What Does the Gluteus Medius Do for Hip Stability?

The gluteus medius is the primary hip abductor, meaning it pulls your leg out to the side and, more importantly, keeps your pelvis level every time you take a step. It sits on the outer surface of your hip, beneath the gluteus maximus, running from the broad wing of your pelvis down to the bony bump at the top of your thighbone (the greater trochanter). While it rarely gets the attention of its larger neighbor, the gluteus medius is arguably more critical for everyday movement. Without it working properly, walking, running, climbing stairs, and even standing on one leg all break down.

How It Keeps You Stable While Walking

Every step you take is briefly a single-leg stance. During that fraction of a second, your entire body weight shifts onto one leg, and the gluteus medius on that side fires to prevent the opposite hip from dropping. It activates right at initial contact with the ground and continues through the loading phase, acting as a pelvic stabilizer against gravity. Without that stabilization, your pelvis would tilt toward the swinging leg with every stride.

This is why gluteus medius weakness shows up so clearly in the way people walk. A classic sign is the Trendelenburg gait: the pelvis sags on the opposite side during each step, producing a noticeable limp. In severe cases, people compensate by lurching their upper body toward the weak side to keep their center of gravity balanced. When the weakness is on both sides, the pelvis rocks back and forth with each step, creating what clinicians call a waddling gait.

More Than Hip Abduction

Pulling the leg sideways (hip abduction) is the textbook function, but the gluteus medius does more than that. Its fibers fan out in different directions, so different portions of the muscle contribute to different movements. The front fibers assist with rotating the thigh inward, while the rear fibers help rotate it outward. Together, the entire muscle also works eccentrically, meaning it controls and slows down the opposite motions. When your knee drifts inward during a landing or a squat, it’s the gluteus medius that’s supposed to resist that collapse by controlling hip adduction and internal rotation on the way down.

Its Role in Protecting Your Knees

Because the gluteus medius controls what happens at the hip in the frontal plane, it has a surprisingly strong influence on knee alignment. When the muscle is weak or slow to activate, the thigh tends to drift inward during activities like jumping, landing, or running. This inward drift pushes the knee into a valgus position, where it angles toward the midline of the body. That position increases stress on the ACL and other knee ligaments.

Research on young trained males performing single-leg jumps found that sufficient gluteus medius activation acts as the primary frontal-plane protector of the knee by reducing valgus and the resulting joint stress. When the hip abductors weren’t pulling their weight, the hamstring on the outer thigh had to compensate as a secondary stabilizer, a pattern that signals the knee is under greater strain than it should be. This chain reaction is one reason physical therapists so often prescribe hip-strengthening exercises for people with knee pain or ACL injury risk.

What Happens When It’s Weak or Injured

Gluteus medius problems generally fall into two categories: weakness and tendon damage. Weakness is extremely common, especially in people who sit for long hours, and it often contributes to a chain of symptoms rather than causing obvious hip pain. You might notice your knee caving inward during squats, lower back soreness after long walks, or an inability to balance comfortably on one leg. A positive Trendelenburg sign, where your pelvis drops on one side when you stand on the opposite foot, is the hallmark clinical finding.

Tendon problems are a different story. Gluteal tendinopathy involves the breakdown or deterioration of the tendon tissue where the muscle attaches at the greater trochanter. It causes chronic, often severe hip pain on the outer side of the hip that typically does not improve with rest alone. This is distinct from gluteal tendinitis, where repetitive movements create small micro-tears that lead to inflammation. Tendinitis pain tends to be deep in the hip area and usually responds to rest and icing, while tendinopathy requires targeted physical therapy exercises to promote tissue repair.

Best Exercises for Activation

Not all exercises work the gluteus medius equally. EMG studies measuring muscle activation as a percentage of maximum voluntary contraction show a wide range depending on the movement. The exercises break down into three tiers.

Moderate activation (21 to 40% of max) includes exercises like bridging on a stable surface (28%), mini-squats on one leg (36%), clamshells (38 to 40%), and sideways lunges (39%). These are good starting points for someone who’s rehabbing an injury or just beginning to target the muscle.

High activation (41 to 60%) comes from movements like wall squats (52%), side-lying hip abduction (56%), pelvic drops (57%), and single-leg deadlifts (58%). These demand more hip stability and are appropriate once you’ve built a baseline of strength.

The highest activation (above 60%) comes from single-leg squats (64%) and the side bridge held in a neutral spine position (74%). These are advanced exercises that challenge the gluteus medius close to its maximum capacity, making them effective for athletes or anyone looking to build serious hip stability. Progressing through these tiers, rather than jumping to the hardest exercises, gives the muscle time to adapt and reduces the risk of compensating with other muscles.
