What Foods to Avoid With Hypothyroidism: Soy, Gluten & More

Most foods are perfectly fine to eat with hypothyroidism, but a handful can interfere with how your thyroid functions or how well your body absorbs thyroid medication. The good news: very few foods need to be eliminated entirely. For most people, it’s more about timing, preparation, and moderation than strict avoidance.

Cruciferous Vegetables: Cook Them, Don’t Cut Them

Broccoli, cauliflower, Brussels sprouts, kale, and cabbage contain natural compounds called goitrogens that can interfere with how your thyroid uses iodine. Since iodine is the raw material your thyroid needs to produce hormones, anything that blocks its uptake sounds alarming. In practice, though, you would need to eat an excessive and unrealistic amount of raw cruciferous vegetables for them to actually disrupt hormone production, according to Northwestern Medicine.

The more practical approach is to cook these vegetables rather than avoid them. Steaming, boiling, or fermenting cruciferous vegetables reduces their goitrogenic activity. A serving of roasted broccoli or sautéed kale with dinner is not going to tank your thyroid levels. These vegetables are packed with fiber, vitamins, and cancer-fighting compounds, so removing them from your diet could do more harm than good. If you’re eating large quantities of raw kale in daily smoothies and your levels aren’t well controlled, switching to cooked greens is a reasonable change to discuss with your provider.

Soy Products and Thyroid Medication

Soy has a complicated reputation in the thyroid world, but the actual evidence is thinner than most people assume. A few case reports over the years suggested that soy products might interfere with levothyroxine absorption. However, the only prospective randomized crossover study on the topic found no difference in medication absorption when levothyroxine and soy were taken at the same time. A separate double-blind, placebo-controlled trial on liquid formulations reached similar conclusions.

A narrative review published in Endocrine Practice concluded that any interference from soy, if it exists at all, appears to have little clinical impact. That said, if you eat a lot of soy (tofu, edamame, soy milk, tempeh) and your thyroid levels have been difficult to stabilize, it’s worth separating your soy intake from your medication by a few hours. There’s no strong reason to eliminate soy entirely.

Millet: The Overlooked Grain

While most people focus on broccoli and soy, millet is a genuinely concerning food for people with hypothyroidism, and it rarely gets the attention it deserves. Millet contains a class of compounds called C-glycosylflavones that can directly interfere with thyroid function. Epidemiological data from Sudan, where pearl millet is a dietary staple, shows that millet consumption plays a role in goiter development beyond what iodine deficiency alone would explain. Animal studies have confirmed that millet consumption modifies thyroid hormone patterns and can enlarge the gland.

This matters most if millet is a regular part of your diet. Occasional millet in a multigrain bread is unlikely to cause problems, but if you’ve switched to millet as a primary grain (some people do for gluten-free eating), it’s worth reconsidering that choice. Other gluten-free grains like rice, quinoa, and oats don’t carry the same thyroid risks.

Gluten and Hashimoto’s Thyroiditis

If your hypothyroidism is caused by Hashimoto’s, an autoimmune condition where the immune system attacks the thyroid, gluten deserves special consideration. There’s an established overlap between Hashimoto’s and celiac disease, and some people with Hashimoto’s who don’t have celiac still report feeling better on a gluten-free diet.

A systematic review and meta-analysis published in Nutrients looked at gluten-free diets in Hashimoto’s patients without celiac disease. The results were mixed: going gluten-free significantly reduced anti-thyroglobulin antibodies (one of two key markers of autoimmune thyroid attack), but it actually increased anti-thyroid peroxidase antibodies (the other marker). The researchers noted the overall evidence quality was very uncertain. This means a gluten-free diet might help some aspects of autoimmune activity while not improving others. It’s not a clear win, and it’s definitely not necessary for everyone with hypothyroidism. If you don’t have Hashimoto’s, there’s little reason to avoid gluten for thyroid purposes.

Highly Processed Foods and Added Sugar

A diet high in refined sugar, processed meats, and saturated fats does not directly suppress thyroid hormone production the way goitrogens do. But it creates a background of chronic inflammation that can worsen autoimmune thyroid conditions and amplify symptoms you’re already dealing with, like fatigue, weight gain, and brain fog.

Research published in Frontiers in Endocrinology describes how the typical Western diet, high in refined sugars, salt, and processed foods while low in vegetables, fruits, and fish, triggers the release of inflammatory molecules throughout the body. This chronic low-grade inflammation can disturb immune balance, promote autoimmune progression, and increase oxidative stress. For someone whose thyroid is already under attack from an autoimmune process, piling on inflammatory dietary patterns makes the situation harder to manage.

You don’t need to follow a rigid anti-inflammatory protocol. Reducing sugary drinks, cutting back on ultra-processed snack foods, and eating more whole foods will lower your inflammatory baseline over time. These changes also tend to help with the weight management challenges that come with a sluggish thyroid.

Foods That Interfere With Medication Absorption

If you take levothyroxine (the standard thyroid hormone replacement), what you eat around your medication matters more than your overall diet. Several common foods and supplements can reduce how much medication your body actually absorbs.

  • Coffee: Traditionally, patients have been told to wait 30 to 60 minutes after taking levothyroxine before drinking coffee. Research from the Endocrine Society suggests that liquid formulations of the medication may not be affected by coffee, but if you take the standard tablet form, keeping that buffer is still a good idea.
  • Calcium-rich foods: Dairy products, fortified orange juice, and calcium supplements can bind to levothyroxine in your gut and reduce absorption. Separate these from your medication by several hours.
  • Iron-rich foods and supplements: Iron competes with levothyroxine for absorption in a similar way to calcium. If you take an iron supplement, take it at least a few hours apart from your thyroid medication.
  • High-fiber meals: Fiber can also slow or reduce levothyroxine absorption. This doesn’t mean you should avoid fiber, which is essential for the constipation many hypothyroid patients experience. Just don’t pair a very high-fiber breakfast with your morning dose.

The simplest approach is to take your medication first thing in the morning on an empty stomach, then wait at least 30 to 60 minutes before eating or drinking anything other than water. This single habit eliminates most food-medication interactions.

Nutrients That Support Thyroid Function

While thinking about what to limit, it’s worth making sure you’re getting enough of the nutrients your thyroid depends on. Selenium is a key player in thyroid hormone metabolism, and deficiency can worsen hypothyroidism. The recommended daily intake for adults is 55 mcg, easily met through one or two Brazil nuts, seafood, or eggs. The upper safe limit is 400 mcg per day, so don’t overdo it with supplements. Selenium deficiency combined with iodine deficiency is particularly harmful and has been linked to severe hypothyroidism in regions like Central Africa.

Iodine is equally important since it’s the core building block of thyroid hormones. Most people in developed countries get enough through iodized salt, dairy, and seafood. Taking extra iodine without guidance can actually make thyroid problems worse, especially with Hashimoto’s, so more is not better here.

The overall pattern matters more than any single food. A diet built around whole grains (other than millet), cooked vegetables, lean protein, and adequate selenium and iodine gives your thyroid the best foundation to work with, whether you’re on medication or not.
