Untreated dry eye can progress from a mild annoyance to a condition that damages your cornea, disrupts your nerve signaling, and affects your mental health. Most people with dry eye experience it as intermittent irritation, but when the underlying problem goes unaddressed for months or years, the consequences compound. The surface of your eye depends on a stable tear film to stay healthy, and without it, a chain of events begins that becomes harder to reverse over time.
Surface Damage and Corneal Scarring
Your tear film does more than keep your eyes comfortable. It washes away debris, delivers oxygen and nutrients to the cornea, and maintains a smooth optical surface for clear vision. When tears are insufficient or evaporate too quickly, particles stay on the eye’s surface longer, creating tiny scratches called corneal abrasions. These micro-injuries are painful and can blur your vision temporarily, but they usually heal within a few days if the tear film is restored.
The problem is that without treatment, these abrasions keep recurring. Repeated damage to the corneal surface can lead to corneal ulcers, which are open sores that penetrate deeper layers of the cornea. Ulcers heal with scar tissue, and scar tissue on the cornea is not transparent. The Mayo Clinic notes that severe untreated dry eye can lead to corneal surface abrasion, corneal ulcers, and vision loss. Once scarring forms, it is permanent. The only way to restore clear vision through a scarred cornea is a corneal transplant.
The Inflammation Feedback Loop
Dry eye is fundamentally an inflammatory condition, and this is what makes it self-perpetuating. When the tear film breaks down, the exposed surface cells become inflamed. That inflammation damages the glands responsible for producing tears and the oily layer that prevents tears from evaporating. With fewer functional glands, tear quality drops further, exposing more surface cells, which triggers more inflammation.
Research into this cycle has revealed how it works at a cellular level. In the tear-producing glands, sustained inflammation activates a specific damage pathway that promotes cell death while simultaneously impairing the gland’s ability to regenerate. Changes in how these gland cells process fats compound the problem: cholesterol accumulates inside the cells, causing toxic damage that keeps the inflammatory cycle running. This means that the longer dry eye goes untreated, the harder it becomes for your body to recover normal tear production on its own. Early-stage dry eye responds well to simple interventions. Late-stage dry eye, after the glands have atrophied, is far more difficult to manage.
Chronic Pain That Outlasts the Cause
One of the most significant and least understood consequences of untreated dry eye is what happens to the nerves in your cornea. The cornea is one of the most densely innervated tissues in your body, and prolonged inflammation doesn’t just irritate those nerves. It changes them.
When corneal nerves are repeatedly damaged, they attempt to regrow, but the regeneration is often disorganized. Nerve endings become hypersensitive, firing in response to stimuli that wouldn’t normally register as painful, like a light breeze or air conditioning. The firing threshold of these nerves drops, meaning they send pain signals more easily and more often. At the tips of damaged nerve fibers, small tangles called neuromas can form, which sustain inflammation on their own and perpetuate the cycle.
Over time, this process rewires not just the nerves in the eye but the pain-processing pathways deeper in the nervous system. The amplification of pain signaling can become permanent, persisting even after the original surface damage has healed. This condition is called neuropathic ocular pain, and people who develop it experience burning, stinging, or aching in their eyes that no longer correlates with the visible condition of their eye surface. It is notoriously difficult to treat because the problem has shifted from the eye’s surface to the nervous system itself.
Impact on Mental Health
Dry eye is a chronic condition that affects something you use every waking moment: your vision and the comfort of your eyes. The toll this takes on mental health is substantial and well documented. In a large population-based study of over 7,000 people with dry eye disease, those with the condition were nearly three times as likely to experience anxiety (odds ratio 2.8) and depression (odds ratio 2.9) compared to people without it. A separate study found that people with dry eye were 2.5 times more likely to report severe psychological stress.
These aren’t just statistics about people who happen to have both conditions. Chronic eye discomfort interferes with reading, screen use, driving, and sleep. It limits the activities that give people purpose and connection. When dry eye progresses to the point of neuropathic pain, the constant discomfort can be genuinely debilitating, and because the eye often looks normal on examination, people sometimes struggle to have their suffering recognized by others or even by their doctors.
Limitations on Future Eye Procedures
Untreated dry eye can also close doors for elective procedures you might want later. LASIK, the most common vision correction surgery, works by reshaping the cornea, and the procedure temporarily disrupts the nerves that regulate tear production. For someone with healthy eyes, this causes a few weeks of dryness that resolves on its own. For someone with pre-existing untreated dry eye, it can trigger prolonged discomfort, delayed healing, and visual outcomes that fall short of expectations.
Severe or persistent dry eye can disqualify you from LASIK entirely. Alternative procedures exist, but they tend to be more invasive or expensive. Getting dry eye under control before it progresses keeps more options on the table.
What Infection Risk Actually Looks Like
Many sources list eye infections as a major risk of untreated dry eye, and the logic seems straightforward: a damaged surface should be easier for bacteria to invade. But the evidence is more nuanced than that. A review of the research found that convincing evidence linking dry eye to infectious keratitis (a serious corneal infection) is actually minimal. People with dry eye appear to mount a stronger innate immune response at the eye’s surface, which may compensate for the disrupted barrier.
That said, this doesn’t mean infection risk is zero. Corneal ulcers from severe dry eye can become infected, and any break in the surface is a potential entry point. The takeaway is that infection is a possible complication rather than an inevitable one, and the more pressing concerns are the inflammatory damage, nerve changes, and scarring described above.
How Severity Progresses
Dry eye isn’t a single condition with one trajectory. It progresses through stages, and where you are on that spectrum determines what’s at stake. Clinicians assess severity partly through tear film osmolarity, a measure of how concentrated the salts in your tears have become. Normal tear osmolarity averages around 302 mOsm/L. In mild to moderate dry eye, it rises to about 315. In severe cases, it reaches 336 or higher, reflecting a tear film that has become significantly more concentrated and irritating to the eye surface.
The glands along your eyelid margins that produce the oily component of tears (meibomian glands) also show a clear progression. Early on, their secretions thicken. In moderate disease, the gland openings become plugged and the lid margins show increased blood vessel growth, a sign of chronic inflammation. In severe disease, the glands themselves begin to drop out, meaning they physically disappear and cannot regenerate. This is why early treatment matters so much. You can unplug a clogged gland. You cannot bring back one that no longer exists.
