Age spots can be faded and sometimes eliminated entirely, but the approach that works best depends on how quickly you want results and how much you’re willing to invest. Over-the-counter brightening products take weeks to months for modest improvement. Prescription creams work faster and more noticeably. Laser and light-based procedures can clear individual spots in one to two sessions. Here’s what actually works, ranked from least to most aggressive.
Why Age Spots Form in the First Place
Age spots are flat, brown or tan patches where melanin has accumulated in the top layer of skin. Years of sun exposure triggers an inflammatory response that ramps up the skin’s pigment production. Specifically, the skin produces signaling molecules that tell pigment-producing cells to go into overdrive, and a key receptor in the affected skin gets expressed at nearly seven times its normal level. That’s why age spots cluster on the hands, face, shoulders, and forearms: the areas that have absorbed the most cumulative UV over your lifetime.
The spots themselves are harmless. But because some early skin cancers can mimic the look of an age spot, any spot that changes shape, develops an irregular border, darkens unevenly, or appears alongside unusual hair repigmentation in older adults is worth having a dermatologist examine. Distinguishing a benign spot from something more concerning sometimes requires magnified imaging that can’t be done at home.
Over-the-Counter Brightening Products
The most accessible starting point is a serum or cream containing ingredients that interrupt melanin production. Three of the most studied brightening agents are tranexamic acid, kojic acid, and niacinamide. In a clinical evaluation of a serum combining all three, users saw a 60% improvement in hyperpigmentation and roughly 54% improvement in overall skin tone after 12 weeks. Over 90% of participants were satisfied with the results at every check-in point.
Other common ingredients include vitamin C (ascorbic acid), azelaic acid, and alpha arbutin. These work by slowing the enzyme responsible for melanin production or by speeding up the turnover of pigmented skin cells. The tradeoff is patience: most nonprescription products require several weeks to months of consistent daily use before spots visibly lighten. You’ll get the most out of them by applying the product to clean skin and following it with sunscreen every morning.
Prescription Creams
When OTC products aren’t cutting it, prescription-strength topicals offer a significant step up. The two most commonly prescribed options are hydroquinone and tretinoin, used alone or in combination.
Hydroquinone is a depigmenting agent available in concentrations from 2% (some OTC formulations) up to 10% by prescription. It works by suppressing the enzyme that produces melanin. Tretinoin, a retinoid, accelerates skin cell turnover so pigmented cells shed faster. When the two are combined, sometimes with a mild steroid to reduce irritation, spots gradually fade over several months. In studies of similar retinoid-based treatments, the median time to see at least one shade of improvement was about 12 weeks on the face and closer to 14 weeks on the forearms.
Prescription creams can cause redness, peeling, and increased sun sensitivity, especially in the first few weeks. Using them at night and applying sunscreen during the day minimizes these side effects.
Chemical Peels
Chemical peels use controlled acid solutions to remove the outermost layers of skin, taking accumulated pigment with them. The two most common acids for pigmentation are glycolic acid (typically 35% to 70% in a clinical setting) and trichloroacetic acid, or TCA (10% to 20%). These are effective for surface-level pigmentation like age spots.
Lighter peels cause redness that fades within a few days. Medium-depth peels can leave the skin red for several weeks. You’ll likely need more than one session before you notice meaningful fading, and the total course of treatment stretches over a few months. Peels work best on lighter skin tones; darker skin carries a higher risk of post-inflammatory pigment changes that can temporarily make discoloration worse before it gets better.
Laser and Light-Based Treatments
For the fastest, most dramatic results, lasers and intense pulsed light (IPL) target pigment directly. These devices deliver concentrated energy that breaks apart melanin clusters, which the body then clears naturally.
Q-switched lasers are among the most precise options. In a head-to-head trial comparing a Q-switched alexandrite laser to IPL for treating age spots in Asian patients, a single laser session produced results comparable to two IPL sessions spaced four weeks apart. Both treatments led to statistically significant improvement. However, the laser carried a higher risk of post-inflammatory darkening in patients prone to it, while IPL produced better outcomes in that subgroup specifically because it was gentler.
The practical takeaway: if you have lighter skin and want the quickest fix, a Q-switched laser can often clear a spot in one visit. If you have a darker complexion or are prone to pigment reactions, IPL spread over two or three sessions may be safer. Either way, treated spots typically crust lightly and heal within a week or two.
Cryotherapy
Cryotherapy involves applying liquid nitrogen to freeze individual spots. It’s fast, usually done in a dermatologist’s office without anesthesia, and works well for isolated spots on the hands or face.
After treatment, the area turns red and may blister and swell. Healing takes about 7 to 10 days, during which you should avoid scented soap, makeup, or lotion on the treated skin for at least 10 days. The frozen, pigmented skin peels away as it heals.
The main risk is discoloration at the treatment site. Pinkness, redness, or lighter or darker skin can persist for up to a year after the procedure, and in some cases the color change is permanent. This makes cryotherapy a better fit for people with fair skin, where any residual lightening is less noticeable.
Microdermabrasion
Microdermabrasion uses fine crystals or a diamond-tipped wand to buff away the surface layer of skin. It’s the gentlest in-office procedure, with no downtime, but it also produces the most modest results. You’ll need a series of sessions spread over months, and the improvement is often temporary. It works best as a complement to topical treatments rather than a standalone fix.
How to Keep Spots From Coming Back
Every treatment for age spots is undermined if you skip sun protection afterward. The same UV exposure that caused the spots in the first place will trigger new melanin production in the freshly treated skin, and spots can return darker than before.
Use a broad-spectrum sunscreen with at least SPF 30 every day you’ll be outdoors. Reapply every two hours in direct sun. If you’re swimming, reapply within 45 minutes to an hour, and again after toweling off. Sweating from exercise or yard work also dilutes sunscreen and may require reapplication within an hour. A higher SPF number offers more protection per application but does not last longer on the skin, so the reapplication schedule stays the same regardless of SPF.
