For most adults, a fever becomes too high at 103°F (39.4°C), which is the threshold where major medical organizations recommend calling a doctor. At 104°F (40°C) and above, the situation is more urgent. And a temperature over 106.7°F (41.5°C) is a medical emergency that can damage organs and become life-threatening without immediate treatment.
But temperature alone doesn’t tell the whole story. What’s happening alongside the fever matters just as much, and in some cases matters more.
The Key Temperature Thresholds
Normal body temperature hovers around 98.6°F (37°C), though it fluctuates throughout the day. A reading of 100.4°F (38°C) or higher is generally considered a fever. From there, the risk increases in stages.
A low-grade fever, roughly 100.4°F to 102°F, is your immune system doing its job. It’s uncomfortable but rarely dangerous on its own. You can usually manage it at home with rest, fluids, and over-the-counter fever reducers if needed.
At 103°F (39.4°C), the Mayo Clinic recommends contacting your healthcare provider. This doesn’t necessarily mean rushing to the ER, but it does mean getting guidance, especially if the fever isn’t responding to medication or has lasted more than a couple of days.
At 104°F (40°C) and above, Harvard Health advises calling your doctor promptly. Fevers in this range are less likely to be a routine viral infection and more likely to signal something that needs medical attention.
The most dangerous threshold is 106.7°F (41.5°C), a condition called hyperpyrexia. At this level, your organs start struggling to function. Your brain, heart, lungs, liver, and kidneys can all be affected. Without rapid treatment, hyperpyrexia can cause brain swelling, permanent brain damage, coma, or death. Actual brain tissue damage, however, doesn’t typically occur until temperatures exceed 108°F (42°C). Reaching that extreme from an infection alone is rare, though it can happen with heat stroke or certain drug reactions.
Symptoms That Matter More Than the Number
A fever of 101°F with confusion is more dangerous than a fever of 103°F with mild body aches. Certain symptoms alongside any fever signal a need for immediate medical attention, regardless of what the thermometer reads:
- Confusion, strange behavior, or altered speech
- Seizures or convulsions
- Loss of consciousness
- Stiff neck, especially with pain when bending your head forward
- Difficulty breathing or chest pain
- Persistent vomiting
- Severe headache or unusual sensitivity to bright light
- Rash
- Pain when urinating
- Severe pain or swelling anywhere in the body
These can point to serious infections like meningitis, sepsis, or pneumonia. The combination of a stiff neck, fever, and light sensitivity is a classic warning sign of meningitis and warrants an emergency visit even if the fever itself seems moderate.
Lower Thresholds for Older Adults
If you’re over 65 or caring for someone who is, the standard fever thresholds can be misleading. Older adults, particularly those who are frail or living in care facilities, tend to have lower baseline body temperatures than 98.6°F. Their immune systems also mount weaker fever responses, which means a serious infection might produce only a modest temperature rise.
The CDC’s clinical guidelines for older adults in long-term care define a fever as a single oral reading above 100°F (37.8°C), repeated readings above 99°F (37.2°C), or any rise of more than 2°F above that person’s normal baseline. So for an elderly person who normally runs at 97°F, a reading of 99.5°F could be just as significant as 102°F in a younger adult. People with weakened immune systems from chemotherapy, organ transplants, or conditions like HIV face similar concerns: their fevers may stay deceptively low even during serious infections.
Getting an Accurate Reading
Where you take your temperature affects the number you get. Rectal thermometers give the most accurate core body temperature reading, but most adults use oral, ear, or forehead thermometers. Each method can read slightly differently, and there’s no reliable formula to convert between them. A forehead thermometer might read lower than an oral one taken at the same moment.
The most practical approach is to use the same method and the same thermometer each time. This lets you track whether your fever is rising or falling, which is more useful than any single reading. If you’re monitoring a fever at home, take your temperature at consistent intervals and note the time, since body temperature naturally runs higher in the late afternoon and evening.
Managing a Fever at Home
Most fevers below 103°F in otherwise healthy adults don’t require aggressive treatment. Fever is a defense mechanism, and letting it run its course can help your body fight infection. That said, if you’re miserable, there’s no harm in bringing the temperature down for comfort.
Acetaminophen and ibuprofen are both effective fever reducers. The key safety limit for acetaminophen is 4,000 milligrams (4 grams) in a 24-hour period. Going over that threshold risks liver damage, and it’s easier to exceed than you might think if you’re also taking combination cold or flu products that contain acetaminophen. Check the labels on everything you’re taking.
Stay hydrated. Fever increases fluid loss through sweating and faster breathing, and dehydration can make you feel significantly worse. Water, broth, and electrolyte drinks all help. Dress in light layers rather than bundling up, and keep the room at a comfortable temperature. Ice baths and alcohol rubs are outdated approaches that can actually cause shivering, which raises your core temperature further.
If your fever reaches 103°F, persists for more than two or three days, or keeps returning after seeming to break, that pattern is worth a call to your doctor. A fever that climbs to 104°F or higher warrants more urgent attention, even if you otherwise feel “okay.” And any fever paired with the red-flag symptoms listed above means seeking care right away.
