Low blood oxygen means your body isn’t carrying or receiving enough oxygen to meet its needs. For most adults, a normal oxygen saturation level (the percentage of hemoglobin in your blood that’s carrying oxygen) falls between 95% and 100%. When that number drops below 95%, oxygen delivery to your organs starts to suffer, and below 92% is generally considered a level worth calling your doctor about. At 88% or lower, you need emergency care.

How Your Body Detects and Responds to Low Oxygen

Your body has built-in oxygen sensors, primarily located in specialized cells near the arteries in your neck. When these sensors detect a drop in oxygen, they trigger a rapid chain reaction: your breathing rate increases to pull in more air, and your adrenal glands release adrenaline and noradrenaline. That surge raises your heart rate and blood pressure, pushing whatever oxygen-rich blood is available to your organs faster. This is why a racing heart and rapid breathing are often the first signs something is off.

The brain also has its own oxygen-sensing system. Because brain tissue is extremely sensitive to oxygen deprivation, even mild drops can cause subtle changes in attention, judgment, and coordination. More severe drops can lead to slurred speech, confusion, and eventually unconsciousness.

Symptoms to Recognize

Mild low oxygen often shows up in ways you might not immediately connect to breathing. You may feel mentally foggy, have trouble concentrating, or notice your coordination feels slightly off. Some people describe a sense of something being “not right” before more obvious symptoms appear.

As oxygen levels fall further, symptoms become harder to ignore:

  • Shortness of breath, especially during activities that don’t normally wind you
  • Rapid heartbeat as your body compensates
  • Bluish tint to your lips, fingertips, or nail beds (called cyanosis)
  • Confusion or poor judgment, which can make it difficult to recognize the problem yourself
  • Headache and restlessness

Severe oxygen deprivation to the brain can cause complete unresponsiveness, loss of breathing, and pupils that no longer react to light. This is a medical emergency.

What Causes It

Low blood oxygen has two broad categories of causes: problems getting oxygen into the blood, and problems moving that oxygen where it needs to go.

Lung and Airway Problems

The most common culprits are conditions that interfere with how well your lungs transfer oxygen into your bloodstream. Chronic obstructive pulmonary disease (COPD), pneumonia, asthma attacks, pulmonary embolism (a blood clot in the lung), and fluid buildup in the lungs can all reduce the amount of oxygen that makes it from the air you breathe into your blood. COVID-19 and other respiratory infections can cause inflammation that thickens the barrier between your air sacs and blood vessels, making oxygen transfer less efficient.

Blood and Circulation Problems

Sometimes the lungs work fine, but the blood can’t carry enough oxygen. Anemia, a condition where you don’t have enough healthy red blood cells, is a common example. The protein hemoglobin inside red blood cells is what physically binds to oxygen molecules, so anything that reduces hemoglobin levels or changes its structure limits your blood’s carrying capacity. Congenital heart defects, present from birth, can also mix oxygen-poor blood with oxygen-rich blood before it reaches the rest of the body, lowering overall oxygen saturation.

It’s worth noting that you can have low oxygen in your blood without your tissues being starved (your body compensates enough), or your tissues can be starved of oxygen even when blood levels look normal (as with severe anemia or shock). Both situations matter, but they require different approaches to treatment.

Environmental Factors

Altitude is a major factor. At elevations above 8,000 feet, the air contains less oxygen per breath, and your saturation levels will naturally drop. Most people’s bodies adjust over days to weeks. Animal research has shown that in the first days of low-oxygen exposure, activity levels plummet and blood chemistry shifts, but by the end of three weeks, these markers largely return to normal. If you develop a cough, rapid heartbeat, or weakness at high altitude, that may signal fluid leaking into your lungs, a dangerous condition called high-altitude pulmonary edema.

How Oxygen Levels Are Measured

The most common way to check your oxygen at home is with a pulse oximeter, a small clip-on device that sits on your fingertip. It shines light through your skin and estimates how much of your hemoglobin is carrying oxygen, displayed as a percentage called SpO2.

Pulse oximeters are convenient but not perfect. The FDA has acknowledged that current scientific evidence shows accuracy differences between individuals with lighter and darker skin pigmentation. In people with darker skin, these devices can overestimate oxygen levels, potentially masking dangerously low readings. The FDA has proposed updated testing recommendations that would require manufacturers to demonstrate accuracy across a range of skin tones before marketing their devices, using both subjective and objective methods for measuring skin pigmentation. Prospective clinical studies in both adults and children are helping to inform these new standards.

For a more precise measurement, a doctor can draw arterial blood (typically from your wrist) and directly measure the partial pressure of oxygen. This is the gold standard in clinical settings and gives a fuller picture of how well your lungs are working.

How Low Oxygen Is Treated

Treatment depends on what’s causing the drop and how severe it is. The immediate goal is almost always the same: get more oxygen into your blood.

Supplemental oxygen is the most direct approach. At home, this typically means breathing concentrated oxygen through a nasal cannula (a lightweight tube with two small prongs that sit just inside your nostrils) or a face mask. The oxygen comes from one of three types of systems. Compressed gas tanks store pure oxygen under pressure with a regulator to control flow. Liquid oxygen systems store super-cooled liquid oxygen in smaller, more portable containers that convert the liquid to breathable gas. Oxygen concentrators are electric machines that pull room air in, filter out the nitrogen, and deliver concentrated oxygen, making them useful for long-term home use since they don’t need refilling.

For chronic conditions like COPD, supplemental oxygen may become part of daily life. Some people use it only during sleep or exercise, while others need it around the clock. In emergency situations where someone can’t breathe on their own, oxygen is delivered through a tube placed directly into the windpipe.

Beyond supplemental oxygen, treating the underlying cause is critical. That might mean antibiotics for pneumonia, iron supplementation for anemia, inhalers for COPD flares, or surgical repair for structural heart defects. Oxygen therapy keeps you stable while the root problem is being addressed.

Oxygen Readings That Need Attention

If you’re monitoring at home with a pulse oximeter, the key thresholds are straightforward. Readings of 95% to 100% are normal for most people. A consistent reading of 92% or lower warrants a call to your healthcare provider. At 88% or lower, get to the nearest emergency room.

Context matters, though. If you have a chronic lung condition, your doctor may have given you a different personal baseline. Some people with long-standing COPD function at saturations that would alarm someone without lung disease. What matters most is a drop from your normal, not just the absolute number.

Seek emergency care if you experience sudden shortness of breath that interferes with basic tasks, especially if it’s accompanied by chest pain. At altitude, a combination of cough, rapid heartbeat, and weakness is a red flag for high-altitude pulmonary edema, which can be fatal without treatment.
