Alcohol affects nearly every organ system in your body, starting within minutes of your first sip. In the short term, it slows brain signaling, impairs coordination, and raises your heart rate. Over months and years of heavy use, it can damage your liver, shrink your brain, increase cancer risk, and weaken your heart muscle. How much damage occurs depends largely on how much and how often you drink.
How Alcohol Changes Brain Signaling
Your brain relies on a balance between signals that excite nerve cells and signals that calm them down. Alcohol tips that balance sharply toward the “calm” side. It enhances the activity of your brain’s main inhibitory signaling system while simultaneously blocking the main excitatory one. This is why even a couple of drinks can make you feel relaxed, less anxious, and socially uninhibited.
At higher doses, that same mechanism produces slurred speech, poor coordination, slowed reaction time, and impaired judgment. At a blood alcohol concentration (BAC) of 0.05%, you may feel uninhibited with lowered alertness. At 0.08%, the legal driving limit in most U.S. states, muscle coordination drops noticeably and detecting danger becomes harder. By 0.15%, mood shifts, nausea, vomiting, and loss of balance are common. A BAC between 0.30% and 0.40% can cause loss of consciousness and alcohol poisoning, which is potentially fatal.
With chronic heavy drinking, the brain adapts to alcohol’s constant presence by ramping up its excitatory signals and dialing down its calming ones. This is why tolerance builds: you need more alcohol to feel the same effect. It also explains why sudden withdrawal can be dangerous. Once alcohol is removed, the brain is left in a hyperexcitable state, which can trigger insomnia, anxiety, palpitations, agitation, seizures, and in severe cases, delirium.
What Happens in Your Liver
Your liver does the heavy lifting when it comes to clearing alcohol from your bloodstream. Enzymes in the liver first convert alcohol into acetaldehyde, a toxic compound and known carcinogen. A second set of enzymes then breaks acetaldehyde down into acetate, which your body can eliminate as water and carbon dioxide. Under normal conditions, acetaldehyde is short-lived. But when you drink heavily or frequently, the liver’s capacity to keep up is overwhelmed, and toxic byproducts linger longer.
Alcohol-related liver disease progresses through three stages. In the earliest stage, called fatty liver, excess fat accumulates in liver cells because you’re regularly consuming more alcohol than the liver can process. This stage is usually silent, with no obvious symptoms, and it’s reversible if you stop drinking. If heavy drinking continues, the fat buildup triggers inflammation, a condition called alcohol-induced hepatitis. Prolonged inflammation gradually damages liver tissue. The final stage is cirrhosis, where scar tissue has replaced so much healthy liver tissue that the organ begins to fail. Symptoms across these later stages include fatigue, fever, fluid buildup in the abdomen or ankles, jaundice (yellowing of the skin and eyes), appetite loss, and vomiting.
Heavy drinking thresholds differ by sex. For men, that means three or more drinks per day or 21 or more per week. For women, it’s two or more per day or 14 or more per week.
Cardiovascular Effects
Alcohol’s relationship with the heart is dose-dependent, and not in a forgiving way. Even modest regular drinking nudges blood pressure upward. People who average one drink per day have systolic blood pressure about 1.25 mm Hg higher than non-drinkers. At three drinks per day, that gap widens to nearly 5 mm Hg. Sustained high blood pressure is one of the strongest risk factors for heart attack and stroke.
In a single drinking session, one or two drinks generally don’t change blood pressure much. Three or more drinks, however, create a biphasic pattern: blood pressure dips for up to 12 hours, then rises above baseline for the next 12 to 24 hours. The good news is that cutting back works relatively quickly. Among people drinking six or more drinks per day who reduced their intake by roughly half, systolic blood pressure dropped by about 5.5 mm Hg and diastolic pressure by about 4 mm Hg.
Long-term heavy drinking also directly damages heart muscle. Years of excessive consumption can enlarge the left ventricle, thin its walls, and eventually weaken the heart’s pumping ability, a condition called alcoholic cardiomyopathy. Research from the American Heart Association links roughly 7 to 15 drinks per day over a 5 to 15 year period with measurable declines in heart function, though some studies have found changes in heart relaxation with as few as four drinks per week.
Cancer Risk
Alcohol is a confirmed carcinogen. The National Cancer Institute links it to six types of cancer: mouth and throat, voice box, esophagus, liver, breast, and colorectal. Risk rises with the amount consumed, and there is no threshold below which cancer risk is zero.
The primary mechanism involves acetaldehyde, the same toxic byproduct your liver produces while breaking down alcohol. Acetaldehyde damages DNA and proteins inside cells. When DNA repair mechanisms can’t keep pace with the damage, mutations accumulate, and cancer risk climbs. This process happens in any tissue that comes into direct contact with alcohol or acetaldehyde, which is why cancers of the mouth, throat, and esophagus are especially strongly linked to drinking.
Gut Health and Digestion
Alcohol disrupts the intestinal barrier, the thin lining that separates the contents of your gut from your bloodstream. In people who drink heavily, this barrier becomes more permeable, allowing bacterial toxins to leak into the blood and trigger inflammatory responses throughout the body. Alcohol also shifts the composition of gut bacteria, favoring populations that produce harmful metabolic byproducts like phenol, which is found at high levels in people with increased gut permeability and nearly absent in those with intact barriers.
The encouraging finding is that gut permeability can recover. In one study, after 19 days of abstinence, gut permeability in alcohol-dependent individuals dropped back to levels seen in healthy controls. The inflammatory pathways activated by leaked bacterial products also partially recovered within about three weeks of stopping drinking.
Brain Structure Over Time
Chronic heavy drinking physically shrinks the brain. The most vulnerable region is the prefrontal cortex, the area responsible for planning, impulse control, and decision-making, along with its connecting white matter. Pathology studies have measured a greater than 20% decrease in white matter volume among the heaviest drinkers (those consuming around 300 grams of ethanol per day, equivalent to roughly 20 standard drinks). Overall cortical atrophy of about 5% has been observed even at lower consumption levels.
These structural changes show up as specific functional deficits rather than a global decline. Heavy drinkers tend to struggle most with executive functions: planning, working memory, and behavioral flexibility. The pattern points to targeted damage in prefrontal regions rather than uniform deterioration across the whole brain.
Sleep Disruption
Alcohol may help you fall asleep faster, but it wrecks the quality of sleep you get. The main casualty is REM sleep, the deep, restorative phase associated with memory consolidation and emotional processing. Alcohol fragments your sleep cycle, causing brief awakenings throughout the night that send you back to lighter sleep stages. Each of these micro-awakenings chips away at total REM time. If you already have sleep apnea, alcohol makes it worse, further fragmenting sleep and reducing REM even more.
The result is waking up feeling unrested even after a full night in bed. Over time, chronically disrupted sleep contributes to problems with mood, concentration, immune function, and metabolic health.
How Much Is Considered Safe
The Dietary Guidelines for Americans, 2020-2025, recommend that men limit intake to two drinks or fewer per day and women to one drink or fewer per day, on days when alcohol is consumed. The scientific advisory committee behind those guidelines actually recommended a stricter limit of no more than one drink per day for both men and women, though the final published guidelines kept the two-drink limit for men.
These limits represent a ceiling, not a target. They apply only to adults of legal drinking age who choose to drink. They do not imply that drinking up to those amounts is beneficial. Given the cancer risk associated with any level of alcohol consumption, less is better from a purely health-focused perspective.
