Shoulder bursitis typically feels like a deep, dull ache around the outer or top part of the shoulder that sharpens when you lift your arm. The pain is often poorly localized, meaning it’s hard to point to one exact spot, and it gets noticeably worse with overhead movements or when pressure is applied to the area. The sensation can range from a stiff, achy background discomfort at rest to a sharp, shooting pain during activity.
The Baseline Ache vs. the Sharp Catch
Most people with shoulder bursitis describe two distinct layers of pain. The first is a constant or near-constant dull ache that sits deep in the shoulder. It can make the whole joint feel stiff and heavy, even when you’re not using it. This ache is often hard to pinpoint because it doesn’t come from a specific tendon or muscle. It radiates from the inflamed bursa, a small fluid-filled cushion that sits between the bone at the top of your shoulder and the tendons beneath it.
The second layer is a sharper, more specific pain that kicks in with movement. Lifting your arm out to the side, reaching overhead, or pushing something away from your body can produce a pinching or catching sensation inside the shoulder. This sharp pain is most pronounced in a specific arc of motion, roughly between 60 and 120 degrees of arm elevation. Below that range, the inflamed bursa has room. Above it, the bursa clears the bony overhang. But in that middle zone, the swollen tissue gets compressed, and you feel it.
Movements That Make It Worse
Overhead activities are the most reliable trigger. Reaching into a high cabinet, washing your hair, throwing a ball, painting a ceiling, or lifting boxes above shoulder height all force the top of the arm bone closer to the bony roof of the shoulder. That narrows the space where the bursa sits and squeezes the inflamed tissue. The result is a flare of pain that can linger for minutes or hours after the activity stops.
Repetitive motions are a common culprit both for causing bursitis and for aggravating it once it’s established. Sports that involve repeated overhead swings or throws (tennis, baseball, swimming, volleyball) put the bursa under friction over and over. So does physical labor like hammering, lifting heavy objects, or any job that keeps your arms above shoulder level for extended periods. Even smaller, everyday tasks like buckling a seatbelt or reaching behind your back to tuck in a shirt can provoke a jolt of pain if the bursa is already inflamed.
Why It Hurts More at Night
One of the most frustrating features of shoulder bursitis is that it often feels worse when you lie down. During the day, your brain is occupied with tasks and can push the pain to the background. At night, without those distractions, the ache becomes much more noticeable. Lying on the affected side directly compresses the inflamed bursa against the mattress, which can turn a manageable ache into a throbbing pain that wakes you up repeatedly.
Sleeping on your back tends to be the most comfortable position. If you can only sleep on your side, avoid the affected shoulder. Some people find relief by placing a flat pillow at waist height and leaving a gap between it and their head pillow, creating a channel for the sore arm to rest without pressure. Front sleeping generally makes things worse because it pushes the shoulder forward and pinches the bursa.
How It Changes Over Time
Acute shoulder bursitis, the kind that flares after a weekend of painting or a new workout routine, tends to feel like a sudden onset of pain and stiffness. The sharp, pinching quality is front and center, and the shoulder may feel warm or slightly swollen on the outside. At this stage the pain often responds quickly to rest and reducing the activity that triggered it.
When bursitis becomes chronic, the character of the pain shifts. The sharp catches during movement may still happen, but the dominant sensation becomes a persistent, low-grade ache that doesn’t fully go away even with rest. Stiffness increases, and you may start unconsciously avoiding certain movements, which over weeks and months can lead to muscle weakness and reduced range of motion. The shoulder starts to feel “heavier” and less reliable, and tasks that were manageable early on, like pulling on a jacket, become more difficult.
How It Differs From a Rotator Cuff Tear
Shoulder bursitis and rotator cuff problems often overlap, since the bursa sits right next to the rotator cuff tendons and both structures get irritated by the same movements. In many cases, bursitis and rotator cuff tendinopathy occur together. But there are differences that help distinguish them.
Bursitis pain is typically a dull, poorly localized ache that worsens during that 60-to-120-degree arc of lifting. A partial rotator cuff tear can feel similar. A complete rotator cuff tear, though, produces sudden, significant weakness. You may not be able to hold your arm up at all, and rotating your arm outward (like turning a doorknob away from your body) becomes noticeably weak. With bursitis alone, you still have the strength to move through the pain. The pain limits you, but the muscles still work. If you notice weakness that isn’t just from guarding against pain, that points more toward a structural tear.
Frozen shoulder is another condition people confuse with bursitis. With frozen shoulder, the range of motion shrinks in every direction, including when someone else tries to move your arm for you. Bursitis limits movement primarily because of pain, not because the joint itself is locked.
What Happens During a Physical Exam
If you see a provider about shoulder pain, they’ll likely run through a series of movement-based tests to narrow down the cause. The painful arc test is one of the simplest: you lift your arm out to the side and back down while the examiner notes where in the arc pain starts and stops. Pain between roughly 60 and 120 degrees strongly suggests bursitis or impingement.
Two other common tests are more specific. In one, the examiner lifts your arm forward to about 90 degrees and then rotates it inward, compressing the bursa. In another, they push your arm upward through its full range while holding your shoulder blade still to prevent it from compensating. Pain during either of these movements points toward subacromial impingement, the mechanical pinching that drives most shoulder bursitis. In some cases, a numbing injection into the bursa can be used to confirm the diagnosis. If the pain disappears after the injection, the bursa was the source.
Imaging isn’t always necessary for a straightforward case, but an MRI or ultrasound can rule out a rotator cuff tear or other structural damage if the clinical picture is unclear or if symptoms don’t improve with initial treatment.
