What Does Tadalafil Do? Effects, Uses & Warnings

Tadalafil relaxes smooth muscle in blood vessels, allowing more blood to flow to specific areas of the body. It does this by blocking an enzyme called PDE5, which normally breaks down a chemical signal that keeps muscles relaxed. This single mechanism is why tadalafil treats three distinct conditions: erectile dysfunction, enlarged prostate symptoms, and high blood pressure in the lungs.

How Tadalafil Works in the Body

When you’re sexually aroused, nerve endings and blood vessel walls release nitric oxide. This triggers production of a molecule called cGMP, which acts like a “relax” signal for smooth muscle cells. It lowers calcium levels inside those cells, causing them to loosen up. In the penis, that relaxation opens blood vessels and fills the spongy tissue with blood, producing an erection.

Normally, PDE5 breaks down cGMP almost as fast as it’s made. Tadalafil blocks PDE5 from doing its job, so cGMP accumulates and the relaxation effect is stronger and lasts longer. One important detail: tadalafil doesn’t create the relaxation signal on its own. It amplifies the signal that’s already there. Without sexual stimulation triggering nitric oxide release, the drug has no erection-producing effect.

What Tadalafil Treats

Erectile Dysfunction

This is the most common use. By enhancing blood flow to the penis during arousal, tadalafil helps men achieve and maintain erections firm enough for sex. It’s available in two dosing approaches: an as-needed dose (10 or 20 mg taken before sexual activity) or a lower daily dose (2.5 or 5 mg taken every day regardless of planned activity). The daily option keeps a steady level of the drug in your system so you don’t need to plan around a pill.

Enlarged Prostate Symptoms

PDE5 is also present in the smooth muscle of the prostate and bladder. By relaxing that tissue, tadalafil at a low daily dose (5 mg) can ease urinary symptoms caused by benign prostatic hyperplasia, or BPH. That includes weak stream, frequent urination, and the feeling of not fully emptying the bladder. For men who have both ED and BPH, a single daily dose can address both problems.

Pulmonary Arterial Hypertension

In the lungs, PDE5 inhibition widens the blood vessels that carry blood from the heart to the lungs. This lowers the abnormally high pressure in those vessels, reducing strain on the right side of the heart. The dose used for this condition is significantly higher (typically 40 mg daily) and is prescribed under a different brand name than the ED version.

How Long It Lasts

Tadalafil stands out from similar drugs because of its unusually long duration. It can start working in as little as 15 minutes, though most men find it takes closer to two hours to reach full effect. The half-life is about 17.5 hours in younger men and roughly 21.6 hours in older men, which translates to a practical window of effectiveness lasting up to 36 hours. That extended timeline is why it earned the nickname “the weekend pill.” You can take it Friday evening and still have reliable effect Sunday morning.

For comparison, similar drugs in this class wear off in four to six hours. The long window doesn’t mean you’ll have an erection for 36 hours. It means that during that period, sexual stimulation can produce an erection more easily than it otherwise would.

Common Side Effects

Most side effects stem from the same blood-vessel-relaxing action that makes the drug work. In clinical trials, headache was the most frequent complaint, reported by up to 42% of participants at higher doses. Muscle pain (up to 14%) and back pain (up to 12%) are more specific to tadalafil than to other drugs in its class, likely because PDE5 is present in skeletal muscle tissue. Indigestion affected up to 13% of users.

These side effects are generally mild and tend to fade within a few hours. They’re also dose-dependent, so men on the low daily dose experience them less often than those taking 20 mg as needed. Back pain and muscle aches, when they occur, typically show up 12 to 24 hours after taking the pill and resolve within two days.

Interactions That Matter

The most dangerous interaction is with nitrate medications, commonly prescribed for chest pain. Nitrates work by boosting the same nitric oxide pathway that tadalafil amplifies. Combining them causes additive, powerful blood vessel dilation that can drop blood pressure to life-threatening levels. This includes nitroglycerin tablets, nitroglycerin patches, and isosorbide pills. Because tadalafil stays in your system so long, at least 48 hours must pass between your last tadalafil dose and any nitrate use.

A drug called riociguat, used for pulmonary hypertension, is also strictly off-limits for the same reason. It stimulates the same enzyme that nitric oxide activates, and the combination with tadalafil can cause a severe blood pressure drop.

Alpha-blockers, often prescribed for high blood pressure or BPH, can also interact. Both alpha-blockers and tadalafil lower blood pressure, so using them together increases the risk of dizziness or fainting, especially when standing up quickly.

Food, Alcohol, and Absorption

Unlike some similar medications, tadalafil absorption is not significantly affected by food. You can take it with or without a meal and expect roughly the same effect, which adds to its convenience.

Alcohol is a different story. Both tadalafil and alcohol lower blood pressure, and combining them amplifies that effect. Keeping intake to no more than a few drinks reduces the risk of dizziness, lightheadedness, or fainting. Large amounts of grapefruit juice can also increase tadalafil levels in the blood by interfering with how your liver processes the drug, potentially intensifying both its effects and its side effects.

Who Should Avoid It

Beyond the nitrate and riociguat restrictions, tadalafil is not recommended for people who have had a heart attack within the past 90 days, a stroke within the past 6 months, or unstable angina. Uncontrolled high or low blood pressure and certain heart failure classifications also rule it out. These aren’t side effects of the drug itself but reflect the cardiovascular demands that sexual activity places on the body.

Rare but serious events to be aware of include prolonged erection lasting more than four hours (which requires emergency treatment to prevent tissue damage), sudden vision loss in one or both eyes, and sudden hearing loss. These are uncommon but warrant immediate medical attention if they occur.
