Water poisoning typically starts with nausea, a bloated feeling, and a headache that comes on within an hour or two of drinking too much water. These early warning signs can escalate quickly into confusion, muscle weakness, and in severe cases, seizures or loss of consciousness. The underlying problem is that excess water dilutes the sodium in your blood, causing your cells, especially brain cells, to swell.

The first symptoms of water poisoning feel deceptively ordinary. You’ll likely notice nausea, a sense of fullness or bloating in your stomach, and a dull headache. These can appear after drinking roughly a gallon (3 to 4 liters) of water over just one to two hours, though the exact threshold varies from person to person based on body size, kidney function, and how much sodium you’ve taken in with food.
Other early signs include drowsiness, muscle cramps or weakness, and general fatigue. Your hands, feet, or belly may start to swell as your body holds onto the excess fluid. At this stage, the fix is simple: stop drinking. If you feel nauseous, bloated, or headachy during or after heavy water intake, those are your body’s signals that you’ve had too much.

What makes water poisoning dangerous is what happens at the cellular level. When you flood your body with more water than your kidneys can process, the sodium concentration in your blood drops. This creates an imbalance between the fluid outside your cells and the fluid inside them. Water rushes into cells to equalize the difference, causing them to swell.
Most of your organs can tolerate some swelling. Your brain cannot. Encased in your rigid skull, swollen brain tissue has nowhere to expand, which creates pressure. This is why neurological symptoms dominate the picture as water poisoning progresses, and why the condition can turn life-threatening so fast. Severe symptoms are the ones that land people in emergency rooms: vomiting, extreme drowsiness where you can barely stay awake, seizures, coma, and in the worst cases, respiratory or cardiac arrest.
The speed of this escalation depends largely on how fast sodium levels drop. A gradual decline over days (which can happen with certain medications or chronic health conditions) tends to produce subtler symptoms because the brain has time to adapt. But when sodium plummets rapidly, as it does when someone drinks massive amounts of water in a short window, the onset can be explosive. Who Is Most at Risk
Water poisoning doesn’t only happen to people who chug water on a dare. Several common scenarios raise the risk substantially.
- Endurance athletes. Marathoners and triathletes lose sodium through sweat and then replace it with plain water, diluting their blood sodium even further. Drinking aggressively during long events without electrolyte replacement is one of the most common real-world causes.
- Recreational drug use. Ecstasy (MDMA) increases the risk of severe and even fatal water poisoning. The drug triggers excessive thirst, impairs the body’s ability to regulate water balance, and people at parties or festivals may drink far more than their kidneys can handle.
- Certain medications. Some diuretics (water pills), antidepressants, and pain medications interfere with the hormonal and kidney processes that normally keep sodium levels stable. People on these drugs can develop dangerously low sodium even with water intake that would be fine for someone else.
- Older adults. Kidney function declines with age, and many elderly people take medications that affect sodium balance. A change in mental status, new nausea, or unexplained confusion in an older person should raise suspicion.
What Happens at the Hospital
If water poisoning progresses beyond mild symptoms, it becomes a medical emergency. Treatment centers on carefully raising blood sodium levels back to a safe range. This has to be done gradually because correcting sodium too quickly carries its own serious neurological risks. In practice, you can expect IV fluids with a controlled sodium concentration, close monitoring through repeated blood draws, and possibly fluid restriction. Mild cases where someone catches the symptoms early and simply stops drinking may resolve on their own as the kidneys catch up and excrete the excess water.
Recovery time depends on how far sodium dropped and how quickly it happened. People with mild overhydration who stop drinking and eat something salty often feel better within a few hours. Those who reach the point of seizures or coma face a longer and less predictable recovery, with the possibility of lasting neurological effects if treatment is delayed.
