In more pronounced cases, you might faint, feel unusually sluggish, or behave in ways that seem out of character, like sudden confusion or agitation.

Athletes and people who exercise regularly often have lower resting blood pressure as a sign of cardiovascular fitness. The concern begins when symptoms show up or when your numbers drop suddenly from where they normally sit.

Types of Low Blood Pressure

Orthostatic Hypotension

This is the type you feel when you stand up too quickly and the room briefly spins. It’s diagnosed when your top number drops by 20 mmHg or your bottom number drops by 10 mmHg within two to five minutes of standing. Your body is supposed to tighten blood vessels and speed up your heart rate the moment you go upright, but sometimes that response is too slow or too weak. Older adults, people who are dehydrated, and those on blood pressure medications are especially prone to it.

Postprandial Hypotension

Blood pressure can also drop after eating. When you digest a meal, your body sends extra blood to the digestive system. Normally, your heart rate increases and blood vessels elsewhere tighten to compensate. When that compensation falls short, blood pressure drops. For most people with this pattern, it happens within 30 to 60 minutes of eating, though it can occur up to two hours after a meal. Large, carbohydrate-heavy meals tend to be the biggest trigger.

Common Causes

Dehydration is one of the most frequent and fixable causes. When your blood volume drops because you haven’t been drinking enough fluids, or you’ve lost fluids through vomiting, diarrhea, or heavy sweating, your blood pressure follows. Even mild dehydration can produce noticeable symptoms.

Nutritional deficiencies play a role too. When your body lacks vitamin B12 or folate, it produces red blood cells that are too large and don’t carry oxygen efficiently. This type of anemia reduces the oxygen supply reaching your tissues, which can lower blood pressure and leave you feeling weak and exhausted.

Heart conditions that affect how much blood the heart pumps, including very slow heart rates and problems with heart valves, can lead to chronically low pressure. Endocrine disorders that affect hormone production, particularly those involving the adrenal or thyroid glands, are another category of underlying causes worth investigating if low blood pressure is persistent and unexplained.

Medications are a major contributor. Drugs prescribed for high blood pressure, including diuretics (which reduce fluid volume) and beta blockers (which slow heart rate), can overshoot their target. Certain antidepressants and medications for Parkinson’s disease can also lower blood pressure as a side effect. If your symptoms started or worsened after beginning a new medication, that connection is worth exploring with your prescriber.

Pregnancy

Blood pressure commonly dips during pregnancy, particularly in the middle months. Medical textbooks have traditionally taught that pressure drops by 10 to 15 mmHg mid-pregnancy, but a large-scale analysis from the NIHR Oxford Biomedical Research Centre found the actual dip in diastolic pressure averages just 1 to 2 mmHg below early pregnancy levels. So while some decrease is normal, dramatic drops deserve attention.

When Low Blood Pressure Becomes Dangerous

Extreme hypotension can lead to shock, a life-threatening condition where organs don’t get enough blood to function. The warning signs are distinct from ordinary lightheadedness: cold, clammy skin, a rapid but weak pulse, pale or ashen skin color, rapid shallow breathing, confusion (especially in older adults), little or no urine output, and loss of consciousness. Shock requires emergency treatment. If someone shows these signs, they need immediate medical help.

Managing Low Blood Pressure Day to Day

For people with ongoing low blood pressure that causes symptoms, several practical strategies can help. Drinking more water increases blood volume and counters dehydration. Adding more salt to your diet can raise blood pressure, though this needs to be balanced carefully since excess sodium carries its own risks, particularly for heart health in older adults.

Compression stockings that press on the legs help push blood back toward the heart and improve circulation. Some people find abdominal compression belts easier to wear and equally effective. Small, frequent meals instead of large ones can reduce postprandial drops. Standing up slowly, especially first thing in the morning, gives your cardiovascular system time to adjust.

When lifestyle changes aren’t enough, medications are available for orthostatic hypotension. These work either by increasing blood volume or by tightening blood vessels to raise standing blood pressure. The choice depends on your specific pattern of symptoms and what’s driving them.

A single low reading at the doctor’s office doesn’t necessarily mean much. Blood pressure fluctuates throughout the day based on hydration, meals, stress, sleep, and activity. The more useful picture comes from tracking your numbers over time and noting whether symptoms line up with the dips. If you consistently read below 90/60 and feel fine, your body is likely well-adapted.
