Lexapro (escitalopram) is a prescription antidepressant that increases serotonin levels in the brain. It’s FDA-approved for two conditions: major depressive disorder in adults and adolescents 12 and older, and generalized anxiety disorder in adults. Most people start at 10 mg once daily and begin noticing improvements within two weeks, though full effects typically take four to six weeks.

How Lexapro Works in the Brain

Serotonin is a chemical messenger that helps regulate mood, sleep, and anxiety. After nerve cells release serotonin to send a signal, they normally reabsorb it through a protein called the serotonin transporter. Lexapro blocks that transporter, keeping serotonin active in the gap between nerve cells for longer. This is why it’s classified as a selective serotonin reuptake inhibitor, or SSRI.

What makes Lexapro slightly different from other SSRIs is how it attaches to the serotonin transporter. It binds to two spots on the transporter simultaneously. The second binding point slows down the rate at which the drug detaches from the first, giving it a longer-lasting grip. This dual-binding mechanism is one reason Lexapro is considered particularly potent at low doses compared to its predecessor, Celexa (citalopram). Celexa contains a mix of two mirror-image molecules, only one of which is therapeutically active. Lexapro contains only that active molecule, making it more efficient. In meta-analyses, escitalopram has shown superiority over citalopram, particularly in severe depression.

What It Treats

Lexapro is prescribed primarily for major depressive disorder and generalized anxiety disorder. For depression, it can help lift persistent low mood, restore interest in activities, and improve concentration. For generalized anxiety, it reduces the cycle of excessive worry, muscle tension, and restlessness that characterizes the condition.

Doctors also prescribe Lexapro off-label for other anxiety-related conditions like social anxiety, panic disorder, and obsessive-compulsive disorder, though these uses don’t carry formal FDA approval.

What the First Few Weeks Feel Like

Lexapro doesn’t work like a pain reliever where you feel the effect within hours. The brain needs time to adjust to higher serotonin availability. Sleep, energy, and appetite often improve first, sometimes within one to two weeks. The emotional benefits, like feeling less hopeless or less consumed by worry, typically take four to six weeks to fully develop.

This lag is important to understand. Many people assume the medication isn’t working after a week or two and want to stop. The early improvements in physical symptoms like sleep and energy are actually signs that the drug is taking hold, even if your mood hasn’t shifted yet. If there’s no noticeable change after six to eight weeks, that’s when a dose adjustment or a different medication is worth discussing.

Common Side Effects

Most side effects show up in the first week or two and fade as your body adjusts. The most frequently reported include nausea, headache, drowsiness or insomnia, dry mouth, increased sweating, and dizziness. These tend to be mild for the majority of people and often resolve within the first few weeks.

Sexual side effects are a bigger concern because they’re common and don’t always fade with time. Research estimates that 57% to 77% of people taking antidepressants experience some form of sexual dysfunction, including reduced desire, difficulty with arousal, or trouble reaching orgasm. One study found that about 34% of women on escitalopram specifically reported sexual difficulties. Higher doses carry a significantly greater risk: moderate to high dosing was associated with nearly five times the odds of sexual dysfunction compared to lower doses.

Weight changes are another common worry. While some people gain weight on SSRIs over the long term, the data on escitalopram specifically is limited, and weight gain is generally less pronounced than with some other antidepressants.

Typical Dosing

The standard starting dose is 10 mg once a day, taken in the morning or evening, with or without food. For most adults, 10 mg is also the recommended maintenance dose. If needed, the dose can be increased to a maximum of 20 mg per day, but not sooner than one week after starting (or three weeks for adolescents). Adults 65 and older typically stay at 10 mg. The simplicity of once-daily dosing is one reason Lexapro is widely prescribed.

Sexual Side Effects and Dose

If sexual side effects are a problem, dose plays a key role. The research is clear that moderate to high doses dramatically increase the likelihood of sexual dysfunction. Staying at the lowest effective dose is the most straightforward strategy for reducing this risk. Some people also find that timing the dose differently or adding exercise helps, though individual responses vary.

Stopping Lexapro Safely

Lexapro carries a moderate risk of discontinuation syndrome, meaning your body can react if you stop suddenly. Symptoms include flu-like achiness, fatigue, headaches, dizziness, nausea, vivid or disturbing dreams, and burning or shock-like tingling sensations. Mood changes like irritability, agitation, and rebound anxiety are also common.

These symptoms aren’t a sign of addiction. They reflect the brain readjusting to lower serotonin activity after being accustomed to higher levels. The standard approach is a gradual taper, slowly reducing the dose over weeks or months rather than stopping abruptly. The timeline varies depending on how long you’ve been on the medication and your dose, so tapering schedules are individualized.

Drug Interactions to Know About

The most serious risk with Lexapro is serotonin syndrome, a potentially dangerous condition caused by too much serotonin in the brain. This can happen when Lexapro is combined with other medications that also raise serotonin levels, including certain migraine medications (triptans), some pain relievers (tramadol, fentanyl), the herbal supplement St. John’s wort, and especially a class of older antidepressants called MAO inhibitors. Symptoms of serotonin syndrome include agitation, rapid heart rate, high blood pressure, muscle twitching, fever, and in severe cases, seizures.

Blood thinners like warfarin and anti-inflammatory drugs like ibuprofen can also interact with Lexapro by increasing bleeding risk. If you take any other medications or supplements, flagging them before starting Lexapro helps avoid dangerous combinations.

Suicidal Thoughts in Young People

Lexapro carries an FDA black box warning, the most serious type of safety alert, regarding an increased risk of suicidal thoughts and behavior in children, adolescents, and young adults under 25. This risk is highest in the first few months of treatment or when the dose changes. It doesn’t mean the medication causes suicide, but it does mean close monitoring during early treatment is essential for younger patients. In adults over 25, antidepressants have not been associated with increased suicidal thinking and are generally considered protective over time.
