Certain dog breeds carry a significantly higher burden of genetic health problems than others, and the list is dominated by two groups: flat-faced (brachycephalic) breeds and giant breeds. English Bulldogs, French Bulldogs, Cavalier King Charles Spaniels, Great Danes, and Golden Retrievers consistently rank among the most health-prone dogs, each for different reasons. Understanding what those reasons are can help you make a more informed choice before bringing a dog home.
English Bulldogs and French Bulldogs
Flat-faced breeds top nearly every list of health-prone dogs, and English Bulldogs and French Bulldogs are the most extreme examples. About 50% of English Bulldogs, French Bulldogs, and Pugs are affected by brachycephalic obstructive airway syndrome, a condition where the shortened skull compresses the airways and makes normal breathing difficult. These dogs snore, overheat quickly, and can struggle with exercise that other breeds handle easily.
Breathing issues are just the starting point. French Bulldogs are also prone to spinal problems, cataracts, and eyelid abnormalities. English Bulldogs face skin infections in their deep facial folds, joint problems, and reproductive difficulties so severe that the vast majority require cesarean sections to deliver puppies. The cumulative toll is reflected in lifespan data from a UK study of over 30,000 dogs: French Bulldogs had the shortest life expectancy of any breed at just 4.53 years, and English Bulldogs came in next at 7.39 years. The national average for all dogs was 11.23 years.
Insurance costs reflect the risk. French Bulldog owners pay an average of $64 to $114 per month for pet insurance, making them one of the most expensive breeds to cover.
Cavalier King Charles Spaniels
Cavalier King Charles Spaniels look healthy on the surface, but they carry one of the highest rates of heart disease of any breed. As many as 90% develop myxomatous mitral valve disease by age 10. In this condition, the valve between two chambers of the heart gradually degenerates, allowing blood to leak backward. Early on, the only sign is a heart murmur your vet picks up during a routine exam. Over time, it can progress to congestive heart failure, causing coughing, exercise intolerance, and fluid buildup in the lungs.
What makes this especially concerning is how early it starts. While mitral valve disease is common in many older small breeds, Cavaliers develop it years sooner. They’re also prone to a neurological condition called syringomyelia, where the skull is too small for the brain, forcing fluid into the spinal cord. This can cause neck pain, sensitivity to touch around the head, and in severe cases, partial paralysis.
Great Danes
Great Danes are the poster breed for a life-threatening emergency called gastric dilatation-volvulus, commonly known as bloat. The stomach fills with gas and then twists on itself, cutting off blood supply. Without emergency surgery within hours, it’s fatal. Great Danes have a 14% prevalence rate for this condition, the highest of any breed. For comparison, the lifetime risk of bloat across large and giant breeds ranges from about 4% to 37%, and Great Danes sit near the top of that range.
Beyond bloat, Great Danes are susceptible to heart disease (particularly a condition where the heart muscle stretches and weakens), hip dysplasia, and bone cancer. Their sheer size also makes every medical intervention more expensive, from anesthesia to medication dosing. Monthly pet insurance premiums run $60 to $85, and that’s before any major claims. Their average lifespan is only about 7 to 10 years.
Golden Retrievers
Golden Retrievers are one of the most popular family dogs in the world, which makes their cancer rates especially striking. A study spanning nearly three decades at a veterinary academic center found that hemangiosarcoma, an aggressive cancer of the blood vessel lining, accounted for 22.64% of cancer deaths in the breed. Lymphoma was the second most common, responsible for 18.40% of cancer deaths.
These aren’t rare cancers appearing in unlucky individuals. Cancer is the leading cause of death in Golden Retrievers overall, and the breed’s risk is substantially higher than the general dog population. The Morris Animal Foundation launched a lifetime study following over 3,000 Golden Retrievers specifically to understand why cancer hits this breed so hard. Beyond cancer, Goldens are also prone to hip and elbow dysplasia, skin allergies, and ear infections.
German Shepherds
German Shepherds are strongly associated with orthopedic problems, particularly hip dysplasia, which affects about 16.3% of the breed. Hip dysplasia occurs when the ball and socket joint of the hip doesn’t develop properly, leading to grinding, pain, and eventually arthritis. In practical terms, you may notice a German Shepherd bunny-hopping when running, having trouble getting up from a lying position, or becoming reluctant to climb stairs.
They’re also prone to a degenerative spinal condition that progressively weakens the hind legs, skin allergies, digestive sensitivities (including a higher rate of exocrine pancreatic insufficiency, where the pancreas can’t properly digest food), and bloat. Their combination of joint, digestive, and immune issues means many German Shepherd owners become frequent visitors to the vet by the time their dog reaches middle age.
Dachshunds
Dachshunds face a very specific structural problem. Their long backs and short legs put enormous stress on the spinal discs, making them 10 to 12 times more likely than other breeds to develop intervertebral disc disease. An estimated 19% to 24% of Dachshunds show signs of this condition during their lifetime, which can range from mild back pain and stiffness to sudden paralysis of the hind legs.
A Dachshund with a ruptured disc may yelp when picked up, refuse to jump onto furniture, or drag its back legs. Mild cases can sometimes be managed with rest and anti-inflammatory medication, but severe cases require surgery, and recovery can take weeks of crate rest and physical therapy. Owners can reduce risk somewhat by preventing their Dachshund from jumping on and off furniture and by keeping them at a lean weight, but the underlying vulnerability is baked into the breed’s body shape.
Why Purebreds Are More Vulnerable
The thread connecting all these breeds is inbreeding. Purebred dogs are bred within closed gene pools, and some breeds have been narrowed dramatically. The degree of inbreeding varies widely: Jack Russell Terriers have a relatively low inbreeding coefficient of about 0.10, while Boxers sit at 0.40 and Basenjis as high as 0.57. Higher inbreeding means less genetic diversity, which reduces the immune system’s ability to fight off disease and increases the chance that harmful genetic mutations get passed to every generation.
Research has shown that purebred dogs carry higher levels of genomic damage compared to mixed breed dogs, specifically more micronuclei, which are markers of DNA breakage in cells. In humans, elevated micronuclei levels are linked to higher rates of cancer, cardiovascular disease, and neurological problems, and researchers believe the same relationship holds in dogs. This helps explain why certain purebred lines seem to develop the same cancers, heart conditions, or immune disorders generation after generation.
The Cost of Breed-Related Health Problems
Veterinary bills add up quickly for high-risk breeds, and pet insurance pricing reflects exactly which dogs insurers expect to cost the most. Mastiffs top the list at $69 to $130 per month, driven by hip dysplasia, cataracts, cancer, and a short lifespan. Rottweilers run $65 to $110 monthly due to bone cancer, joint problems, and heart disease. Bernese Mountain Dogs cost $60 to $103 per month, largely because of their extremely high cancer risk, particularly a type called histiocytic sarcoma.
These premiums aren’t arbitrary. Insurers base them on statistical claims data, so the breeds with the highest premiums are, by definition, the ones generating the most medical expenses. If you’re considering one of these breeds, budgeting $1,000 to $1,500 per year just for insurance is realistic, and that doesn’t cover deductibles or procedures that may be excluded as pre-existing conditions.
