neck pain that radiates into the arm from a pinched nerve, intermittent traction, stretching, strengthening, and manual therapy can all reduce symptoms.

If your neck pain comes with headaches, especially those that start at the base of the skull and wrap forward, a combination of hands-on treatment with neck mobility and upper back strengthening exercises tends to be most effective. These cervicogenic headaches are often misidentified as migraines or tension headaches, so if your headaches always seem to coincide with neck stiffness, it’s worth exploring that connection.

Over-the-counter anti-inflammatory medications can help manage acute flare-ups, but they work best as a bridge to let you participate in active treatment rather than as a long-term solution. The consistent finding across treatment guidelines is that passive approaches (rest, medication alone, collars) produce worse long-term outcomes than active ones that involve movement and strengthening.
