It’s FDA-approved for managing anxiety disorders and for short-term relief of anxiety symptoms, and it remains one of the most widely recognized medications in its class.

When GABA binds to its receptors, it opens tiny channels that let negatively charged particles flow into nerve cells, quieting them down. Valium doesn’t activate these receptors on its own. The drug itself has a half-life of up to 48 hours, meaning it takes roughly two days for your body to clear half of a single dose. But Valium also breaks down into an active metabolite that continues working, with a half-life of up to 100 hours. This long tail means the drug accumulates with repeated doses, producing a smooth, sustained effect rather than sharp peaks and valleys.
How Valium Compares to Other Benzodiazepines
All benzodiazepines work through the same GABA-boosting mechanism, but they differ in potency, speed, and duration. Valium sits on the longer-acting end of the spectrum. Alprazolam (Xanax), by comparison, has a half-life of about 12 hours, so it wears off much faster and needs to be taken more frequently. Lorazepam (Ativan) falls in between at around 15 hours.
In terms of dose equivalence, roughly 5 to 10 mg of Valium provides a similar effect to 0.5 to 1 mg of alprazolam or 1 to 2 mg of lorazepam. Valium’s longer duration can be an advantage for steady, around-the-clock anxiety control, but it also means the drug lingers if side effects occur. The choice between benzodiazepines often comes down to whether a person needs quick, short-acting relief (like alprazolam for panic attacks) or steadier coverage throughout the day.
Typical Dosing for Anxiety
The usual starting dose for anxiety is 2 mg taken three times a day. This can be increased to 5 to 10 mg three times daily depending on severity and response. People over 65, or those with kidney, liver, or breathing problems, typically start at a lower dose because the drug is cleared more slowly in these groups, which raises the risk of excessive sedation.
Common Side Effects
The most frequently reported side effects are drowsiness, fatigue, muscle weakness, and ataxia (unsteadiness or poor coordination). These effects are essentially exaggerated versions of what the drug is designed to do. Because Valium enhances inhibition across the brain, not just in the circuits responsible for anxiety, it can slow motor control, reaction time, and mental sharpness alongside the desired calming effect.

Why It’s Meant for Short-Term Use
Clinical guidelines generally recommend limiting benzodiazepine use to one to four weeks. This isn’t based on the drug losing effectiveness at that point. It’s driven by the risk of physical dependence, which can develop with regular use even at prescribed doses. The brain adapts to the constant presence of the drug by reducing its own sensitivity to GABA, so stopping abruptly after prolonged use can trigger a rebound of the very symptoms Valium was treating, plus additional withdrawal effects.
Withdrawal symptoms from long-acting benzodiazepines like Valium can include heightened anxiety, panic attacks, tremor, sleep disruption, sweating, muscle spasms, and hypersensitivity to loud noises. In severe cases, withdrawal can cause seizures or psychotic symptoms. Because diazepam and its active metabolite leave the body slowly, withdrawal symptoms may not begin until several days after the last dose, and the timeline varies widely between individuals.
Long-term use beyond four weeks is not ruled out entirely, but guidelines treat it as an exception rather than a default.
