What Does Mineral Water Do for the Body?

Mineral water supplies your body with calcium, magnesium, and other dissolved minerals in a form that’s surprisingly well absorbed. To legally carry the “mineral water” label in the U.S., bottled water must contain at least 250 parts per million of total dissolved solids and come from a protected underground source. That mineral content is what separates it from regular bottled or tap water, and it’s what gives mineral water its potential health effects.

Calcium Absorption Rivals Dairy

One of the most practical things mineral water does is deliver calcium your body can actually use. A study published in The American Journal of Clinical Nutrition compared calcium absorption from a calcium-rich mineral water (containing 467 mg of calcium per liter) against milk in healthy young women. The fractional absorption rate was 23.8% from the mineral water versus 25.0% from milk, a difference that wasn’t statistically significant. In other words, your body pulls calcium from mineral water about as efficiently as it pulls calcium from a glass of milk.

This matters for bone health, especially if you’re lactose intolerant, vegan, or simply don’t drink much dairy. A liter of high-calcium mineral water can deliver roughly a third to half of your daily calcium needs, depending on the brand. Not all mineral waters are equally rich in calcium, though. Labels vary widely, so checking the nutrition panel is the only reliable way to know what you’re getting.

Magnesium and Blood Pressure

Magnesium is the other headline mineral, and its link to blood pressure is well documented. A systematic review and meta-analysis published by the American Heart Association found that magnesium supplementation lowered systolic blood pressure by about 2.8 mm Hg and diastolic pressure by about 2.0 mm Hg compared to placebo. Those are modest numbers for the general population, but the reductions were much larger in people who already had high blood pressure or low magnesium levels: systolic drops of roughly 6 to 8 mm Hg and diastolic drops of 3 to 5 mm Hg.

In people with normal blood pressure, the effect didn’t reach statistical significance. So mineral water rich in magnesium is most likely to make a measurable difference if your magnesium intake is already low or if you’re managing hypertension. One study cited in the review specifically found that mineral water reduced blood pressure in people with low urinary magnesium and calcium levels.

Digestive Benefits

Sulfate-rich mineral water has a direct effect on your gut. Magnesium sulfate works osmotically in the large intestine, meaning it draws water into the bowel. This softens stool, speeds up transit time, and can relieve functional constipation. A double-blind, randomized, placebo-controlled study confirmed this mechanism, finding that sulfate-rich mineral water improved both stool consistency and bowel movement frequency.

Bicarbonate, another common mineral in these waters, helps neutralize stomach acid. If you’ve ever noticed that certain mineral waters settle your stomach after a heavy meal, the bicarbonate content is likely why. Together, sulfate and bicarbonate make some mineral waters a mild, food-based digestive aid.

Effects on Blood Sugar and Cholesterol

Research in Frontiers in Nutrition examined how different mineral water compositions affected markers of metabolic syndrome. Waters rich in sulfate, magnesium, and bicarbonate were the most effective at reducing blood levels of total cholesterol, HDL cholesterol, and glucose when combined with a low-calorie diet. The study noted that mineral water “drinking therapy” can influence glucose and lipid metabolism, but the effects depend heavily on the specific mineral composition.

This is an area where the type of mineral water matters as much as whether you drink it at all. Not every bottle on the shelf has the right combination of minerals to move the needle on metabolic markers. The research points to sulfate and magnesium as the key players, so if metabolic health is your goal, look for those on the label.

Kidney Stones: Help, Not Harm

A common concern is whether the calcium in mineral water increases kidney stone risk. The evidence suggests the opposite. Calcium from mineral water binds with oxalate (a compound found in foods like spinach, nuts, and chocolate) in the digestive tract. Once bound together, the calcium-oxalate complex is excreted rather than absorbed. This actually reduces the amount of free oxalate that reaches your kidneys, where it would otherwise crystallize into stones.

In clinical observations, urinary calcium levels didn’t increase despite the high calcium content of the mineral water consumed. The calcium went to work in the gut rather than accumulating in the urinary tract. For people prone to calcium oxalate stones, drinking calcium-rich mineral water with meals may be protective rather than risky.

Hydration Compared to Tap Water

If you’re wondering whether mineral water hydrates you better than regular water, the straightforward answer is no. Ohio State University’s health center notes that all types of water provide adequate hydration and none is more hydrating than another. The advantage of mineral water isn’t superior hydration. It’s the minerals riding along with the water. You’d get the same fluid replacement from tap water, but you wouldn’t get the calcium, magnesium, or sulfate that drive the other benefits on this list.

What to Look for on the Label

Mineral content varies enormously between brands. Some mineral waters contain over 500 mg of calcium per liter, while others have barely any. The same goes for magnesium and sulfate. If you’re drinking mineral water for a specific benefit, the label is everything. Here’s what to prioritize based on your goal:

  • Bone support: Look for calcium content above 300 mg/L.
  • Blood pressure: Choose waters with higher magnesium, typically above 50 mg/L.
  • Digestion: Sulfate-rich varieties (above 200 mg/L) have the strongest evidence for constipation relief.
  • Kidney stone prevention: High-calcium water consumed with meals helps bind dietary oxalate.

Sodium content is also worth checking. Some mineral waters contain enough sodium to matter if you’re watching your intake, particularly European brands sourced from mineral-heavy springs. For most people, though, the sodium levels in mineral water are far below what you’d get from processed food and aren’t a concern.
