Xanax shows up as a benzodiazepine on a standard drug test. It won’t be listed by name on an initial screening result. Instead, it falls under the broader benzodiazepine category, often abbreviated as BZO on the test panel. Only a follow-up confirmatory test can pinpoint Xanax specifically and distinguish it from other benzodiazepines like diazepam or lorazepam.
How Screening Tests Detect Xanax
Most workplace and clinical drug tests start with an immunoassay, a quick screening method that uses antibodies to detect drug classes rather than individual medications. The antibodies in standard benzodiazepine panels are typically designed to react with certain metabolites. Xanax (alprazolam) breaks down in the body into a metabolite called alpha-hydroxyalprazolam, and most immunoassay kits have good cross-reactivity with that metabolite. So while the test isn’t looking for Xanax by name, it reliably picks up the chemical byproduct your body produces after taking it.
If you take Xanax with a valid prescription, the result will simply read positive for benzodiazepines. At that point, you can typically provide documentation of your prescription to the medical review officer overseeing the test.
What Happens if a Positive Needs Confirmation
When an initial screen comes back positive for benzodiazepines and more detail is needed, the sample goes through confirmatory testing. Labs use advanced techniques like mass spectrometry to separate and identify the exact substances present. This second step can distinguish alprazolam and its metabolite from other benzodiazepines with high accuracy. It’s also the stage that weeds out false positives from the initial screening.
It’s worth noting that benzodiazepines are not part of the standard federal five-panel drug test used in many workplaces, which covers marijuana, cocaine, opiates, amphetamines, and PCP. Benzodiazepines only appear on expanded panels, commonly 10-panel or 12-panel tests. If your employer or testing program uses a basic five-panel screen, Xanax won’t be tested for at all.
Detection Windows by Test Type
How long Xanax remains detectable depends on the type of sample collected:
- Urine: Up to 5 days after the last dose. This is the most common testing method.
- Blood: 1 to 6 days after the last dose.
- Saliva: Up to 2.5 days after the last dose.
- Hair: Detectable starting 1 to 7 days after use, with a detection window spanning up to 3 months.
Alprazolam has a half-life of roughly 6 to 27 hours, meaning the body eliminates half the drug in that window. But “eliminated from the body” and “no longer detectable on a test” are two different things. Trace metabolites linger in urine and hair well after the drug’s effects have worn off.
Factors That Affect How Long Xanax Stays Detectable
These detection windows aren’t fixed numbers. Several variables push the timeline shorter or longer for any individual person.
Dosage and frequency matter most. Higher doses mean more drug for the body to process, and regular daily use leads to accumulation in body tissues. Someone who takes Xanax every day will test positive for a longer stretch than someone who took a single dose once. Body composition plays a role too: Xanax can be stored in body fat, so people with higher body fat percentages may retain detectable levels longer.
Age slows things down. Metabolism generally declines with age, so older adults tend to clear the drug more slowly than younger people. Liver function is another major factor, since the liver is responsible for breaking down alprazolam. Conditions like hepatitis or cirrhosis can significantly extend how long the drug remains in your system and shows up on a test.
Medications That Can Trigger a False Positive
A small number of medications can cause a benzodiazepine immunoassay to light up even when you haven’t taken Xanax or any other benzodiazepine. Known culprits include oxaprozin (an anti-inflammatory), sertraline (an antidepressant), and efavirenz (an HIV medication). These substances share enough structural similarity with benzodiazepine metabolites to fool the initial screening antibodies.
If you’re taking any of these and face a drug test, a confirmatory test will clear the false positive. The advanced testing methods used in confirmation are specific enough to tell these medications apart from actual benzodiazepines. Disclosing your current medications to the testing provider beforehand can also help avoid unnecessary complications.
