Red urine can mean something as harmless as eating beets for dinner or as serious as blood leaking from your kidneys or bladder. The color alone doesn’t tell you which one it is, so the first step is figuring out whether the red you’re seeing is actually blood or just a pigment passing through.
Foods That Turn Urine Red
Beets are the most common culprit. They contain a reddish pigment called betalain that your body excretes through urine, sometimes producing a startlingly vivid pink or red color. This phenomenon, sometimes called “beeturia,” is harmless and typically clears within a day or two after you stop eating beets. Rhubarb and blackberries can produce a similar effect.
Interestingly, not everyone who eats beets will notice a color change. Whether you do depends partly on your stomach acid levels and what else you ate at the same meal. Eating beets alongside foods high in oxalate, like spinach or rhubarb, can actually increase the chance of discoloration in people who don’t normally experience it. If you recently ate any of these foods and your urine returned to normal within 24 to 48 hours, the food was almost certainly the explanation.
Medications That Change Urine Color
Several common medications can turn urine red, orange, or reddish-brown without any blood being present. A tuberculosis drug called rifampin is well known for turning urine reddish-orange. Phenazopyridine, an over-the-counter bladder pain reliever often taken during urinary tract infections, produces a bright reddish-orange color that can look alarming if you’re not expecting it. Laxatives containing senna can also cause this shift.

When Red Urine Is Actually Blood
If food and medications don’t explain the color, red urine likely contains blood. Visible blood in urine is called gross hematuria, and it deserves prompt medical attention even if it happens only once and doesn’t hurt. The most common causes include:
- Urinary tract infections (UTIs). Blood often appears alongside burning during urination, urgency, and frequent trips to the bathroom. Kidney infections, a more serious type of UTI, can add fever, back pain, and chills.
- Kidney or bladder stones. Stones scrape the lining of the urinary tract as they move, causing blood that may come with sudden, intense flank or lower abdominal pain.
- Enlarged prostate. In men, a noncancerous enlarged prostate can compress the urethra and cause blood to appear in urine, often alongside difficulty starting or stopping the stream.
- Kidney disease. Conditions that damage the tiny filtering units inside the kidneys can leak red blood cells into urine. Sometimes this blood is only detectable under a microscope.
- Cancer. Bladder, kidney, or prostate cancer can all cause visible blood in urine. Painless red urine with no other symptoms is actually the pattern that raises the most concern for malignancy.
- Injury. A blow to the kidneys or lower abdomen, whether from a car accident, a fall, or a contact sport, can produce bloody urine.
Blood thinners, aspirin, and certain antibiotics like penicillin can also increase the likelihood of blood appearing in your urine. But taking these medications doesn’t mean the blood can be ignored. Guidelines from the American Urological Association state that people on blood thinners should receive the same evaluation as anyone else with blood in their urine.
Exercise-Related Red Urine
Intense physical activity, especially long-distance running, can temporarily turn urine pink or red. This is surprisingly common: microscopic blood appears in up to 95% of athletes after strenuous exercise. The degree of discoloration correlates with how hard you pushed yourself.
Several mechanisms are at play. During intense exercise, blood flow shifts away from the kidneys, and the kidney’s filtering membranes become more permeable, letting red blood cells slip through. In runners specifically, the bladder can bounce against the pelvic bone repeatedly, causing mild internal bruising. Breakdown of red blood cells and muscle tissue during extreme exertion also contributes to the color change in roughly 30% of cases.
Exercise-induced red urine typically resolves within 24 to 72 hours of rest. If it persists beyond that, something else is going on.
Causes Specific to Men and Women
In men, the prostate is a frequent factor. Benign prostate enlargement is extremely common with age and can cause blood in urine even without pain. Prostate cancer is a less common but more serious possibility, particularly in men over 50 with risk factors like smoking.
In women, menstrual blood can mix with urine during collection and create the appearance of bloody urine when the urinary tract is perfectly healthy. If you notice red urine around your period, collecting a clean sample at a different time can help clarify whether the blood is truly coming from the urinary tract. Endometriosis, a condition where tissue similar to the uterine lining grows outside the uterus, can also involve the bladder and cause cyclical blood in urine.
How Red Urine Is Evaluated
A standard urinalysis is the starting point. A dipstick test can detect blood, but a positive dipstick alone isn’t definitive. A microscopic exam of the urine sample looks for actual red blood cells. Three or more red blood cells per high-powered field on a single properly collected specimen confirms microscopic hematuria.
What happens next depends on your risk level. Your doctor will consider your age, smoking history, and whether the blood was visible or only microscopic. For people at low risk, repeat testing in six months is often sufficient. For intermediate risk, an ultrasound of the kidneys and a cystoscopy (a thin camera inserted into the bladder) are typically recommended. High-risk patients usually get a CT scan of the urinary tract along with cystoscopy to check for tumors or structural problems.
The reason for this thorough approach: among people with visible blood in their urine, roughly 17% are eventually diagnosed with bladder cancer. About 2% have kidney cancer. These numbers are high enough that painless, unexplained red urine always warrants investigation.
Symptoms That Need Urgent Attention
Some combinations of symptoms alongside red urine signal a more urgent problem. Blood clots in your urine, especially if they make it difficult to urinate, need same-day evaluation. Red urine with high fever and flank pain suggests a kidney infection that can progress quickly. Bloody urine after a significant injury to your back or abdomen also warrants an emergency visit, since it may indicate damage to the kidneys or bladder.
Painless red urine that appears once and clears is less immediately dangerous, but it still needs follow-up. Bladder and kidney cancers often announce themselves with a single, painless episode of bloody urine that then disappears for weeks or months before returning. The absence of pain is not reassuring in this context.
