Some bleeding after a tooth extraction is completely normal and can last up to 24 hours. But if you’re still dealing with steady or heavy bleeding beyond that window, or if blood is pooling rapidly in your mouth within the first few hours despite applying pressure, something is preventing a stable clot from forming. The good news: most post-extraction bleeding can be controlled at home with the right technique. The key is knowing what to do first, what’s actually going wrong, and when the situation calls for professional help.
What Normal Bleeding Looks Like
After an extraction, your body immediately starts forming a blood clot in the empty socket. This clot is the foundation for healing. During the first several hours, you’ll notice pink or reddish saliva, and the socket may ooze lightly. This is expected. The oozing typically tapers off within a few hours if the clot stays intact, though trace amounts of blood-tinged saliva can appear for up to 24 hours.
What’s not normal: bright red blood that fills your mouth, bleeding that requires you to spit or swallow frequently, or blood that soaks through gauze within minutes. If you’re changing gauze pads repeatedly and they’re saturated each time, the clot either hasn’t formed properly or has been disturbed.
How to Stop the Bleeding at Home
The most effective first step is direct, sustained pressure. Fold a piece of clean gauze into a small, thick pad and place it directly over the extraction site. Bite down with firm, steady pressure and hold it for at least 20 to 30 minutes without checking on it. Peeking at the wound resets the clock. If bleeding is heavy, increase to firm pressure for a full hour. If it continues after that, apply a fresh gauze pad for another 30 minutes.
A moistened black tea bag works as a surprisingly effective alternative to gauze. Black tea contains tannins that promote blood clotting by constricting blood vessels and open capillaries. Place the damp tea bag directly on the socket and bite down with the same steady pressure for 20 to 30 minutes. The tannins may also help reduce bacteria at the wound site.
While you’re managing the bleeding, keep your head elevated, even while sleeping. Lying flat increases blood flow to the head and can restart or worsen bleeding.
What Disrupts the Blood Clot
The clot that forms in your socket is fragile, especially in the first 48 to 72 hours. Several common actions can dislodge it and restart bleeding or lead to a painful condition called dry socket.
- Drinking through a straw. The suction pulls directly on the clot. Avoid straws for at least a week after extraction.
- Smoking or using tobacco. The inhaling motion creates suction similar to a straw, and chemicals in tobacco impair blood flow to the healing tissue.
- Vigorous rinsing or spitting. Swishing mouthwash or water too forcefully can knock the clot loose. If you need to rinse, let liquid gently fall out of your mouth.
- Hard, crunchy, or chewy foods. These can physically dislodge the clot. Stick to soft foods during recovery.
- Warm or carbonated drinks. Heat dilates blood vessels and increases bleeding. Carbonation can also disturb the wound.
- Physical exertion. Heavy exercise raises blood pressure and can restart bleeding. Take it easy for the first couple of days.
Medications That Make Bleeding Worse
If your bleeding is harder to control than expected, your medications may be a factor. Blood thinners and antiplatelet drugs are among the most common reasons post-extraction bleeding lingers. These include warfarin (Coumadin), apixaban (Eliquis), rivaroxaban (Xarelto), dabigatran (Pradaxa), clopidogrel (Plavix), prasugrel (Effient), and aspirin.
These medications work by slowing your body’s ability to form clots, which is exactly what you need to happen in that socket. If you take any of these, your dentist should already know, and they may have placed special materials in the socket during the procedure to help with clotting. Never stop taking a prescribed blood thinner without your doctor’s guidance, even if you’re bleeding. The risks of stopping the medication are typically more serious than the dental bleeding itself.
Underlying Conditions That Affect Clotting
For some people, persistent post-extraction bleeding is the first sign of an undiagnosed bleeding disorder. Von Willebrand disease, the most common inherited bleeding condition, involves a deficiency in a protein that helps platelets stick together and attach to damaged blood vessel walls. Without enough of this protein, the initial clot at the extraction site fails to form properly.
Hemophilia A and B, caused by deficiencies in specific clotting factors, create a different problem. The initial platelet plug may form, but the clot can’t stabilize and strengthen the way it should, leading to delayed or prolonged bleeding. Liver disease can also impair clotting because the liver produces most of the proteins your blood needs to coagulate. If you notice that you bleed easily from minor cuts, bruise without clear cause, or have heavy menstrual periods alongside difficult post-extraction bleeding, mention this pattern to your dentist or doctor.
What a “Liver Clot” Means
Sometimes a clot does form in the socket, but it’s the wrong kind. A liver clot (also called a currant jelly clot) is a red, jelly-like mass that looks different from the firm, dark clot you want to see. It’s rich in red blood cells rather than the fibrin and platelets needed for proper healing. This type of clot typically results from slow venous bleeding that fills the socket without forming a stable structure.
A liver clot is problematic because it’s fragile and easily dislodged. Saliva enzymes can break it down before it has a chance to organize and stabilize, which restarts bleeding. If you notice a soft, gelatinous, bright red mass bulging from the socket, this is likely what’s happening. It usually needs to be removed by your dentist so a proper clot can form in its place.
What Your Dentist Can Do
When home measures fail, your dentist has several tools to control the bleeding. The most straightforward approach is placing a resorbable gelatin sponge into the socket combined with sutures to hold everything in place. Research on patients taking blood thinners found that this combination alone was sufficient to manage bleeding risk in the vast majority of cases, with only about 9% experiencing any post-operative bleeding.
Other options include oxidized cellulose packing (a material that promotes clotting on contact), calcium-based compounds placed directly in the socket, and in some cases a special adhesive or medicated rinse that stabilizes the clot from the surface. Your dentist may also identify the specific vessel that’s bleeding and address it directly with cauterization or a targeted suture.
Signs You Need Immediate Help
Most post-extraction bleeding, even when it feels alarming, resolves with proper pressure and patience. But certain signs indicate you should go to an emergency room rather than wait for a dental office to open.
Seek emergency care if you feel dizzy, lightheaded, or faint, which can signal significant blood loss. A rapid heartbeat, feeling cold or clammy, or difficulty breathing alongside oral bleeding are serious warning signs. From a volume standpoint, emergency protocols define severe oral bleeding as losing more than 200 mL (roughly a cup) in a single hour. If blood is flowing freely and you cannot slow it with firm gauze pressure sustained for a full hour, don’t keep waiting at home.
Difficulty breathing or a sensation that blood is pooling in the back of your throat, especially if you’re lying down, also warrants immediate attention. Blood that enters the airway is a more urgent concern than the bleeding itself.
