What Does Varicose Vein Pain Feel Like?

Varicose vein pain typically feels like a deep, dull ache or heaviness in the legs, often accompanied by burning, throbbing, or muscle cramping in the lower legs. It’s not sharp or sudden like a pulled muscle. Instead, it builds gradually over the course of the day and tends to peak after long periods of standing or sitting. The sensation is distinctive enough that most people describe it less as “pain” and more as legs that feel swollen, tired, and uncomfortably full.

The Core Sensations

People with varicose veins most commonly report an achy, heavy feeling in the legs. This heaviness can feel like wearing ankle weights you can’t take off. Alongside that, you may notice burning or throbbing along the path of a visible vein, muscle cramps (especially in the calves), and swelling in the lower legs. Some people also describe itching or a tight, restless sensation around the affected veins.

The pain is rarely constant at first. It tends to follow a daily pattern: minimal in the morning, then gradually worsening as the day goes on. Gravity is the main driver. Every hour you spend upright, more blood pools in your lower legs, stretching the already weakened vein walls further. By evening, the cumulative pressure makes legs feel noticeably heavier, achier, and more swollen. Lying down or elevating your legs brings relief relatively quickly because it lets gravity drain the pooled blood back toward your heart.

Why Varicose Veins Hurt

The pain comes from blood pooling in veins that can no longer push it efficiently back up to the heart. Normally, one-way valves inside your leg veins prevent blood from flowing backward. When those valves fail, blood collects and stretches the vein walls outward. This stretching directly activates nerve endings embedded in and around the vein walls. Research in physiology has shown that these nerve fibers are “polymodal,” meaning they respond to both physical stretching and chemical signals released by the swollen tissue. So the pain has two sources: the mechanical pressure of an overfilled vein and the inflammatory irritation that develops around it.

This is also why the pain worsens with heat. Warm weather, hot baths, and heated floors all cause veins to dilate further, increasing the pooling and the stretch on those nerve endings. Cold temperatures and cool water tend to have the opposite effect, temporarily shrinking veins and easing discomfort.

Nighttime Symptoms and Restless Legs

Many people with varicose veins notice their legs feel restless or “crawly” at night, making it hard to fall asleep. This isn’t coincidental. A study of 207 patients found that nearly 79% of those with confirmed superficial vein problems also reported restless leg symptoms, compared to about 44% of those without vein issues. That’s a significant overlap, and it suggests the same venous pressure that causes daytime aching can trigger that irresistible urge to move your legs at night. If you’ve been told you have restless legs but also have visible varicose veins, the two may be connected.

How Pain Changes as Veins Get Worse

Varicose vein disease progresses through stages, and the character of the pain shifts at each one. Doctors classify venous disease on a scale from C0 (no visible signs) to C6 (open skin ulcers). Early-stage varicose veins (C2) produce the classic aching and heaviness. Once swelling becomes persistent (C3), the legs may feel tight and uncomfortable even in the morning.

At more advanced stages (C4 and beyond), the skin itself starts to change. You may notice darkened, discolored patches around the ankles, dry or flaky skin, and a condition called lipodermatosclerosis, where the fat and skin in your lower legs gradually harden. In its acute phase, this causes sudden discoloration, warmth, and tenderness that can look like an infection. Over time, the skin becomes thick, firm, and persistently painful, particularly when standing or walking for extended periods. The lower leg near the ankle can narrow while the calf and foot stay swollen, creating a shape sometimes compared to an inverted champagne bottle. Pain at this stage shifts from intermittent aching to a chronic, burning soreness that doesn’t fully resolve with rest.

When the Pain Changes Suddenly

Varicose veins can occasionally develop a clot in the superficial vein, a condition called superficial thrombophlebitis. This feels distinctly different from the usual dull ache. You’ll notice a specific spot along a vein that becomes red, warm, hard, and tender to the touch. The vein itself may feel like a firm cord under the skin. This is usually treatable and not dangerous on its own, but it warrants medical attention.

The more serious concern is deep vein thrombosis (DVT), where a clot forms in a vein deep inside the muscle. DVT pain tends to involve sudden swelling of the entire leg (not just around a visible vein), along with diffuse tenderness and pain that doesn’t match the usual pattern of your varicose vein symptoms. If leg swelling and pain become severe, or if you develop shortness of breath, chest pain, or start coughing up blood, that combination could indicate a clot has traveled to your lungs, which is a medical emergency.

What Eases the Pain

The simplest relief comes from changing your position. Elevating your legs above heart level for 15 to 20 minutes lets pooled blood drain and reduces the pressure on stretched vein walls. Regular walking also helps because calf muscles act as a pump, squeezing blood upward through the veins with each step. Prolonged sitting or standing without movement is the worst scenario for varicose vein pain.

Compression stockings are the most widely recommended tool for daily symptom management. They work by applying graduated pressure, tighter at the ankle and looser toward the knee, which helps veins move blood upward more efficiently. Over-the-counter options typically provide 10 to 15 mmHg of pressure, which is enough for mild discomfort. Medical-grade stockings range from low pressure (under 20 mmHg) to medium (20 to 29 mmHg) to high (30 to 40 mmHg). Most people with symptomatic varicose veins find meaningful relief in the 20 to 30 mmHg range. Higher pressures are typically reserved for more advanced swelling or skin changes, and those usually require a fitting to make sure they work properly without being too tight.

Cool water on the legs, avoiding prolonged heat exposure, and staying physically active all reduce symptoms over time. For people whose pain persists despite these measures, several minimally invasive procedures can close off or remove the faulty veins, which eliminates the source of the pooling and usually resolves the pain within weeks.
