Skin that hurts to the touch, burns without a visible cause, or feels painful from something as light as clothing brushing against it is usually a sign that your nervous system is processing pain signals abnormally. The medical term for this is allodynia: pain from a stimulus that wouldn’t normally cause pain. It can also show up as hyperalgesia, where something mildly uncomfortable feels far more intense than it should. Both point to the same core issue: your nerves or brain are amplifying pain signals, and the reasons range from temporary and harmless to conditions that benefit from treatment.
How Normal Touch Becomes Painful
Your skin contains millions of nerve fibers designed to detect different sensations. Some respond to light touch, temperature, or pressure. Others, called nociceptors, specifically detect damage and send pain signals. When your nervous system is working normally, a soft brush against your arm activates only the light-touch fibers, and your brain registers it as harmless contact.
When something disrupts this system, your brain starts interpreting those harmless signals as threats. This is called central sensitization: a structural or chemical change in your central nervous system that lowers the threshold for pain. Think of it like a security alarm that’s been set too sensitive. A gust of wind triggers the same response as a break-in. The pain you feel is real, not imagined, even though nothing is actually injuring your skin.
Shingles and Viral Nerve Pain
One of the most common causes of unexplained skin pain is shingles, a reactivation of the chickenpox virus that lies dormant in nerve tissue. The hallmark of shingles is that skin pain typically appears one to five days before any rash shows up. During this window, you might feel tingling, burning, or sharp pain on one side of your body or head with no visible explanation. The pain follows the path of a single nerve, so it usually appears as a stripe across one side of the torso, shoulder, neck, or forehead.
If your skin hurts in a band-like pattern on just one side of your body and you’ve had chickenpox at any point in your life, shingles is a strong possibility. The rash that follows typically confirms it, but those early days of pain-without-rash can be confusing and alarming.
Small Fiber Neuropathy
If your skin pain is concentrated in your feet and gradually spreading upward, or if it follows a “stocking and glove” pattern (feet and hands), small fiber neuropathy is a likely explanation. This condition damages the tiny nerve fibers in your skin responsible for pain and temperature sensation, while leaving your strength, reflexes, and coordination completely intact.
The most characteristic symptom is burning pain in the feet that worsens at night and often disrupts sleep. Many people also describe electric shock-like sensations, stabbing pains, or a constant pins-and-needles feeling. On examination, affected skin often shows allodynia (pain from gentle touch) alongside reduced ability to sense pinprick or temperature changes.
Small fiber neuropathy is notoriously tricky to diagnose because standard nerve conduction tests come back normal. The small fibers it affects are too thin to show up on those tests. Diagnosis typically requires a skin biopsy that measures the density of nerve fibers in the outer layer of skin, or a sweat function test that evaluates the tiny autonomic nerves. Diabetes is the most common underlying cause, but autoimmune conditions, thyroid disorders, and vitamin deficiencies can also trigger it.
Fibromyalgia and Central Sensitization
Fibromyalgia is one of the most recognized conditions associated with widespread skin sensitivity. The underlying problem is central sensitization: your central nervous system amplifies incoming signals, so stimuli that shouldn’t hurt (like the pressure of a waistband or the weight of a blanket) register as painful. This isn’t limited to one area. It tends to affect large regions of the body and can shift locations over time.
People with fibromyalgia often describe their skin as feeling bruised, sunburned, or raw even though it looks completely normal. The sensitivity can fluctuate with sleep quality, stress levels, weather changes, and physical activity. Because there’s no visible sign on the skin and standard blood tests don’t reveal it, many people spend years searching for an explanation before receiving a diagnosis.
Stress, Anxiety, and Pain Amplification
Your emotional state has a direct, measurable effect on how you experience pain. The brain regions that process negative emotions like anxiety, sadness, and fear overlap significantly with the regions that process physical pain. When those emotional centers are already highly active, it takes less input to trigger a pain response, and the pain that does register feels more intense.
This doesn’t mean the pain is “in your head” in a dismissive sense. Your brain processes stress-related pain using the exact same pathways it uses for injury-related pain. The sensation can feel identical to any other kind of pain: burning, aching, stinging, or soreness across your skin. Periods of high stress or anxiety can genuinely make your skin more sensitive to touch, temperature, and pressure. Chronic pain itself can then worsen anxiety and depression, creating a feedback loop that sustains the sensitivity.
Nutritional Deficiencies
Vitamin B12 deficiency can cause neurological symptoms including numbness, pins and needles, and altered skin sensation. B12 is essential for maintaining the protective coating around your nerve fibers. When levels drop low enough, those fibers begin to malfunction, sending distorted signals that your brain may interpret as pain, tingling, or burning. This is more common in people over 50, those who follow strict vegan diets, or anyone with conditions that impair nutrient absorption in the gut.
Erythromelalgia
If your skin pain comes with visible redness, warmth, and swelling, particularly in the feet or hands, erythromelalgia is worth considering. This condition causes episodes (called flare-ups) of burning pain that can start as itching before escalating. Flare-ups last anywhere from a few minutes to several days and are triggered by increases in body temperature: exercise, warm weather, tight clothing, hot showers, stress, alcohol, or spicy food.
Between episodes, the affected skin may feel cold and appear blue or grey. On darker skin tones, the redness during a flare-up can be harder to spot, so the burning sensation and warmth are more reliable indicators than color changes alone.
When Skin Pain Signals Something Urgent
Most causes of skin pain are manageable and not emergencies. However, certain combinations of symptoms warrant immediate medical attention. Skin pain paired with a rapidly spreading rash and fever can indicate serious drug reactions or infections. Skin that is painful, red, and expanding quickly, especially with fever or feeling generally unwell, could point to a spreading skin infection like cellulitis.
Pay particular attention if skin pain involves the eyes, hands, or groin, as these areas carry higher risk of complications. A widespread rash with peeling skin, high fever, and a drop in blood pressure is a medical emergency. In one review of children who arrived at an emergency department with fever and full-body redness, 45% progressed to shock. These scenarios are rare, but recognizing them matters.
Narrowing Down Your Cause
The pattern of your skin pain offers the strongest clues to its source. Pain on one side of the body in a stripe suggests shingles. Burning that starts in the feet and creeps upward points to small fiber neuropathy. Widespread sensitivity that fluctuates with stress and sleep fits fibromyalgia. Redness and warmth triggered by heat suggest erythromelalgia. Pain that tracks closely with periods of high anxiety or emotional distress may involve central pain amplification.
Note when the pain is worst (night versus day), what makes it better or worse (temperature, clothing, activity, stress), whether it’s spreading, and whether you see any changes on the skin itself. These details help distinguish between conditions that otherwise feel very similar from the inside. Many of these causes overlap, and more than one can be present at the same time, so a clear timeline and symptom diary gives any clinician a significant head start.
